Matrix metalloproteinases (MMPs) are known to play a key role during orthodontic treatment leading to periodontal remodelling and tooth movement. MMPs may be induced by mechanical forces. However, the role played by toll-like receptors (TLRs) in modulating the effects of the mechanical force on periodontal fibroblasts is not known. To investigate the interaction between mechanical force and TLR stimulation, primary cultures of human periodontal fibroblasts were submitted to centrifugation in the presence of LPS and Pam3Cys, which are known TLR-4 and TLR-2 ligands, respectively. The expression of MMP-1, -2, -3, -8, -9, -10 and -13; TIMP (Tissue Inhibitor of Metalloproteinases) -1, -2 and -4; TNF-α (Tumour Necrosis Factor alpha); IL-1β (Interleukin 1 beta); ERK 1/2 (Extracellular Signal-Regulated Kinase 1/2); p38; JNK (c-jun N-terminal Kinase); IRAK1 (Interleukin-1 Receptor-Associated Kinase); and NF-κB (Nuclear Factor kappa B) were measured by antibody array, ELISA and immunoblotting methods. The activation of TLRs associated with centrifugation induced an increase in the secretion of MMPs 1, 3 and 10, with no increase in TNF-α or IL-1β. An increase in the phosphorylation of the MAP kinases p38 and JNK and the transcription factor NF-κB, without an increase in TIMPs was also observed. These findings suggest that the secretion of MMPs by cultured periodontal fibroblasts that is induced by combined TLR activation and mechanical force stimulation is regulated via the p38, JNK and NF-κB pathways. The increased secretion of MMPs by TLR activation may be an important factor that should be considered during orthodontic treatment.

Hyaluronan (HA), a major glycosaminoglycan of synovial fluid, is synthesised by a class of membrane-bound HA synthase (HAS) proteins. In the present study, we investigated the regulatory roles of IL-1beta on HAS gene expression and HA production by the fibroblastic synovial lining cells. The synovial lining cells from synovial membrane in human temporomandibular joint (TMJ) were cultured and characterised using immunocytochemistry with CD14, CD44, and vimentin monoclonal antibodies. With or without treatment with IL-1beta, the production of HA was detected with radiometric assay and the expression of HAS mRNAs were analysed with a semi-quantitative reverse transcribed polymerase chain reaction (RT-PCR). HA synthesis was significantly augmented with 1ng/ml of IL-1beta for both 24 and 48h stimulation, however the production of HA declined if stimulated with 10ng/ml of IL-1beta. The expression of HAS2 and 3 mRNA were enhanced about 4.2- and 7.2-fold after 4h stimulation with 1ng/ml of IL-1beta, respectively. From these results, it is concluded that IL-1beta functions on regulating HAS expression and consequently promoting the secretion of HA in synovial lining cells from TMJ.

Type 17 immune response promotes oral epithelial cell proliferation in periodontitis.

This manuscript aims to provide a comprehensive review of the current knowledge in the pathophysiology, diagnosis, prevention, and other relevant forensic and clinical aspects of a potentially severe complication known as medication-related osteonecrosis of the jaw (MRONJ) while synthesizing state-of-the-art information on bisphosphonates and introducing a possible differential diagnosis. An extensive search was conducted in PubMed (U.S. National Library of Medicine) without a time or language constraint, focusing on the epidemiology, pathophysiology, risk factors, site specificity, signs and symptoms, differential diagnosis, prevention, and forensic aspects of MRONJ. All types of original articles, reviews, case reports, short communications, opinion articles, guidelines, and letters to editors were considered to produce a complete review on this subject. MRONJ prevention relies on a multidisciplinary approach and is critical since truly effective treatments are lacking. This therapeutic challenge is partly due to uncertainty regarding this condition's pathophysiology. Differential diagnosis of osteonecrosis of the jaws associated with krokodil abuse, one of the most dangerous psychoactive illicit substances, should be considered. Further research into the etiology and site specificity of MRONJ is encouraged, aiming to develop novel treatment prospects. Indeed, comprehending this would allow for increased efficacy and therapeutic options while emphasizing the importance of prevention. In addition, we advocate for greater consensus among the various societies regarding MRONJ's treatment and management.
