ABSTRACT Objectives To analyze the effects of maxillary tooth distalization by clear aligner (CA) treatment with variations in the angular direction of the distalization force, presence of attachments, and force-application method used. Materials and Methods A finite element model containing alveolar bone, dentition, and periodontal ligament was constructed. Analytical model groups were as follows: (1) distalization with buttons (without attachments), (2) buttons on canines (with attachments), (3) precision cuts on the canines (without attachments), and (4) precision cuts on the canines (with attachments). A distalization force of 1.5 N was applied to the button or precision cut at −30°, −20°, −10°, 0°, 10°, 20°, and 30° to the occlusal plane. Results As the direction of force approached +30°, the dentition inclined posteriorly. The posterior movement pattern was significantly influenced by the presence of an attachment and the direction of force, rather than the area where the force was applied. Distal inclination was dramatically reduced with attachments. A disengagement or deformation of the CA may reduce the distalization efficiency of the dentition or move the dentition in an inappropriate direction. Conclusions Attachments for efficient distalization by the CA are necessary. The use of miniscrews in the direction of force parallel to the occlusal plane is more advantageous than the use of Class II elastics. Due to CA deformation, distalization with the button on the canines can be more effective for distal movement of the maxillary dentition.

Effects of different distalization directions and methods on maxillary total distalization with clear aligners: a finite element study

ABSTRACT Objectives To detect any association between palatally displaced canine (PDC) and nasal septal deviation (NSD), palatal bone thickness and volume, and nasal airway dimensions and volume. Materials and Methods A total of 92 patients were included and subdivided into two groups: group 1, unilateral PDCs (44 patients), and group 2, normally erupted canines (NDCs) (48 subjects). The following variables were measured using cone-beam computed tomography: presence and type of NSD, nasal width, inferior conchae, hard palate and nasal septum thickness, maxillary bone and nasal airway volumes. Results NSD was detected in 77% and 50% of PDC and NDC subjects, respectively. Within the PDC subjects, significant differences between the displaced and nondisplaced sides were detected. Palate thickness was increased in the canine region and reduced in the molar region. Compared with the control group, PDC subjects had reduced palate thickness and lower nasal airway volume. Two predictors were significant for predicting the odds of PDC occurrence: NSD and maxillary bone volume. Conclusions NSD is more frequent in PDC subjects. PDC subjects have reduced palate thickness and decreased nasal airway volume. In the presence of NSD, the odds of developing PDC increase by 3.35 times, and for each one-unit increase in the maxillary bone volume, the odds of developing PDC decrease by 20%.

Evaluation of nasal septal deviation and maxillary bone and nasal airway dimensions and volumes using cone-beam computed tomography in patients with unilateral palatal canine displacement: a retrospective study

ABSTRACT Treatment of hyperdivergent skeletal Class III malocclusion is challenging for orthodontists, and orthognathic-orthodontic treatment is usually required. This report presents the successful nonsurgical treatment of a 20-year-old man who had a skeletal Class III malocclusion with anterior open bite, anterior and posterior crossbite, hyperdivergent growth pattern, steep occlusal plane, early loss of three first molars, and an uncommon convex profile with a retruded chin. An orthodontic camouflage treatment plan was chosen based on the etiology and the patient's complaints. Tooth #37 was extracted. Miniscrews were used for uprighting and intruding of the lower molars, distalization of the lower dentition, and flattening of the occlusal plane. After 34 months of active treatment, Class I relationships, proper anterior overjet and overbite, flat occlusal plane, and an esthetic facial profile were achieved. The results demonstrated that the biomechanics involved in the nonsurgical treatment assisted with miniscrews to distalize the mandibular dentition and flatten the occlusal plane while keeping the mandibular plane stable was effective for treating this hyperdivergent skeletal Class III patient with a convex profile and anterior open bite.

