The Victorian Poisons Information Centre has been based in the Pharmacy Department at the Royal Children's Hospital in Melbourne for 25 years. This article outlines changes in childhood poisoning and service development, and challenges for the future. Mortality from childhood poisoning has decreased with most serious poisonings now involving medicines. Inducing emesis is no longer recommended in the treatment of poisoning. Information sources are now CD‐ROM and Internet based. The Centre has developed a web site to allow users an alternative means of access. A national telephone number for poisons information centres has facilitated inclusion of the number on product labels and advertising of the service. Challenges for the future include developing a relationship with clinical toxicologists, ensuring users understand the benefits of ringing the Centre rather than accessing information on the Internet, enhancing interaction between Australian poisons information centres, and undertaking relevant research.

Aim: To describe a review of drug information activities in a teaching hospital in Mysore, India. Method: Drug information enquiry forms were reviewed for a 6‐month period. Randomly selected enquiries were audited by experienced clinical pharmacists. Results: 370 enquiries were handled at an average of 62 per month. Most enquiries were from doctors (52%) and postgraduate medical students (30% ). Questions most frequently related to administration/dosage (30%) and adverse drug reactions (27% ). Standard textbooks were used for 64% of enquiries. Of the 16 enquiries directly audited, 15 were rated as good or excellent and 1 as ‘can improve’. Conclusions: The service was well used. In developing countries, it is important to establish independent drug information centres beginning with basic resources. Output should be monitored with quality assurance programs.

Review of a Drug Information Service in an Indian Teaching Hospital

Aim: To investigate the use of long‐term antiarrhythmic (rhythm‐control [RHY] and rate‐control [RAC]) therapy for the management of atrial fibrillation (AF) in elderly patients. Method: Data were collected retrospectively from the medical records of 255 patients with AF who were ≥65 years of age and were admitted to Royal North Shore Hospital in the period 1 July 1996 to 30 June 1997. Results: Overall, 192 (75%) patients were discharged on antiarrhythmic therapy for the long‐term management of AF. Sixty‐six patients (26%) received RHY only, 90 (35%) received RAC only, 36 (14%) were prescribed the combination of RHY + RAC therapy, and 63 (25%) received no therapy. Patients ≥80 years of age were equally likely to receive longterm antiarrhythmic therapy as those &gt;80 years (74% versus 77%, p = 0.65), but a significantly lower proportion of them received RHY only than in those &gt;80 years (17% versus 34%, p = 0.001). The preferred option in older patients was RAC only (42% ). Cardiology patients were more likely to receive RHY ± RAC therapy (53%) than Aged Care (16%) or General Medicine patients (16%; p &gt; 0.05). Logistic regression analysis indicated that old age and medical specialty were significant predictors for the choice of agent: patients ≥80 years were 2.2 times more likely to receive RAC only in preference to RHY only therapy than their younger counterparts (p = 0.044). Conclusion: Long‐term rhythm‐control therapy (e.g. amiodarone, sotalol) is potentially underutilised in AF patients ≥80 years of age. Reliance on the use of rate‐controlling agents such as digoxin persists in clinical practice.

A Survey of Long‐Term Antiarrhythmic Therapy in Elderly Patients with Atrial Fibrillation

Objective: This article describes a project aimed at resolving inefficiencies associated with the use of the Pharmaceutical Benefits Scheme (PBS) in private hospitals. Method: Six alternative models were formulated and evaluated using quantitative and qualitative means. The preferred model was implemented for trial. Data sources: Drug utilisation records from medical and pharmacy histories of almost 2500 patients in 5 private hospitals, and focus groups and questionnaire surveys of medical, nursing and pharmacy personnel were used. Results: The model eliminated legal and operational problems associated with outstanding prescriptions, ensured patients received PBS subsidies to which they were entitled and was favoured by all stakeholder groups. Conclusion: The model should be available for implementation in all hospitals using the PBS.

Use of the Pharmaceutical Benefits Scheme in Private Hospitals: 2. An Alternative Model

Aim: To develop a practical guide for pharmacists for the provision of clinical pharmacy services that would: 1. facilitate the delivery of a uniform standard of care; and 2. enhance communication between pharmacists and other members of the medication team. Method: A generic pharmaceutical care plan based on the ‘SHPA Standards of Practice for Clinical Pharmacy’ was developed as an appropriate framework for the model. The plan is outcome oriented. It uses the patient's medication chart to document clinical information and activities and follows the patient from admission to discharge. The medication chart was chosen as the medium for documenting pharmaceutical care following a survey of the medical staff. Previously this information was recorded in patient profiles in individual pharmacists' ward folders. Results: Major outcomes included a change in clinical practice, improved discharge planning, visibility of the pharmacists' contribution to patient care, and an increase in the pharmacists' profile within the hospital. The plan also proved to be a useful tool for performance management and education purposes, and was well accepted by pharmacists. Conclusion: A formalised pharmaceutical care plan is a practical and efficient model for providing and documenting clinical pharmacy services. Using the medication chart as the medium for documenting clinical services supports continuity of care in the hospital, improves productivity and streamlines the discharge process.

The Medication Chart as an Integral Tool in the Pharmaceutical Care Plan

Objective: To evaluate the potential impact on appropriateness of discharge medication if discharge prescriptions were reviewed by the ward pharmacist before being dispensed. Method: Over a four‐week period discharge prescriptions were reviewed by either the ward pharmacist involved in the patient's care (cases) or a dispensary pharmacist who had no knowledge of the patient's care (controls). A second review with the availability of patient records was carried out to identify additional queries that should have been raised prior to dispensing. Results: Fifty‐five prescriptions (29 case, 26 control) containing 279 items (140 case, 139 control) were reviewed. Forty‐four queries (23 case, 21 control) were found on initial checking of the prescription, 12 more queries (1 case, 11 control) were found on the second review of the prescription. There were 13 potentially clinically significant queries in the case group, 12 (92%) were made during initial prescription review. There were 23 potentially clinically significant queries in the control group, only 12 (52%) were made on initial prescription review. Changes to the prescription resulting from potentially clinically significant queries prior to dispensing occurred in 19 (79%) queries (10 case, 83%; and 9 control, 75% ). Of queries raised prior to dispensing in the control group, 18 involved contacting hospital staff for resolution where they could have been resolved using the inpatient prescription chart, notes or the patient themselves. This compared to only one in the case group. Conclusion: Review of discharge prescriptions by the ward pharmacist involved in the patient's care optimises the appropriateness of discharge medication and the efficiency of the discharge process.
