In a rapidly aging society, many patients will have multiple medical co-morbidities and on polypharmacy. Dental patients rarely have medical emergencies during their treatment and it is because of this rarity, that the odd emergency that presents can overwhelm the dentist’s ability to cope despite their theoretical knowledge. The authors discuss how to adapt the clinic facilities for managing an emergency and provide an overview of management of common emergencies that dentists may encounter.

Medical emergencies in Dentistry: Practical tips in Implementation

This cross sectional study investigated the clinical and radiographic status of crowned teeth with and without root canal treatment.&nbsp; The study used secondary data from records of patients who received crown treatment at the Faculty of Dentistry, University of Malaya before 2015 and walk-in patients attending the primary care clinic between July - December 2016. The inclusion criterion was crown fitted for more than 12 months. Once identified, the patients were assessed for clinical and radiographic status. Data were entered into and analysed using SPSS Version 20 software. A total of 66 crowns [35 root canal treated (RCT) and 31 non-root canal treated (n-RCT)] were identified. Chi-square test was carried out to determine the health of crowned teeth and for its longevity in the oral cavity. There were 43 crowns (46.5% RCT; 53.5% n-RCT) with plaque accumulation, 15 (66.7% RCT; 33.3% n-RCT) with gingival swelling/recession, 17 (46.5% RCT; 53.5% n-RCT) with widening of the periodontal ligament, 13 (53.8% RCT; 46.2% n-RCT) with bone loss and 5 (60% RCT; 40% n-RCT) crowns with periapical lesion. There was no statistically significant difference except for poorer alveolar bone condition for crowns that have been in the mouth for &gt;10 years. Based on the study, no significant adverse pulpal involvement between crown with RCT and crown without RCT was observed.

Assessment of clinical and radiographic status of crowned teeth

Fixed functional appliances are non-compliance dependent devices, which can assist the correction of Class II malocclusion. The mechanics of the fixed functional appliances may vary depending on the type of device used. We report the observed biomechanical effects of using the ForsusTM Fatigue Resistant Device (FRD) fixed functional appliance. The patient was a 14-year-old female presented with a Class II division I malocclusion on a Class II skeletal base. The malocclusion was complicated by a 10mm overjet and increased traumatic overbite. She was treated with the ForsusTM FRD appliance worn for 5 months after the alignment with fixed appliances (0.022” X0.028” MBT prescriptions) and finished on a non-extraction base. Dento-alveolar changes were the main contribution to the correction of the malocclusion and simultaneously improved the patient’s profile in the end of treatment. In conclusion, ForsusTM FRD appliance can be suggested to effectively correct Class II malocclusion.

Solanum ferox L. is a plant species which belongs to the Solanaceae family and the genus Solanum. The Solanum genus was found to exhibit anticariogenic activity and was traditionally used to treat oral diseases. However there are no scientific studies done specifically for Solanum ferox L. Hence the aim of the study is to determine the anticariogenic properties of flesh and leaf of ethanolic extract of Solanum ferox L. Alkaloids, flavonoids and tannins were detected in the leaf ethanolic extract via preliminary phytochemical screening. &nbsp;The presence of these phytochemicals may contribute to the anticariogenic activity. Treatment with different concentrations of flesh and leaf of ethanolic extract were used against Streptococcus pyogenes and Staphylococcus aureus via the method of agar well diffusion to indicate zones of inhibition. The antibiofilm activity of the flesh and leaf ethanolic extracts was tested. The flesh and leaf ethanolic extracts possess significant (p&lt;0.05) dose-dependent on antimicrobial activity and positive antibiofilm activity against respective pathogens. Flesh ethanolic extract has stronger anticariogenic activity compared to leaf ethanolic extract against Staphylococcus aureus. Streptococcus pyogenes exhibited higher susceptibility as compared to Staphylococcus aureus. &nbsp;In conclusion, it has been shown that the ethanolic extract of Solanum ferox L. exhibit anticariogenic properties against Streptococus pyogenes and Staphylococcus aureus.

Anticariogenic properties of Solanum ferox L. ethanol extract

This pilot study evaluated the effect of manuka honey as a subgingival adjunct to scaling and root surface debridement in the treatment of periodontitis. This study used a split-mouth design with a 3-month follow-up in seven participants diagnosed with periodontitis Stage III Grade B or C. Root surface debridement was performed on one side of the mouth (control); the other side received debridement plus manuka honey application (test). Clinical parameters were recorded at baseline, 6- and 12-weeks. Gingival crevicular fluid and subgingival plaque were sampled. Microbiological outcomes were analysed using benzoylarginine p-nitroanilide assay and polymerase chain reaction assays. Single application of manuka honey to periodontal pockets did not result in additional reduction of pocket depth, improvement of attachment levels or changes in p-nitroaniline enzymes when compared with root surface debridement alone. However, test sites exhibited greater reduction in bleeding than control sites, mean differences 1.3 (95%CI 1.2-1.5) and 1.7 (95%CI 1.5-1.9) at 6-weeks and 12-weeks, respectively. The proportion of mutans streptococci decreased at 6-weeks in test sites but increased at 12-weeks in control sites. Adjunctive application of manuka honey to periodontal pockets improved gingival inflammation but did not demonstrate significant clinical benefits compared with root surface debridement alone.

