Even early on thromboembolic events were observed in patients with systemic lupus erythematosus (SLE) until the antiphospholipid syndrome (APS) was described in the 1980s as an independent disorder. The APS is a systemic autoimmune disease often overlapping with SLE in which antiphospholipid autoantibodies, including lupus anticoagulant, can cause a hypercoagulation state, which clinically by definition is manifested as arterial and venous occlusions or pregnancy complications. The pathophysiology has not yet been entirely delineated and the clinical spectrum of associated concomitant manifestations is large. As the mortality is increased with SLE and simultaneous APS, focused diagnostics and risk assessment are indispensable. According to the recently published recommendations of the European League Against Rheumatism the therapeutic strategy comprises individualized secondary prevention of thromboembolic complications by means of anticoagulation (with unaltered importance of vitamin K antagonists) and thrombocyte aggregation inhibition, usually lifelong. Statins and antimalarial drugs are recommended for vascular protection while immunosuppressive treatment has not so far been sufficiently proven for APS but remains the subject of current research.

[Lupus and thrombophilia : Antiphospholipid syndrome].

The aim of the present work is an overview of current concepts in the treatment of osteoporotic vertebral fractures, risk factors, clinical presentation and conservative and operative management. Osteoporotic vertebral fractures are a widespread problem, affecting between 700,000 and 1.5 million adults annually in the USA alone. Osteoporotic vertebral fractures can cause severe physical impairment, including back pain, dysfunction, and progressive thoracic kyphosis. Mortality after osteoporotic vertebral fractures is still increased compared to age-matched controls. In the reality of care, advocates of purely conservative therapy as well as advocates of surgical treatment can be found all over the world. In summary, understanding the risk factors, appropriate clinical evaluation and treatment strategies are crucial. When surgery is indicated, balloon kyphoplasty shows significantly better pain reduction and lower mortality compared to nonsurgical treatment.

[Osteoporotic vertebral fractures-From diagnosis to treatment].

骨质疏松性椎体骨折-从诊断到治疗。

Zusammenfassung Das VEXAS-Syndrom ist eine neu identifizierte autoinflammatorische Systemerkrankung. Das Akronym VEXAS steht hier für V acuoles, E 1 enzyme, X -linked, A utoinflammatory, S omatic. Die Erkrankung beruht auf einer erworbenen somatischen Mutation des UBA1 -Gens. Dieses kodiert für das E1-Enzym, welches wiederum für die Ubiquitinierung von Proteinen verantwortlich ist. Aufgrund der Lage des Gens auf dem X‑Chromosom betrifft die Erkrankung überwiegend Männer (in der zweiten Lebenshälfte). Die Patienten weisen eine Plethora an inflammatorischen klinischen Symptomen – oft mit Überlappung von hämatologischen, dermatologischen und rheumatologischen Krankheitsbildern – auf. Insbesondere das Vorliegen von zytoplasmatischen Vakuolen im Knochenmark ist charakteristisch. In dieser Arbeit berichten wir über den klinischen Fall eines VEXAS-Patienten und geben einen Überblick über die Pathophysiologie, Klinik und Diagnostik des Erkrankungsbildes.

Antineutrophil cytoplasmic antibody (ANCA)-associated vasculitides (AAV) are autoimmune inflammatory small-vessel disorders with potentially life-threatening organ manifestations. Recent disease definitions and classification criteria allow distinction between granulomatosis with polyangiitis (GPA), eosinophilic granulomatosis with polyangiitis (EGPA), and non-granulomatous microscopic polyangiitis (MPA). The discovery of ANCA-autoantibodies directed against proteolytic enzymes of neutrophil granules-has enabled earlier diagnosis of AAV and paved the way to stage-adapted treatments. ANCA testing initially relied on different immunofluorescence patterns, i.e., cytoplasmic ANCA (C-ANCA) vs. perinuclear ANCA (P-ANCA), in ethanol-fixed neutrophils. This is nowadays outperformed by well-standardized immunoassays against the ANCA target antigens proteinase 3 (PR3) and myeloperoxidase (MPO) for the diagnosis of small-vessel vasculitides. The discovery of ANCA has contributed substantially to unravelling the pathogenesis of AAV, which comprises neutrophil degranulation, NETosis with concurrently amplified ANCA antigen presentation, and intra- and transmural vascular inflammation involving the alternative complement system in susceptible individuals. There is a genetic disposition concerning certain HLA alleles and polymorphisms of the proteinase 3 gene. Furthermore, epigenetic modifications impact on disease activity and relapse. During follow-up, the ANCA titer is not a reliable mirror of disease activity; however, PR3-ANCA positivity is associated with a greater likelihood of relapse and a better treatment response to rituximab as compared to cyclophosphamide/azathioprine. Within the past 60 years, the discovery of ANCA has revolutionized the ability to diagnose, understand, classify, and treat AAV in a targeted manner.

Implementation of the new DGRh S2e guideline on diagnostics and treatment of adult-onset Still’s disease in Germany

Zusammenfassung Die Musterweiterbildungsordnung definiert die Weiterbildungsinhalte zur Erlangung der Facharztbezeichnung Innere Medizin und Rheumatologie. Kriterien zur Erteilung der Weiterbildungsbefugnis liegen nicht vor. Das Positionspapier beschreibt die von der Deutschen Gesellschaft für Rheumatologie und Klinische Immunologie (DGRh) vorgeschlagenen Kriterien, die bei der Erteilung einer Weiterbildungsbefugnis im Gebiet Innere Medizin und Rheumatologie und für die Bemessung ihres zeitlichen Umfangs zugrunde gelegt werden sollten. Dabei fungieren die Musterweiterbildungsordnung 2018 und der fachlich empfohlene Weiterbildungsplan als Basis. Anhand der Kriterien kann die Weiterbildungsbefugnis zur Facharztweiterbildung Innere Medizin und Rheumatologie standardisiert, abgestuft und transparent im gesamten Bundesgebiet vergeben werden. So wird eine qualitätsoptimierte Weiterbildung in der Rheumatologie ermöglicht, die an zukünftige Entwicklungen des Faches angepasst werden kann.
