The Southeast Asian Journal of Tropical Medicine and Public Health was established in 1967 with the aim of improving the health and living standards of people in Southeast Asia by pooling human resources from participating SEAMEO member countries. The project focuses on enhancing the training capabilities of existing facilities and aims to minimize the waste of duplicated programs and activities through effective regional cooperation among the participating national centers. In 1992, the project was renamed the SEAMEO Regional Tropical Medicine and Public Health Network.

Studies of the potential association between latent toxoplasmosis and schizophrenia spectrum and other psychotic disorders (SSOPD) have yielded mixed results. In this study we aimed to investigate the potential association between a positive serology test for Toxoplasma gondii and the presence of SSOPD and selected demographic factors in order to determine if SSOPD patients should be screened for T. gondii. In this study, subjects were chosen from 2 groups: those with SSOPD (cases) and those without SSOPD (controls). Cases were purposely recruited from patients attending the Hospital Canselor Tuanku Muhriz, Kuala Lumpur and controls were recruited from a healthy population. The diagnosis of SSOPD was made by a mental health professional following the criteria of the Diagnostic and Statistical Manual of Mental Disorders, Fourth Edition (DSM-IV). In both groups, demographic factors were recorded and each subject had blood obtained to test for indirect ELISA for T. gondii antibodies. A total of 218 subjects were included in the study: 109 cases and 109 controls. The prevalences of confirmed positive tests for T. gondii among cases and controls were 24% and 32%, respectively (p = 0.227). Those aged 18-35 years had a significantly lower seroprevalence of T. gondii among cases than controls (45% vs 70%, p = 0.001). Ethnic Malays had a significantly lower seroprevalence of T. gondii among cases than controls (41% vs 80%, p<0.001). Those with a tertiary education had a significantly lower seroprevalence of T. gondii among cases than controls (51% vs 93%, p<0.001). Unemployed subjects had a significantly greater seroprevalence of T. gondii among cases than controls (77% vs 30%, p<0.001). Those who were unmarried had a significantly greater seroprevalence of T. gondii among cases than controls (67% vs 38%, p<0.001). Those with a family history of psychiatric illness had a significantly greater seroprevalence of T. gondii among cases than controls (51% vs 6%, p<0.001). These other factors may have affected our study results. In our study, there was no significant association between toxoplasmosis and SSOPD. SSOPD patients in the study population do not need to be routinely screened for the presence of T. gondii. Further studies in the study population need to control for demographic factors significantly associated with positive serology for T. gondii in order to determine if this changes our findings.

The Bacillus Calmette-Guerin (BCG) vaccine, given to prevent Mycobacterium tuberculosis, may cause disseminated BCG, an infection and inflammatory reaction in immunocompromised individuals. In this study, we aimed to determine the chest and abdominal computed tomography (CT) findings among patients with disseminated BCG disease in order to identify some potential consequences of this vaccine and their frequencies. This study was conducted among all patients aged 2 of the following: a temperature >38.5°C for >2 weeks, a familial history of immunodeficiency, weight loss, recurrent or persistent diarrhea, recurrent or persistent oral candidiasis, body aches, a hemoglobin <10 mg/dl or hepato- or splenomegaly. Exclusion criterion for study subjects were having a history of a lung disease unrelated to disseminated BCG disease or a diagnosis of tuberculosis or other atypical mycobacterial infections. Chest and abdominal CT scans were performed within 48 hours of admission on each study subject. A total of 22 subjects were included in the study; 14 males. The mean (+standard deviation) age of study subjects was 68 (+51) (range: 3-192) months. The chest and abdominal CT scan results among study subjects revealed: 55% (n=12) had a mediastinal mass, 36% (n=8) had a lung parenchymal mass, 36% (n=8) had a fusion of lung and mediastinal tissue, 36% (n=8) had invasion of the mediastinum, 32% (n=7) had lung parenchyma collapse, 27% (n=6) had invasion of the chest wall, 18% (n=4) had invasion of the ribs, 9% (n=2) had lobulated soft tissue lesions and 9% (n=2) had a cutaneous tract. In summary, we identified the most common chest and abdominal CT findings among subjects with disseminated BCG disease. We conclude that due to the not uncommon problem of immunocompromised patients in the study population and relatively common problem of disseminated BCG disease, these CT results should be considered as possible manifestations of disseminated BCG disease. Further studies are needed to determine if any of these chest and abdominal CT findings can be used for the early diagnosis of disseminated BCG disease.

