The co-occurrence of substance use disorders (SUD) and other mental disorders (OMD) is assumed to be high but is, in fact, unknown in Spain; and it is approached from different healthcare networks. The objective of the present study was to know the prevalence of this co-occurrence, both in specific addiction treatment networks and in mental health networks, in Spain. An observational, multicenter cross study, with a randomized sample, of patients under treatment for SUD or OMD in different Autonomous Communities of Spain was carried out (N=1,783). A specific ad hoc online questionnaire collecting sociodemographic variables, substance use and diagnoses of SUD and OMD was completed. The data obtained in the survey show a significant concurrence of SUD and OMD diagnoses (in more than 60% of the patients). A high prevalence of OMD was found in those patients receiving treatment for their SUD (71%), and also of diagnoses of any SUD (68.9%) and active substance use (50%, except tobacco) in people receiving treatment for diagnoses of OMD. Also were found significant relationships between addiction to certain substances and specific mental disorders: personality disorders with all SUDs; psychotic disorders with cannabis use disorder, but not cocaine use disorder; affective disorders with cocaine use disorder, and anxiety disorders with cannabis use disorder. This study provides preliminary information about the high coexistence in routine clinical practice of addictive disorders and other mental disorders in different treatment settings in Spain.

Detailed knowledge of the epidemiology of alcohol consumption at the population level is essential to the design of effective public health programs. The objectives of this study were to estimate the prevalence of daily, sporadic, occasional, binge and hazardous alcohol consumption among the Galician population aged 16 years and over, and to characterize hazardous drinkers, taking into account the gender perspective. A cross-sectional study was conducted in 2017 among people aged over 15 years residing in Galicia in the framework of the Risk Behavior Information System (SICRI). In 2017, 7,841 adults were interviewed by stratified random sampling. Questions related to alcohol consumption in the last month were included and the prevalence of hazardous consumption was estimated using the AUDIT test. A multivariate logistic regression model was fitted to characterize hazardous drinkers. A total of 18.8% of the Galician population consumed alcohol daily and 3.4% had hazardous consumption. The prevalence of daily, occasional, sporadic, binge and hazardous drinking was higher among men and varied by age. The prevalence of daily-occasional and sporadic drinking was highest at middle age (45-64 years) and binge and hazardous drinking among those aged 16 to 24 years. The characteristics associated with hazardous drinking varied by gender, and are identified by common characteristics to both genders such as age or being a tobacco or cannabis smoker, and differential characteristics such as educational level. Typologies of consumption changed according to age and gender. Surveillance systems should regularly monitor alcohol consumption in order to adapt prevention measures to changes in drinking patterns.

Drug use is deeply rooted in Western ‘culture’ and its use is related to traditions, celebrations or leisure space. However, such use is not free of risks that depend on the amount, frequency and pattern of consumption, as well as the characteristics of the consumer such as age, sex and some health conditions. Despite mounting scientific evidence proving the toxicity of drugs, their supply and demand for them continues to grow, mainly due to the appearance or rediscovery of new psychoactive substances and the ‘attractiveness’ of new forms or patterns of use. Paradoxically, although concern aroused by these new consumption patterns is rising, very few studies have been carried out in Spain that analyze this issue.

We aimed to characterize the availability and promotion of alcohol at alcohol outlets in Madrid and to compare them according to type of outlet and area-level socioeconomic status. We used the OHCITIES instrument to characterise the alcohol outlets in 42 census tracts of Madrid in 2016. We specified alcohol availability as the density of alcohol outlets and the number of alcohol outlets with extended opening hours (12 or more). We registered any type of promotion associated to alcohol outlets that could be perceived from outside the outlet. We calculated and compared proportions of availability and promotion by alcohol outlet (on- and off-premise) using chi-squared and Fisher Exact tests. We estimated the availability and promotion of alcohol densities per census tract according to area-level socioeconomic status. To assess statistical significance, we used Kruskal-Wallis tests. We recorded 324 alcohol outlets, 241 on-premise and 83 off-premise. Most of the outlets had extended opening hours (73.77%) and at least one sign promoting alcohol (89.51%). More on-premise outlets had extended opening hours and higher presence of alcohol promotion than off-premise (p < 0.001). Higher density of alcohol outlets, extended opening hours and presence of alcohol promotion were found in higher socioeconomic areas (all p < 0.001). These results were also observed for on-premise alcohol outlets. Alcohol availability and promotion were associated with alcohol outlets in Madrid. Future alcohol policies regulating the availability and promotion of alcohol should consider outlet types and area-level socioeconomic status.El objetivo fue caracterizar la disponibilidad y promoción de alcohol asociados a los locales de venta y consumo de alcohol en Madrid, así como explorar las diferencias en su distribución en función de la tipología del local y las características socioeconómicas del área. Se utilizó el instrumento OHCITIES para caracterizar locales situados en 42 secciones censales de Madrid durante 2016. Se registró la densidad de locales y el número de locales con amplios horarios de apertura (12 o más horas). Se registró cualquier tipo de promoción asociada al local visible desde el exterior. Se compararon los porcentajes de características de disponibilidad y promoción asociada a los locales de consumo y venta de alcohol utilizando el test de chi cuadrado y la prueba exacta de Fisher. Se estimó la densidad de disponibilidad y promoción por sección censal y se exploró su distribución en función de las características socioeconómicas del área mediante el test de Kruskal-Wallis. Se registraron 324 locales, 241 de consumo y 83 de venta. La mayoría tenía un horario amplio de apertura (73,77%) y algún elemento promocional (89,51%). Los locales de consumo tenían horarios más amplios de apertura y más elementos promocionales que los de venta (p < 0,001). Se encontraron mayor densidad de locales, amplitud de horarios y elementos promocionales en áreas de nivel socioeconómico alto (todos p < 0,001). La disponibilidad y promoción estuvieron asociadas con los locales de venta y consumo de alcohol en Madrid. Futuras políticas cuyo objetivo sea la prevención del consumo de alcohol deben tener en cuenta la influencia de los tipos de locales y las características socioeconómicas del área en la distribución de la disponibilidad y promoción de alcohol.