Manuka Honey as an Adjunct to Nonsurgical Periodontal Treatment: A Pilot Study

This study aims to determine the prevalence of dental anomalies and MSX1 gene 799G&gt;T polymorphism and its association with non-syndromic cleft lip with or without palate (NSCL±P) attending Hospital Universiti Sains Malaysia. Clinical and radiological assessments on 37 NSCL±P patients and 80 non-cleft children were done to detect dental anomalies. The buccal cells were collected and polymerase chain reaction restriction fragment length polymorphism (PCR-RFLP) was used to identify polymorphism. NSCL±P was higher among males (54%) and mostly unilateral cleft lip and palate (51.3%). The prevalence of dental anomalies in morphology in NSCL±P was 18.9% (95% CI: 5.7%, 32.2%) and non-cleft was 6.3% (95% CI: 0.8%, 11.7%). Hypodontia in NSCL±P was 75% (95% CI: 61.2%, 90.2%) and non-cleft was 7.5% (95% CI: 1.6%, 13.4%). There was a significant association between NSCL±P and anomalies in morphology (P= 0.04; OR=3.5)) and number (P&lt; 0.01; OR= 40). There was an absence of rare 799G&gt;T polymorphism in all NSCL±P and non-cleft children indicating that all samples contain common 799G polymorphism. In conclusion, the prevalence of dental anomalies in morphology and number was significantly higher in NSCL±P compared to non-cleft children. However, it was not significantly associated with MSX1 799G&gt;T polymorphism.

Dental Anomalies and Muscle Segment Homeobox1 Gene Polymorphism in Non-syndromic Cleft Lip with or without Palate Children

Accessory cusps are occasional variations in tooth morphology that can be detected clinically. Third molars have a wide range of eruption patterns and shapes but are commonly impacted. Unlike root morphology, variations in the crown morphology of the third molar have rarely been reported. Variations in crown morphology are important because they can affect clinical outcomes. Here, we present a rare case of fully erupted mandibular third molar (MTM) with multiple accessory cusps. This case report describes a case of a 31-year-old female Kenyah with a unique crown morphology of her fully erupted, vital, and functional mandibular third molar (MTM) which has multiple accessory cusps seen on the buccal surface, giving the appearance of a double tooth. Her MTM presented with an incipient caries lesion, which was managed conservatively. Documenting variations in tooth morphology is crucial for individual identification. While most people opt to have their MTMs extracted, early management of well-erupted and functional teeth should be undertaken to prevent disease progression.

Multiple Accessory Cusps Expression at Mandibular Third Molar: A Unique Variation of Dental Crown Morphology - A Case Report

Zygomycosis is a rare opportunistic fungal infection caused by fungi in the phylum Mucoromycota, subphylum Mucoromycotina and order Mucorales. Immunocompromised patients may harbour the disease by inhalation of the spores from the environment into the paranasal sinuses. Rhino-orbital-cerebral is the most common form which extends into the oral cavity, especially to the palate. We present twelve cases of zygomycosis diagnosed in the Stomatology Unit, Institute for Medical Research (IMR), Malaysia. Most of the patients are immunocompromised and presented as the rhino-orbital-cerebral form. Routine haematoxylin and eosin (H&amp;E) staining were used for all cases while selected cases required additional special staining. The characteristic histological features of broad, non-septate, branched, amphophilic fungal hyphae are seen. We aim to report this rare occurrence to contribute to the literature and raise awareness about this condition among clinicians. To date, this is the first compilation of oral zygomycosis cases known to be reported in Malaysia.

Zygomycosis of the Oral Cavity in a Main Referral Centre in Malaysia: A Case Series (1967-2021)

This study was undertaken to assess knowledge regarding furcation defects among dental fraternity. This is an open-ended, randomized, web-based cross-sectional multinational online survey. A total of 904 responses were gathered by sending 13 questions through a google form. Data were expressed in frequency and percentages. Chi-square test was performed to determine significant difference between gender, education level and variation between dentists of India and other countries. P value &lt;0.05 was considered to be statistically significant. Present study had 650 dentists from India and 254 dentists from other countries. For overall knowledge regarding furcation, 90.7% (average of first and second question) of dentists had knowledge regarding furcation. There was no significant difference between males (range 54%-93%) and females (range 46%- 95%) (p&gt;0.05) in terms of knowledge. Majority (88.7%) of post-graduates knew about the use of Nabers probe for furcation measurements and values were significant (p&lt;0.05). Present study also revealed 65%-67% of undergraduates and post-graduates knew that Degree II furcation defects showed predictable periodontal regeneration, however values were non-significant (p&gt;0.05). There was no statistical significance (p&gt;0.05) between dentists of India and other countries. Dental fraternity in general had adequate knowledge regarding the furcation defects, their treatment modalities and expected complications.