Nonsurgical treatment of a hyperdivergent skeletal Class III patient with mini-screw–assisted mandibular dentition distalization and flattening of the occlusal plane

To assess the effects of 10-14 days of rapid maxillary expansion (RME) on nocturnal enuresis (NE) in children who have long-standing resistance to medical therapy and to evaluate the long-term success rate after 10 years.Twenty-three children with NE, aged 6-15 years old (mean age = 10), who wet their bed almost every night and had never been dry were referred from pediatric specialists. Mean RME was 6.5 mm (range = 5-8), but only 7 of the 23 patients had lateral crossbites. Rhinomanometric measurements were taken before and after RME, and patients were interviewed 10 years after treatment.Positive effects of RME were observed in nearly 50% of the patients within 1 month of treatment: six were completely dry and five had notable improvements. Relapse in the overexpanded arches to a normal transversal occlusion was noted within 1 year. No correlation was found between success and improved airways, familial heritage, school performance, or other social factors. Younger children responded better to the treatment. Results were stable at the 10-year follow-up, and no adverse reactions were reported.Orthodontic RME is a new option for treating children with NE who are resistant to medical therapy; the treatment has no adverse side effects.

Rapid Maxillary Expansion Effects on Nocturnal Enuresis in Children

ABSTRACT Objectives To evaluate the influence of rapid maxillary expansion (RME) on nocturnal enuresis (NE) in children to discuss whether RME can be indicated as an alternative treatment for NE in those patients. Materials and Methods An electronic search was performed in the following databases: PubMed/MEDLINE, Cochrane Library, Scopus, Science Direct, Google Scholar and LILACS. The literature review was blindly performed by two reviewers. References of each selected study were manually searched to identify articles that were not found by the electronic search. Kappa statistics were used to analyze interexaminer agreement after the selection of the articles. After reading the selected full-text articles, the studies that met the inclusion criteria were assessed qualitatively using the Risk Of Bias In Non-randomized Studies of Interventions (ROBINS-I) and the Revised Cochrane risk-of-bias tool for randomized controlled trials-2 (RoB 2). The certainty level of evidence was assessed through the Grading of Recommendations, Assessment, Development and Evaluation tool. Kappa tests were used to analyze the interexaminer concordance level after the quality assessment of the studies. Results A total of 488 articles were found; however, after applying the inclusion and exclusion criteria, only 8 studies were selected for the systematic review. A low certainty level of evidence suggested that RME seems to promote a variable reduction in frequency or a remission of NE in children in both the short (4, 6, and 8 months) and long term (13, 36, 48, and 120 months). Conclusions Based on currently available information, RME seems to promote an improvement in NE in children. However, the low quality of the existing evidence weakens the recommendation.

Influence of rapid maxillary expansion on nocturnal enuresis in children:

Objectives To explore how orthodontic treatment affects different aspects of life, such as self-esteem, well-being, and social interactions and, additionally, to identify the long-term challenges that orthodontic patients experience after treatment. Materials and Methods This qualitative study involved in-depth semistructured face-to-face interviews. A purposive sampling strategy aimed to recruit a representative cohort of orthodontic patients who had their orthodontic treatment in adolescence and had their treatment concluded at least 5 years previously. Twenty-one patients (13 females and 8 males) were interviewed using a prepiloted topic guide. These interviews were transcribed and analyzed verbatim, using the general inductive method of data analysis. Results The thematic analysis revealed four key themes and related subthemes: (1) self-confidence and interpersonal communication, (2) social and professional opportunities, (3) oral health and function, and (4) challenges. In-depth analysis revealed positive treatment outcomes regarding enhanced self-confidence, social interactions, and perceived improvement in oral health and function. It also highlighted ongoing challenges with retainers and dissatisfaction with relapse. Conclusions Orthodontic treatment can significantly impact patient lives. Orthodontic patients described direct long-term positive effects on their social lives and well-being. However, it is equally important to remain mindful of the long-term challenges that may arise after treatment, especially with retainer choices and relapse, enabling proactive measures to mitigate their impact.