Las agresiones sexuales facilitadas por drogas (DFSA) constituyen un tópico que viene suscitando una creciente preocupación social en los últimos años. Pese a ello, son pocos los trabajos empíricos llevados a cabo en España que hayan analizado el fenómeno desde un enfoque preventivo. El objetivo de este trabajo, además de aportar nueva evidencia respecto a las DFSA, concretamente en el ámbito de los menores, ha sido identificar posibles variables asociadas, contribuyendo así a comprender mejor el problema y a diseñar políticas de prevención más eficaces. Para ello se realizó una encuesta entre menores de la comunidad autónoma gallega. Participaron 7.181 estudiantes de 12 a 17 años ( M = 14,79; DT =1,57). La recogida de datos se realizó mediante un cuestionario autoadministrado. Los datos referentes a las DFSA fueron recogidos mediante ítems específicos, pilotados previamente. Se utilizaron también instrumentos para el cribado de consumos problemáticos (AUDIT, CAST, CRAFFT y EUPI-a). Los resultados permiten estimar la tasa de victimización por DFSA en menores gallegos en un 1,7%, de los que únicamente habrían denunciado el 11,4%. Más allá del perfil sociodemográfico de las víctimas (mujeres en 2 de cada 3 casos), éstas presentan patrones diferenciados respecto al patrón de consumo problemático de alcohol y otras sustancias y al patrón de uso problemático de Internet y redes sociales, con una prevalencia significativamente mayor de conductas de riesgo online . Ello sugiere que este fenómeno va mucho más allá de la violencia sexual, por lo que es preciso abordarlo a nivel preventivo desde una perspectiva integral, incluyendo la perspectiva educativa y de salud pública.

There is a need for theory-based approaches to reduce heavy drinking in youths. Anchoring is a process in which people make estimates by starting from an initial value that is adjusted to yield a final answer. In this study, we determined the effectiveness of anchoring heuristics in estimating the price of a bottle of alcohol and its effect on heavy drinking in young people. In a sample of 327 young people in Mexico, participants completed a survey on alcohol drinking and anchoring heuristics. We found that when the anchor was shown and the participants were explicitly directed to the anchor value, the mean estimates of the price reported by the participants increased, indicating that the participants were pulled up toward the anchor. Anchoring effects were found to affect on the willingness of the alcohol consumers to pay. In this context, alcohol prices (anchor condition) were higher for heavy drinking. We discuss the implications of our results and future directions for research on anchoring effects in heavy drinking. We concluded that anchoring provides novel insights into the consequences of anchoring effects in alcohol drinking.

Problematic smartphone use (PSU) is an uncontrollable behavior that impedes the ability to stop using it despite potential negative consequences. This excessive behavior has been considered in the research field as a behavioral addiction, as literature has shown common characteristics with behavioral addictions, including its impulsivity-driven factor. However, impulsivity is a multidimensional construct whose specific traits lead differently to each addiction. Hence, the present study aimed to address the different existent PSU profiles depending on their individual impulsivity personality traits. To this end, N = 412 adults (average age 31.91, SD = 11.70, including 108 men and 304 women) were recruited to explore their daily smartphone usage, level of smartphone addiction (SAS-SV) and impulsivity profile across the five impulsivity personality traits (UPPS-P). Cluster analysis revealed the existence of three different profiles: one without PSU; one showing an excessive impulsively but almost no addictive smartphone use, expressing only a loss of control symptomatology; and one showing excessive impulsively but also addictive smartphone patterns, driven mainly by the impulsivity personality traits of negative urgency, positive urgency and lack of premeditation. Therefore, this study showed the impulsivity personality traits that differentiate excessive from addictive smartphone use, which is valuable information for the development of more precise prevention and interventions programs.

