Background Central catheter-related thrombosis (CRT) is the most common catheter-related complication in patients with end-stage renal disease (ESRD) but is often underappreciated and misdiagnosed by radiologist. Purpose To find the computed tomography angiography (CTA) characteristics of central CRT, then raise the diagnosis of this disorder. Material and Methods A total of 301 eligible patients with ESRD who experienced both chest multi-phase multidetector CTA (MDCTA) and digital subtraction angiography were enrolled in the final analysis. The location, shape, and related signs of the central CRT in MDCTA images were evaluated. Independent-samples T test, chi-square test, and binary logistic regression were analyzed using SPSS software. Results In total, 166 patients were found to have CRT using MDCTA, and this was verified by DSA. Central CRT was usually irregular in the superior vena cava segment, and the angle of the contact area between central CRT and catheter was &lt;180° (all P &lt; 0.05). Age, collateral circulation, and venous stenosis were shown to have significant differences when compared to patients without CRT (all P &lt; 0.05), but there were no significant differences about the sex or catheter insertion site. In addition, age and collateral circulation were the factors found to be significantly associated with thrombosis ( P &lt; 0.05). In particular, the thrombosis was 2.213 times more likely to be found in those patients with collateral circulation (odds ratio = 2.213, 95% confidence interval = 1.236–3.961). Conclusion Chest multi-phase MDCTA can effectively reduce the missed diagnosis and misdiagnosis of central CRT. It is worth paying more attention to the central CRT especially when the collateral circulation is observed.

Characteristic CT angiography findings and clinical value of catheter-related central venous thrombosis in hemodialysis patients with end-stage renal disease

Background Non-invasive imaging technologies for assessing axillary lymph node (ALN) metastasis of breast cancer are needed in clinical practice. Purpose To explore the clinical value of intravoxel incoherent motion (IVIM) and diffusional kurtosis imaging (DKI) for predicting ALN metastasis of breast cancer. Material and Methods A total of 194 patients with pathologically confirmed breast cancer who underwent IVIM and DKI examination were reviewed retrospectively. The IVIM derived parameters of D, D*, and f and DKI-derived parameters of MD and MK were measured. The independent samples t-test was used to compare the parameters between the ALN metastasis and non-ALN metastasis groups. Receiver operating characteristic (ROC) curve analysis was also performed. Results The D and MD in the ALN metastasis group were significantly lower than those in the non-ALN metastasis group ( P &lt; 0.001, P &lt; 0.001). The D*, f, and MK were higher in the ALN metastasis group than in the non-ALN metastasis group ( P = 0.015, P = 0.014, and P = 0.001, respectively). The area under the ROC curve (AUC) of D (0.768) was highest. In addition, the diagnostic efficiency of both IVIM and DKI were higher than that of the conventional MRI ( P = 0.002, P = 0.048). The diagnostic efficiency of IVIM + DKI were higher than that of the IVIM or DKI alone ( P = 0.021, P = 0.004). Conclusion IVIM and DKI can be used for predicting breast cancer ALN metastasis with D as the most meaningful parameter.

Predictive value of intravoxel incoherent motion combined with diffusion kurtosis imaging for breast cancer axillary lymph node metastasis: a retrospective study

Non-invasive modalities for assessing axillary lymph node (ALN) are needed in clinical practice.To investigate the suspicious ALN on unenhanced T2-weighted (T2W) imaging and intravoxel incoherent motion diffusion-weighted imaging (IVIM DWI) for predicting ALN metastases (ALNM) in patients with T1-T2 stage breast cancer and clinically negative ALN.Two radiologists identified the most suspicious ALN or the largest ALN in negative axilla by T2W imaging features, including short axis (Size-S), long axis (Size-L)/S ratio, fatty hilum, margin, and signal intensity on T2W imaging. The IVIM parameters of these selected ALNs were also obtained. The Mann-Whitney U test or t-test was used to compare the metastatic and non-metastatic ALN groups. Finally, logistic regression analysis with T2W imaging and IVIM features for predicting ALNM was conducted.This study included 49 patients with metastatic ALNs and 50 patients with non-metastatic ALNs. Using the above conventional features on T2W imaging, the sensitivity and specificity in predicting ALNM were not high. Compared with non-metastatic ALNs, metastatic ALNs had lower pseudo-diffusion coefficient (D*) (P = 0.043). Logistic regression analysis showed that the most useful features for predicting ALNM were signal intensity and D*. The sensitivity and specificity predicting ALNM that satisfied abnormal signal intensity and lower D* were 73.5% and 84%, respectively.The abnormal signal intensity on T2W imaging and one IVIM feature (D*) were significantly associated with ALNM, with sensitivity of 73.5% and specificity of 84%.

