The purpose of the current study is to determine which variables influence the onset of chronic regional pain syndrome (CRPS) following Sanders type 1 calcaneal fractures, which are treated conservatively, and to discover how Vitamin C (VC) supplementation, which is often mentioned in the literature, affects the outcomes of these fractures. The study incorporated individuals who had a Sanders type 1 fracture that was both closed and non-displaced and were treated with conventional plaster. The current study retrospectively reviewed medical records to gather demographic data, duration of immobilization, smoking habits, administration of VC supplementation, and presence of diabetes mellitus (DM). The Budapest criteria were employed to make the clinical diagnosis of CRPS. The study cohort comprised 258 (75.9%) males and 82 (24.1%) females aged between 18 and 90 years who had sustained Sanders type 1 calcaneal fractures and were managed non-operatively. CRPS was detected in 42 (12.4%) of the 340 subjects. CRPS patients had a statistically higher immobilization duration than those without CRPS (p&lt;0.05). Smoking and DM were more common among patients with CRPS (p&lt;0.05) and CRPS patients had a much lower rate of VC consumption (4.8%) (p&lt;0.05). The current study is the first to examine risk variables for CRPS after calcaneal fractures. Long periods of immobility, female gender, and lack of VC supplementation increase the risk of CRPS. CRPS also increases with diabetes and smoking.

Chronic regional pain syndrome following calcaneal fractures: what causes it and how may Vitamin C aid?

This study aims to analyze the improvement and prognosis of anxiety and depression in patients with knee osteoarthritis after total knee arthroplasty. A retrospective questionnaire survey was conducted on 180 patients with knee osteoarthritis who underwent total knee arthroplasty. The questionnaire consisted of a general information questionnaire, the Zung Self-Rating Anxiety Scale (SAS) for anxiety, the Zung Self-Rating Depression Scale (SDS) for depression, the Visual Analog Scale (VAS), the Pittsburgh sleep quality Index (PSQI), and the American Knee Society Scores (AKSS). The same questionnaires were administered preoperatively and at 3, 6 and 12 months postoperatively, and the results were statistically analyzed. The prevalence of anxiety in preoperative knee osteoarthritis patients was 29.2% and the prevalence of depression was 37.5%. At 3, 6, and 12 months after total knee arthroplasty, anxiety and depression, pain levels, sleep disturbances, and functional status of the knee were significantly improved in patients with knee osteoarthritis compared with the preoperative period (all P&lt;0.05). Patients with knee osteoarthritis have significant anxiety and depression before surgery, and total knee arthroplasty can significantly improve their anxiety, depression and prognosis, and should be performed as early as possible in patients with knee osteoarthritis.

Improvement and prognosis of anxiety and depression after total knee arthroplasty

Fatty infiltration is a degenerative condition of the tendon-muscle unit of rotator cuff muscles, characterized by fatty accumulation within and around the muscles. This study compares a 3D method to the Goutallier classification for evaluating fatty infiltration of the rotator cuff muscles. Employing a retrospective study design, four researchers performed 3D segmentation of the rotator cuff muscles on 65 CT scans of patients with rotator cuff arthropathy. The Goutallier classification was graded on screenshots of the Y view and two axial slices. Measurements, including HU, volume, cross-sectional area, and Goutallier grade, were performed on the 3D segmentations and the respective 2D slices. The inter- and intra-rater variability for 2D and 3D methods were calculated using the Intraclass Correlation Coefficient (ICC). The ICC for 2D and 3D methods was excellent (ICC: 0.90-0.95 and 0.81-0.99, respectively) and good for the Goutallier classification (ICC: 0.62-0.81). Overall, the relative 3D fatty infiltration was 0% for Goutallier grade 0 muscles, 7% for grade 1, 19% for grade 2, 33% for grade 3, and 37% for grade 4. The relative 2D fatty infiltration was 0%, 8%, 25%, 37%, and 43%, respectively. We conclude that 3D segmentation on CT scans is better reproducible and evaluates the muscle entirely. However, the time-intensive nature of the 3D method currently limits its clinical practicality. Quantitive 2D evaluation is excellently reproducible but may overestimate the actual fatty infiltration percentage of the whole muscle.

3D quantitative CT study to assess rotator cuff muscle fatty infiltration

Introduction: A meta-analysis was conducted comparing the impact of Arthroscopic debridement (AD), trapeziectomy (TRAP), and joint replacement (JR) on the change in pain scores on patients with Basilar thumb joint arthritis (BTJA). Methods: Four databases were searched for studies presenting pain outcomes following surgical intervention for BTJA. Pain scores were reported using the Visual Analog Scale (VAS) and compared against the pre-established threshold for Minimal Clinically Important Difference (MCID) of 1.65. Results: Eighteen studies with 763 patients treated with AD(n=102, 13%), TRAP(n=428, 56%), and JR(n=233, 31%) between 2010 and 2023 with a mean follow-up period of 38 ± 28 months were included. There were 25 groups including 4 AD, 14 TRAP, and 7 JR. The mean difference between pre- and post-operative VAS pain was 4.9 ± 2 for all groups. Meta-analysis demonstrated a mean delta VAS of 3.6 (95%CI 1.79-5.38, for AD, 5.1(95%CI, 4.20-6.02) for TRAP and 6.8(95%CI, 5.93-7.97) for JR. ANOVA showed a significant difference between groups (P=0.016). Post-Hoc testing showed a significant difference between AD and JR (P=0.014). A significant improvement in pain scores, surpassing the MCID threshold, was obtained in all surgical interventions. Change in pain score was 2.6 times MCID for AD, 2.9 times for TRAP, and 3.6 times for JR. Conclusions: All interventions showed significant improvement in pain. Variability in treatment options and improvement depends on patient selection and surgeon’s preference. This data can be used to counsel patients regarding the expected pain relief. However, longevity, and long-term outcomes warrant further study.

Clinical Significance of Arthroscopic Debridement, Trapeziectomy, and Joint Replacement for Basilar Thumb Joint Arthritis: A Meta-analysis of Pain Score Improvements

The Ilizarov technique is a well-known procedure for limb deformity corrections. However, in the present study, the purpose was to examine the potential of wound closure by means of an Ilizarov frame. Two main cases are presented, a further seven were retrospectively reviewed. The first case experienced a chronic wound at the tibial tuberosity. A fistula was excised followed by antibiotic therapy, however, dehiscence at the wound occurred for which an Ilizarov procedure was used. After two weeks the frame was removed and the wound was closed. The second patient underwent osteosynthesis of a tibia and fibula fracture but was complicated by infection. An Ilizarov device was applied for bone healing as well as the skin defect. Nine patients were included in total. Four of them attained enough skin length via the Ilizarov procedure for secondary closure. Three had the frame removed before having full wound covering and needed further granulation of the wound. Finally, two more patients underwent graft reconstruction. Three patients suffered from infectious complications. The gold standard in soft tissue closure remains skin or flap reconstruction, however, this is not advisable in poor overall health and decreased local vascularity. If an Ilizarov frame is present for bone reconstruction, it can simultaneously be used for skin closure. The results shown in the current study indicate that a satisfactory outcome can be achieved.