During periods of isolation, people with substance use disorders may reduce tension, stress, uncertainty and possible distress by increasing the use of substances or practices that they have abused. The objective of this study was to evaluate the impact of the pandemic on emergency care and continuity of care for patients with substance use disorders.Study carried out in the only psychiatric emergency service in the province at the Hospital Universitario Santa María de Lérida, which cares for 431,183 people. Sociodemographic, clinical and evolutionary variables were collected from all the patients treated during a period prior to lockdown (January 13, 2020, until March 14, 2020) and during the first (March 15, 2020 until June 20, 2020) and second states of emergency (from October 25, 2020 to May 9, 2021).908 patients attended with substance use disorder, representing 23.8% of all visits. During the first state of emergency, visits increased (p < 0.001) with a decrease in the average age (p = 0.0023). During the second state of emergency, there was an increase in the use of alcohol with respect to the rest of toxic substances (p < 0.001) and an increase in the visits of patients without prior follow-up (p = 0.005).Substance use disorder consultations increased in the first state of emergency, with patients being younger and attending for reasons related to outpatient discontinuity, while in the second state of emergency, alcohol use increased in people without prior follow-up and with small social networks. Admissions in the first state of emergency were shorter, with no subsequent link to other detoxification treatment centers and with an earlier return to the emergency room, especially in female users.Introducción: Durante los períodos de aislamiento, las personas con trastornos por uso de sustancias pueden reducir la tensión, el estrés, la incertidumbre y la posible angustia aumentando el uso de sustancias o prácticas de las que han abusado. El objetivo de este estudio es evaluar el impacto de la pandemia en las atenciones urgentes y continuidad asistencial de pacientes con trastorno por uso de sustancias. Método: Estudio realizado en el único servicio de urgencias de Psiquiatría de la provincia en el Hospital Universitario Santa María de Lérida, que atiende a 431.183 personas. Se recogen variables sociodemográficas, clínicas y evolutivas de todos los pacientes atendidos durante un periodo previo al confinamiento (13 de enero de 2020, hasta el 14 de marzo de 2020) y durante el primer (15 de marzo de 2020 hasta su 20 de junio de 2020) y segundo estado de alarma (desde el 25 de octubre de 2020 hasta el 9 de mayo de 2021). Resultados: 908 pacientes atendidos con Trastorno por Uso de Sustancias, representa el 23,8% de todas las visitas. Durante el primer estado de alarma, aumento de las visitas (p < 0,001) con una disminución de la edad media (p = 0,023). Durante el segundo estado de alarma, aumento del consumo de OH respecto al resto de tóxicos (p < 0,001) y un aumento de las visitas de pacientes sin seguimiento previo (p = 0,005). Conclusiones: Aumentaron las consultas por trastorno por uso de sustancias en el primer estado de alarma siendo más jóvenes y acudiendo por motivos de consulta relacionados con la discontinuidad ambulatoria mientras en el segundo estado de alarma repuntó el consumo de alcohol en personas sin seguimiento previo y con escasa red social. Los ingresos del primer estado de alarma fueron más breves, sin vinculación posterior a otros centros terapéuticos de desintoxicación y con un retorno más precoz a urgencias sobre todo en mujeres consumidoras.

Atenciones urgentes realizadas durante la pandemia por consumo de tóxicos en una provincia española

Given the causal impact of alcohol use on stroke, alcohol control policies should presumably reduce stroke mortality rates. This study aimed to test the impact of three major Lithuanian alcohol control policies implemented in 2008, 2017 and 2018 on sex- and stroke subtype-specific mortality rates, among individuals 15+ years-old. Joinpoint regression analyses were performed for each sex- and stroke subtype-specific group to identify timepoints corresponding with significant changes in mortality rate trends. To estimate the impact of each policy, interrupted time series analyses using a generalized additive mixed model were performed on monthly sex- and stroke subtype-specific age-standardized mortality rates from January 2001-December 2018. Significant average annual percent decreases were found for all sex- and stroke subtype-specific mortality rate trends. The alcohol control policies were most impactful on ischemic stroke mortality rates among women. The 2008 policy was followed by a positive level change of 4,498 ischemic stroke deaths per 100,000 women and a negative monthly slope change of -0.048 ischemic stroke deaths per 100,000 women. Both the 2017 and 2018 policy enactment timepoints coincided with a significant negative level change for ischemic stroke mortality rates among women, at -0.901 deaths and -1.431 deaths per 100,000 population, respectively. Hemorrhagic stroke mortality among men was not affected by any of the policies, and hemorrhagic stroke mortality among women and ischemic stroke mortality among men were only associated with the 2008 policy. Our study findings suggest that the impact of alcohol control policies on stroke mortality may vary by sex and subtype.