Objectives To develop a systematic review of patient perspectives on the treatment of obstructive sleep apnea syndrome (OSAS) after undergoing orthognathic advancement surgery. Materials and Methods This systematic review adhered to the preferred reporting items for systematic reviews and meta-analysis (PRISMA) guidelines, and Patient, Intervention, Comparison, and Outcome (PICO) methodology was employed to formulate the research question. A literature search was performed using the following databases: Cochrane Library (Trials), PubMed via MEDLINE, Embase, and Web of Science (all databases). The Newcastle-Ottawa Scale was used to assess quality of the studies. Results Initially, 1407 articles were retrieved from diverse databases, and these were subjected to initial scrutiny. Subsequently, 17 articles were selected for thorough quality analysis and 6 studies for quantitative analysis. Most studies were classified as good quality. Maxillomandibular advancement surgery appeared to enhance patient satisfaction in cases of moderate to severe adult OSAS. Conclusions Maxillomandibular advancement surgery significantly improves patients’ subjective overall quality of life, improving by 6.36 points in questionnaire ratings and demonstrating long-term stability.

Evaluation of quality of life after orthognathic surgery in obstructive sleep apnea syndrome: a systematic review

ABSTRACT Objectives To evaluate the incidence and pattern of unexpected/excessive condylar displacement after comprehensive orthodontic treatment in adults. Materials and Methods Adult patients (age ≥18 years) who underwent comprehensive orthodontic treatment with pre-(T1) and post-treatment (T2) cone beam computed tomography scans were consecutively collected within an orthodontic cohort (N = 291). T1 and T2 CBCTs were superimposed three-dimensionally (3D) and condylar displacement was estimated by the 3D changes of condylar neck point (CdN) between T1 and T2 (ΔCdN). Participants with excessive condylar displacement (ΔCdN &amp;gt;1 mm) were classified as condylar displacement (+) and otherwise as displacement (−). The incidence and pattern of condylar displacement, association with factors such as sex, age, skeletal relationship, extraction pattern, treatment duration, history of temporomandibular joint disorder, and presence of condylar resorption with the final occlusal outcome were investigated. Results The incidence of unexpected condylar displacement &amp;gt;1 mm in the adult orthodontic cohort was 6.2%. Females (vs males; OR: 9.07; [95% CI: 1.19–69.23]) and Skeletal Class II (vs Classes I and III; OR: 4.57 [95% CI: 1.58–13.20]) demonstrated significantly higher odds of unexpected condylar displacement (P &amp;lt; .05). Condylar resorption was not evident in participants with condylar displacement and did not interfere with the final orthodontic outcome. Conclusions Unexpected 3D condylar displacement exceeding 1 mm was noted in approximately 6% of the adult orthodontic patient cohort. However, the condylar displacement per se was not associated with condylar resorption and did not cause clinical concerns.

Incidence of unexpected condylar displacement following comprehensive orthodontic treatment in adults

ABSTRACT Objectives To compare the biomechanics of labial and lingual fixed orthodontic treatment options for a simulated curve of Spee malocclusion. Materials and Methods An in vitro electromechanical orthodontic simulator was used to measure the three−dimensional forces and moments on each tooth of a mandibular arch. Labial and lingual brackets, both with 0.018−inch slot sizes, were bonded to mechanical teeth. Three archwire forms were considered: labial straight, lingual straight, and lingual mushroom. The simulator was set in a passive levelled position with 0.016−inch × 0.022−inch stainless steel archwires, then teeth were moved to a curve of Spee maloclussion with the first premolar intruded 1.5 mm and the canine and second premolar intruded 0.75 mm. Two−way mixed multivariate analysis of variance (α = 0.05) was used to compare forces and moments generated among the three archwires. Results Statistical differences were found in 55 of 63 comparisons of forces and moments between archwire types for each tooth around the arch. The lowest force magnitudes were measured for labial straight archwires at each tooth position. The lateral incisor experienced the largest gingival forces with all archwire forms. The first premolar and first molar experienced labial−lingual crown tipping moments in opposite directions between labial and the two lingual archwire forms. Conclusions Biomechanical differences between labial, lingual straight, and lingual mushroom treatment modalities for the correction of curve of Spee misalignments were elucidated. Labial straight archwire exerted the lowest force magnitudes overall. For both lingual archwire forms, the labial−lingual inclination of the first premolar could be highly variable during levelling.
