The effects of number of operations, experience of the surgeon, and type of hospital on operative mortality have been studied in 444 patients treated for abdominal aortic aneurysms. In the elective group (n = 279) there was a significant difference in mortality between hospitals in which more than 10 such operations were done compared with those in which less than 10 were done during the study period (p = 0.05; odds ratio (OR) 2.7). In the ruptured group there was no statistically significant difference (p = 0.14; OR 1.9). In the elective group, units with vascular surgical experience had an operative mortality of 4.8% compared with 11.3% for other units (p = 0.05; OR 2.6). In the ruptured group the figures were 52.5% and 73.3% respectively (p = 0.03; OR 2.5). There was no difference in operative mortality between university, county and local hospitals. Outcome of treatment after operations for abdominal aortic aneurysm was related to number of operations carried out and experience, whereas the type of hospital seemed less important.

The colectomy rate in patients with ulcerative colitis has increased during the last 30 years in Stockholm County due to increased incidence and a more liberal attitude to colectomy in acute cases. In an area of Stockholm's size, about 30 patients with UC will require colectomy every year and of these, 12 will be acute cases. The main factor affecting the colectomy rate was the extent of disease at diagnosis. The 25-year cumulative colectomy rate was 65% in patients with total colitis compared to 45% in all patients. The probability of undergoing a colectomy decreased as the duration of disease increased and after five years duration of disease the colectomy rate was about 1% per year. The choice of primary surgical procedure has changed during the study period and subtotal colectomy and ileostomy is now preferred as the surgical procedure of choice, both in acute and elective cases. The frequency of major complications was constant during the period of study but increased with the urgency of intervention, being 25% in elective cases and 46% in acute cases. The overall postoperative mortality fell from 13% during 1960 to 1964 to 2% during 1980-1984. In acute cases the mortality during the same time periods decreased from 36% to 3%. The long-term results of ileorectal anastomosis showed a substantial failure rate and the most common cause of failure was recurrent rectal disease. In patients who still had their ileorectal anastomosis at the time of follow-up the functional results were better than for the pelvic pouch. The ileorectal anastomosis is for some patients a definite solution and in others it avoids or delays an ileostomy and at the same time still permits a pelvic pouch at a later date. The need for secondary surgery after colectomy for UC is great. The cumulative probability of a fist small intestinal obstruction requiring surgery in 483 patients treated by colectomy was 23% after 15 years. The annual incidence was highest in patients treated by pelvic pouch and lowest in patients treated by IRA. The cumulative probability of a first ileostomy revision was 21% after 15 years. In patients with a Kock's pouch the cumulative probability of a first nipple revision was as high as 52% after two years. The need for secondary surgery was initially high in patients treated by pelvic pouch but the need has decreased during the latter years due to improved technique and increasing surgical experience.(ABSTRACT TRUNCATED AT 400 WORDS)

Surgical treatment of ulcerative colitis in Stockholm county.

The effectiveness of superoxide dismutase (SOD), catalase (CAT), dimethyl sulphoxide (DMSO) and allopurinol in prevention of peritoneal adhesion formation induced by complete vascular obstruction and reperfusion of an ileal segment was investigated in rats. The ischaemic period was 30 min. Group A (n = 20) were controls, group B (n = 15) received SOD 15,000 U/kg i.v. and group C (n = 17) the same dose of CAT immediately before induction of ischaemia. In group D (n = 20) DMSO 20 mg/kg was given i.v. 5 min before ischaemia, and group E (n = 20) received allopurinol orally 50 mg/kg daily for 2 days and also 2 hours before ischaemia. Ten days later adhesions had developed in 80% of group A, 40% of group B, 47% of group C and 45% of groups D and E (p less than 0.05). The severity of the adhesions was significantly less in the pretreated groups than in the controls. Oxygen-derived free radicals may be pathogenetically important for such adhesion formation. Xanthine oxidase is the principal source of oxygen radicals after a 30-min period of complete regional intestinal ischaemia.

Homologous, or banked, blood is chronically in short supply. In addition, it is associated with a number of significant risks, including transfusion-related infection with hepatitis and acquired immune deficiency syndrome (AIDS), transfusion reactions, and alloimmunization. Therefore, the benefits of reducing the dependence on banked blood are clear. Several techniques are being used to transfuse the patient's own blood during surgery. With the predeposit technique, a patient anticipating surgery donates blood during the month preceding operation. During or immediately following surgery, the patient's blood can be collected and reintroduced (whole blood salvage and reinfusion) or it can be collected, washed, and reinfused in the form of packed cells. Expanded use of these techniques and efforts to reduce blood loss during surgery can reduce the incidence of transfusion-related complications. In the future, synthetic blood substitutes may further reduce the need for homologous transfusions in surgical patients.