Diagnostic performance of T2-weighted imaging and intravoxel incoherent motion diffusion-weighted MRI for predicting metastatic axillary lymph nodes in T1 and T2 stage breast cancer

Background Prediction of histologic prognostic markers is important for determining management strategy and predicting prognosis. Purpose To identify important features of ultrafast and conventional dynamic contrast-enhanced magnetic resonance imaging (DCE-MRI) that can predict histopathologic prognostic markers in patients with breast cancer. Material and Methods Preoperative MRI scans of 158 consecutive women (mean age = 54.0 years; age range = 29–86 years) with 163 breast cancers between February 2021 and August 2022 were retrospectively reviewed. Inter-observer agreements for ultrafast MRI parameters were analyzed by two radiologists. The qualitative and quantitative MRI parameters were correlated with histopathologic prognostic markers including molecular subtypes and histologic invasiveness. Results Inter-observer agreements for ultrafast MRI parameters were excellent (intraclass correlation coefficients of area under the kinetic curve [AUC], maximum slope [MS], maximum enhancement [ME], and slope = 0.987, 0.844, 0.822, and 0.760, respectively). Triple-negative breast cancers (TNBC) were significantly associated with rim enhancement (odds ratio [OR] = 9.4, P = 0.003) and peritumoral edema (OR = 17.9, P = 0.002), compared to luminal cancers. Invasive cancers were associated with lesion type-mass, increased delayed washout, angiovolume, ME, slope, MS, and AUC, compared to in situ cancers. In regression analysis, the combination of MS (&gt;46.2%/s) (OR = 5.7, P = 0.046) and delayed washout (&gt;17.5%) (OR = 17.6, P = 0.01), and that of AUC (&gt;27,410.3) (OR = 9.6, P = 0.04), delayed washout (&gt;17.5%) (OR = 8.9, P = 0.009), and lesion-type mass (OR = 4.6, P = 0.04) were predictive of histologic invasiveness. Conclusion Conventional DCE-MRI with ultrafast imaging can provide useful information for predicting histologic underestimation and aggressive molecular subtype. MS and AUC on ultrafast MRI can be potential imaging markers for predicting histologic upgrade from DCIS to invasive cancer with high reliability.

The utility of ultrafast MRI and conventional DCE-MRI for predicting histologic aggressiveness in patients with breast cancer

Vascular malformations are rare diseases that are best managed in multidisciplinary vascular anomaly centers (VAC). So far, there are few published data on the logistic structure, patient allotment, and internal workflow in an interdisciplinary VAC.To demonstrate the referral structure and patient allotment in a European VAC.A retrospective cohort study was conducted. All patients treated in the VAC since its establishment in April 2014 until June 2020 were identified. A total of 461 patients were included in this study.Most often, a venous malformation was diagnosed (49.9%), followed by arteriovenous malformations (8.7%) and syndromal vascular malformations (8.2%). Only 45.1% of referral diagnoses were correct. Of referrals, 35.1% were internal on-campus references, 28.0% were external references through physicians in private practice, and 19.7% came via external cooperating hospitals. Of the patients, 17.1% were self-admissions without a referral.The most important referring clinics are internal medicine, pediatric, and vascular surgery. A substantial proportion of referrals came from private practices. Recruiting these cooperation partners should be considered a high priority when establishing a new VAC. The rate of misdiagnosis is high, and more education of fellow clinicians is urgently needed. Therefore, patients with vascular malformations should be referred to and treated in dedicated centers for vascular anomalies.

Performance of a vascular anomaly center: a retrospective analysis of 461 cases over seven years

Background Congenital coronary artery to pulmonary artery fistulas (CPAFs) are extremely rare congenital vascular malformations. Purpose To give a practical approach and consider technical challenges and pitfalls for endovascular embolization of CPAF. Material and Methods Anatomic, technical, and pathophysiologic considerations are given and demonstrated for antegrade and retrograde endovascular embolization of CPAF. Results Antegrade embolization is easier to perform, saves radiation exposure, and is recommended especially in younger patients. In case of a single dominant feeder, antegrade embolization of this feeder might sufficiently treat the CPAF. Retrograde embolization from the pulmonary orifice is technically more challenging but leads to a complete and definite closure of the fistula in one single step. Conclusion Patient age and fistula configuration must be taken into consideration for appropriate treatment approach in CPAF. Prerequisite for successful embolization of CPAF is profound clinical and interventional experience, why we highly recommend to both plan and carry out embolization of CPAF as interdisciplinary procedure.

