We have developed an in vitro cataract model using enucleated animal eyes. A cataract is produced by immersing the enucleated eyes in hypertonic sodium chloride solution for one hour and re-incubating them for another hour in normal saline solution. The model is useful for practicing various techniques of cataract extraction.

Corneal endothelial cell loss is compared in four types of cataract procedures with intraocular lens implants performed by the authors. A total of 110 intracapsular cataract extractions, 182 extracapsular cataract extractions and 107 phacoemulsifications, divided into 47 anterior chamber emulsifications and 60 posterior chamber emulsifications (all with intraocular lens implants) are compared. A significant feature of these procedures is the incidence of endothelial cell loss greater than 1,200 cells/mm2. This occurred in 1% of the intracapsular cataract extractions, 4% of the planned extracapsular cataract extractions, 13% of the posterior chamber phacoemulsifications, and 17% of the anterior chamber phacoemulsifications. Pachymetry is a poor method for judging operative corneal endothelial trauma. Based upon the results of this study, it remains to be seen whether the advantages of phacoemulsification mitigate against the increased incidence of significant endothelial cell loss.

Seven days after extracapsular cataract extraction and intraocular lens implantation, a posterior chamber lens was removed following the patient's sudden death. The lens was studied with the implant cytology technique, which demonstrated a continuous and optically clear separating film of proteinaceous nature. The film was fully developed and covered the entire implant. A few macrophages were found in one area on the surface of the otherwise acellular film; these were involved in blood phagocytosis and did not appear to have been involved in the formation of the acellular proteinaceous film.

A new approach to in-the-bag insertion is described. It combines elements of previously used techniques and includes a D-shaped capsulotomy, a posterior chamber intraocular lens with polypropylene loops and one control tip, a Sinskey, reverse Maltzman, or Similar hook, and a lens loop injector (a simplified version of the Bailey lens shooter).
