We have evaluated a new enzyme immunoassay (EIA) to measure IgG antibodies (Abs) to HIV in patients with AIDS, AIDS-related complex (ARC), AIDS contacts (AC), and low risk controls. Twenty-nine (94%) of 31 AIDS, 27 (96%) of 26 ARC, 12 (54%) of 22 AC, and 1 (2%) of 60 control patients were anti-HIV Ab positive by this assay. Positive results were confirmed by Western blot analysis. The EIA for anti-HIV evaluated in this study is sensitive and specific in identifying individuals who have been infected by this retrovirus.

SENSITIVITY AND SPECIFICITY OF AN HIV ANTIBODY ASSAY

Four different, commercially available ELISA tests for the detection of antibodies against LAV/HTLV III were evaluated for specificity and sensitivity. The relative specificity of the kits was determined by investigating a test panel of 76 sera collected from asymptomatic or symptomatic homosexual men. Completely concordant results were obtained for sera from asymptomatic male homosexuals (11% positive for Anti-LAV/HTLV III) or from patients with the AIDS-related complex (91% positive for Anti-LAV/HTLV III). Differences between the ELISA test kits, however, were observed with sera obtained from patients with AIDS. While Anti-LAV/HTLV III was detected by the Abbott, Electro-Nucleonics, and Organon test in all 17 sera from AIDS patients, the Pasteur test failed to detect Anti-LAV/HTLV III in 7 consecutive sera from an individual patient with late-stage AIDS. The relative sensitivity of the ELISA tests was determined by endpoint titration of confirmed Anti-LAV/HTLV III positive sera from donors of different risk groups for AIDS. The titration experiments demonstrated that the Abbott test clearly was the most sensitive of the ELISA tests studied, followed by the Electro-Nucleonics, Pasteur, and Organon test. The results further indicate that most of the differences of specificity and sensitivity observed between the Anti-LAV/HTLV III tests could be abolished by a modified definition of minimum positive absorbance values.

The 2, 5'-oligoadenylate dependent binding protein activity was measured in peripheral blood lymphocyte extracts of AIDS and pre-AIDS and compared to healthy heterosexual controls. The binding activity in lymphocytes from AIDS (mean, 0.71 fmole/10(6) cells) and pre-AIDS (mean, 0.66 fmole/10(6) cells) was approximately 65% lower than that found in controls (mean, 2.41 fmole/10(6) cells) suggesting that this enzymatic activity may be a useful biochemical marker for the diagnosis of AIDS.

Thirty-nine patients from the Nebraska Regional Hemophilia Center were studied for the prevalence and titers of antibodies to HTLV-III/LAV, and for clinical symptoms of possible progression toward the acquired immunodeficiency syndrome (AIDS). Fourteen of 26 (54%) patients with hemophilia A were positive for HTLV-III/LAV antibodies as determined by immunofluorescence and Western blotting. Retrovirus was detected in cultured lymphocytes of two out of three seropositive individuals by reverse transcriptase assay and molecular hybridization with cloned HTLV-III/LAV probe. Of the twelve factor VIII-deficient patients who were seronegative, one had received only heat-treated factor VIII concentrates, six only cryoprecipitate or fresh frozen plasma, two prothrombin complex concentrates, one had not been transfused since 1983, and two had never been transfused. None of the patients treated only with factor IX concentrates, volunteer donor plasma, or cryoprecipitate had HTLV-III/LAV antibodies. In 12 of 14 seropositive hemophiliacs, titers of serum antibodies to HTLV-III/LAV ranged from 1:1,280 to 1:10,240, indicating a strong immune response against HTLV-III/LAV antigens and/or persistent infection with the virus. In spite of the possible exposure to HTLV-III/LAV during the past four years, none of the patients in this group had AIDS. Whether or not this group of patients has developed immunity to the AIDS retrovirus remains to be determined.

HTLV-III/LAV ANTIBODIES AND VIRUS IN HEMOPHILIA PATIENTS IN NEBRASKA: SURVEY AND INITIATION OF A PROSPECTIVE STUDY

Recent research has identified the human T-lymphotropic virus type III (HTLV-III) as a probable etiologic agent of the acquired immune deficiency syndrome (AIDS). This has prompted the U.S. Public Health Service to recommend that all blood used for transfusions or in the manufacture of blood products be screened. An enzyme-linked immunosorbent assay (ELISA) has been approved for use as a screening test. From the perspective of the low-risk blood donor, however, our analysis indicates that the expected utility of no-testing may exceed that of testing. This is primarily due to the risk of a false-positive test result. It follows that informed low-risk individuals may be hesitant to donate blood. We support the discarding of blood that tests positive on ELISA but, to decrease donor risk, a positive confirmatory test, such as the Western blot, should be considered as necessary before the testing outcome is treated as positive from the donor's perspective. Additionally, individuals should be given the option to donate blood without being told the testing outcome.

Serum from a patient suspected of having AIDS showed positive ELISA tests but the diagnostic p 41 band was absent and the p 24 band was barely discernible on a Western blot. Before another Western blot was performed lentivirions were detected between dendritic reticulum cells in a lymph node biopsy. It is suggested that samples of biopsies of lymph nodes from patients with or at risk for AIDS should be embedded in resin for future ultrastructural study if indicated.

DIAGNOSIS OF HTLV-III INFECTION BY ULTRASTRUCTURAL EXAMINATION OF GERMINAL CENTERS IN LYMPH NODE—A CASE REPORT

The effects of suramin, an antiparasitic agent, upon in vitro hepatitis B surface antigen production by the human hepatoma cell line PLC/PRF/5 and hepatitis B virus associated DNA polymerase activity in the serum of a chronically infected patient were examined. Treatment with suramin resulted in decreases in hepatitis B surface antigen production and hepatitis B-virus associated DNA polymerase activity. The decrease in hepatitis B surface antigen production was paralleled by a general decrease in hepatoma cell viability and cellular protein synthesis. Although the inhibitory effects of suramin for hepatitis B virus appear to be nonspecific as demonstrated in these two in vitro systems, the recently announced trial of suramin for the treatment of the acquired immunodeficiency syndrome should afford an unusual opportunity to evaluate the effectivenss of suramin in the treatment of chronic hepatitis B virus infection.