Strategic approach to embolization of coronary to pulmonary artery fistulas: a technical note

Background Patellar instability (PI) or patellar dislocation (PD) is challenging to diagnose accurately based on medical history and clinical manifestations alone. While X-ray, computed tomography (CT), and magnetic resonance imaging (MRI) are commonly employed for detecting PI or PD, computer vision has not yet been widely utilized for this purpose. Purpose To explore the feasibility of computer vision, specifically the You Only Look Once (YOLO) algorithm, in identifying patellar instability or dislocation. Material and Methods A total of 550 patients (190 diagnosed with patellar instability or dislocation) were divided into a training set (n = 360), validation set (n = 90), and external test set (n = 100). Four indicators were measured on transverse knee MRI scans to determine the presence of patellar instability, and 450 images were labeled using Labelme software. YOLO version 8 (YOLOv8) was refined using these labeled images and validated on 100 unlabeled images. The diagnostic accuracy of YOLOv8 was compared with that of a junior radiologist. Results The sensitivity, specificity, and accuracy of the refined YOLO model and the junior radiologist were 62%, 97%, and 83%, and 62%, 82%, and 74%, respectively. Although the YOLO model demonstrated slightly higher accuracy, the difference did not reach statistical significance ( P = 0.093). The YOLO model required approximately 14.01 ± 10.34 ms to interpret each image, significantly shorter than the 9.55 ± 2.39 s required by the radiologist ( P &lt; 0.001). Conclusion The refined YOLOv8 model is not inferior to junior radiologists in identifying patellar instability or dislocation and offers a significantly faster interpretation time.

YOLOv8 algorithm-aided detection of patellar instability or dislocation on knee joint MRI images

Background 68Ga-labled fibroblast activating protein inhibitor (68Ga-FAPI) represents a new and exciting positron emission tomography-computed tomography/magnetic resonance (PET-CT/MR) radiotracer. Purpose To compare the diagnostic efficacy of 68Ga-FAPI PET CT/MR and 18F-fluorodeoxyglucose (18F-FDG) PET/CT in metastatic lesions of gynecological cancers (GCs). Material and Methods The PubMed, Embase, and Web of Science databases were thoroughly investigated from inception until 22 December 2023. A head-to-head contrast between 18F-FDG PET/CT as well as 68Ga-FAPI PET CT/MR for the assessment of GCs was presented by the included studies. A random variable model was employed to examine the sensitivity in detection of lymph node (LN) and peritoneal metastases (PM). Results The pooled sensitivity for 68Ga-FAPI PET CT/MR and 18F-FDG PET/CT in lymph node metastases (LNM) of GC were 0.98 (95% confidence interval [CI] = 0.86–1) and 0.85 (95% CI = 0.65–0.98), respectively, while the results about peritoneal metastases in ovarian cancer were 0.98 (95% CI = 0.93–1) and 0.71 (95% CI = 0.55–0.86). Compared with 18F-FDG PET/CT, 68Ga-FAPI PET CT/MR exhibited a better sensitivity in peritoneal involvement of ovarian cancer with a relative risk of 0.24 (95% CI = 0.09–0.40) and P = 0.002. Conclusion 68Ga-FAPI PET CT/MR displayed a superior sensitivity over 18F-FDG PET/CT in detecting metastatic lesions of ovarian cancer. However, there was insufficient evidence to favor the superiority of 68Ga-FAPI PET CT/MR in LNM of CC. Further studies are needed for evaluating primary and metastatic lesions of 68Ga-FAPI PET CT/MR in different GC.

Background Radiofrequency ablation (RFA) of subphrenic hepatocellular carcinomas (HCCs) using ultrasound (US) guidance presents a challenge. Purpose To evaluate local tumor progression (LTP) and overall survival (OS) after RFA of right posterior subphrenic HCCs, and to identify the risk factors for LTP and OS after RFA. Material and Methods We screened patients who underwent US-guided RFA for a single HCC &lt;3 cm in the right posterior subphrenic area. Cumulative rates of LTP and OS were compared between multiple- and single-electrode treatments and artificial pleural effusion versus no-effusion groups using Kaplan–Meier curves and the log-rank test. The risk factors for LTP and OS were assessed. Results A total of 36 patients were included. The cumulative 5-year and 10-year LTP rates were 32.9% and 39.6%, respectively, and the corresponding OS rates were 72.2% and 48.7%, respectively. The multiple-electrode group had significantly lower cumulative LTP rates compared to the single-electrode group ( P &lt; 0.001). The group receiving artificial pleural effusion showed a trend towards lower LTP rates than the no-effusion group ( P = 0.076). The albumin-bilirubin (ALBI) grade was the only risk factor for LTP (hazard ratio [HR] = 4.17, 95% confidence interval [CI] = 1.26–14.55; P = 0.020). A lower albumin level was a risk factor for mortality (HR = 0.24, 95% CI = 0.07–0.79; P = 0.020). Conclusion US-guided RFA with multiple electrodes and artificial pleural effusion tends to be linked with lower LTP rates. The ALBI grade and serum albumin level are risk factors for LTP and OS, respectively.