Periodontitis and microvascular diseases are commonly found in poorly controlled type 2 diabetes mellitus (DM). In this study, we aimed to evaluate the association between periodontitis and the microvascular complications of DM in order to determine if diabetics with periodontitis should be screened for the presence of microvascular disease. We recruited diabetic patients who attended the Endocrinology Unit, Department of Medicine, Vajira Hospital, Navamindradhiraj University, Bangkok, Thailand during May-July 2018. Inclusion criteria were: 1) DM patients aged ?30 years who had the results of a HbA1c test during the previous month, 2) who had been evaluated for a diabetic microvascular complication defined as having either diabetic nephropathy (DN) (defined as having an estimated glomerular filtration rate or eGFR ?60 ml/min/1.73 m2 or a urine microalbumin level >30 mg/g urine) or diabetic retinopathy (DR) (defined as having non-proliferative diabetic retinopathy (NPDR) or proliferative diabetic retinopathy (PDR) found on non-mydriatic retinal photography by an ophthalmologist during the previous 12 months), and 3) having ?6 teeth in 2 quadrants in diagonal positions. Subjects were excluded if they were critically ill or had a condition making a dental examination difficult. Demographic data, HbA1c levels, laboratory blood-test results, duration of DM and presence of microvascular complications were obtained from their medical charts. Subjects were examined for periodontitis by measuring: 1) probing depth (PD), 2) clinical attachment level (CAL), and 3) bleeding on probing (BoP). A total of 184 subjects were included in this study; 40% male. The mean (±standard deviation (SD)) age of study subjects was 59.9 (±10.4) (range: 31-88) years. The mean (±SD) HbA1c levels among those with and without complications were 8.8 (±2.1) and 7.8 (±1.8), respectively (p=0.002). The mean (±SD) CALs among patients with and without complications were 3.6 (±1.1) mm and 3.3 (±1.0) mm, respectively (p>0.05). The mean (±SD) PDs among those with and without complications were 2.5 (±0.6) and 2.5 (±0.6), respectively (p=0.75). About one-fourth (24.5%) of subjects had microvascular complications of DM and 88.0% had moderate-to-severe periodontitis. On logistic regression analysis periodontitis was not significantly associated with microvascular complications (p>0.05). In our study population, there was no significantly association between periodontitis and microvascular complications of diabetes; therefore, screening DM patients with periodontitis for microvascular complications is not warranted.

Functional constipation is a common problem in pre-school children precipitated by intrinsic and extrinsic conditions. Risk factors associated with functional constipation among pre-school Thai children were identified in a case-control study employing a questionnaire based on Rome IV criteria among children 1-4 years of age (constipated, n = 45, non-constipated, n = 45) attending an outpatient department between September 2017 and January 2018. A significant common risk factor was a family history of constipation (p-value = 0.012). Univariate logistic regression analysis revealed significant differences between constipated and non-constipated group in exclusive formula feeding during the first year of life (odds ratio (OR) = 9.51, 95% confidence interval (CI): 1.14-79.6), daily vegetable or fruit ingestion of less than 1 ladle (OR = 9.63; 95% CI: 3.67-25.25), less than 250 ml daily water intake (OR = 4.42, 95% CI: 1.69-11.58), daily fruit juice intake of <250 ml (OR = 2.58; 95% CI: 0.62-10.69), weekly energetic (sweating) movement/activity of ?4 days (OR = 8.11; 95% CI: 0.95-68.87), and major stressful event (OR = 9.51, 95% CI: 1.14-79.6). A multivariate logistic regression analysis indicated limited fruit or vegetable ingestion (adjusted OR = 11.07, 95% CI: 3.82-32.08) and water intake (adjusted OR = 5.47, 95% CI: 1.73-17.34) are significant independent risks for constipation in children. The study highlights recognition of these risk factors by parents or caregivers as well as physicians in order to modify feeding behavior and avoid pharmacological intervention of constipation in children in the country.

Cytomegalovirus (CMV) is a viral pathogen that can cause infection in kidney transplant recipients. We report a case of a 50-year-old Thai male kidney transplant recipient with a history of multiple episodes of urinary tract infection, presented with fever and vesicular rash. He was diagnosed with having CMV nephritis proven by immunohistochemistry stains on kidney biopsy and multi-dermatomal herpes zoster (HZ) co-infection proven by pathological findings on skin biopsy. The infections improved with immunosuppressive therapy reduction and intravenous ganciclovir for 2 weeks, followed by oral valganciclovir for 3 months. CMV nephritis and HZ are fairly common in kidney transplant recipients but have never been reported together as a co-infection. CMV nephritis/HZ co-infection should be considered in transplant recipients who present with impaired renal function and multi-dermatomal vesicular lesions.

Orbital tuberculosis is a rare manifestation of extrapulmonary tuberculosis. We report here 2 cases of females who presented with non-tender orbital masses. Both patients presented with orbital masses without other symptoms. Neither had a history of an underlying disease. In both cases excision biopsy was performed on the masses and the pathology of both cases showed granulomatous inflammation with caseation. Both cases were treated with isoniazid, rifampicin, ethambutol and pyrazinamide. In both cases the masses resolved completely without residuals. Orbital tuberculosis is rare but can have a good outcome if treated correctly and early.
