Dysregulation in the circadian system induced by variants of clock genes has been associated with type 2 diabetes. Evidence for the role of cryptochromes, core components of the system, in regulating glucose homeostasis is not supported by CRY1 candidate gene association studies for diabetes and insulin resistance in human, suggesting possible dietary influences. The purpose of this study was to test for interactions between a CRY1 polymorphism, rs2287161, and carbohydrate intake on insulin resistance in two independent populations: a Mediterranean (n = 728) and an European origin North American population (n = 820). Linear regression interaction models were performed in two populations to test for gene–diet interactions on fasting insulin and glucose and two insulin-related traits, homeostasis model assessment of insulin resistance (HOMA-IR) and quantitative insulin sensitivity check index (QUICKI). In addition, fixed effects meta-analyses for these interactions were performed. Cohort-specific interaction analyses showed significant interactions between the CRY1 variant and dietary carbohydrates for insulin resistance in both populations (p < 0.05). Findings from the meta-analyses of carbohydrate–single nucleotide polymorphism interactions indicated that an increase in carbohydrate intake (% of energy intake) was associated with a significant increase in HOMA-IR (p = 0.011), fasting insulin (p = 0.007) and a decrease in QUICKI (p = 0.028), only among individuals homozygous for the minor C allele. This novel finding supports the link between the circadian system and glucose metabolism and suggests the importance this CRY1 locus in developing personalized nutrition programs aimed at reducing insulin resistance and diabetes risk.

Chronotype has been extensively linked to various psychological outcomes, including suicide, which significantly impacts the mortality rate among young adults worldwide. Research on the extent of these links is still ongoing. This cross-sectional study aimed to explore the complex relationships between chronotype, internet addiction, problematic social media use (PSMU), psychological pain, and suicidality in a student population. A total of 571 Turkish students (65.3% female, mean age 20.3 ± 1.63 years) completed the following scales: the Suicide Probability Scale (SPS), the Mee-Bunney Psychological Pain Assessment Scale (MBPPAS), the Social Media Disorder Scale (SMDS), the Internet Addiction Test (IAT), the Morningness – Eveningness Questionnaire (MEQ), and the Hospital Anxiety and Depression Scale (HADS). The results showed that the relationship between chronotype and suicidality was partially mediated by SMDS and MBPPAS but not by anxiety, depression, or IAT. These findings suggest that psychological pain and PSMU may be important factors that contribute to suicidality in evening-type individuals. These findings have significant implications for the development of interventions aimed at reducing suicidality among evening-type individuals. By addressing the underlying factors of psychological pain and PSMU, it may be possible to mitigate the increased risk of suicidality among this population.

The relationship between chronotype, psychological pain, problematic social media use, and suicidality among university students in Turkey

There is evidence that circadian misalignment is linked with an increased risk of stroke and that circadian preference is linked with rehabilitation outcomes after stroke. Circadian preferences refer to preferences based on individuals' characteristics in respect of the timing of physiological functions that are typically assessed by self-report questionnaires. This study set out to explore this relationship by examining whether there was an association between circadian preference and stroke characteristics in a sample of stroke outpatients, considering the time of stroke onset, the topography of the stroke and the resulting disability. We also examined whether sleep complaints (snoring, insomnia, sleep apnea) were associated with circadian preferences (i.e., morning-, evening-, and intermediate-types). We also compared circadian preferences and comorbidities in this sample, matched by age and sex, with those of healthy controls who took part in an epidemiological study (EPISONO) comprising a representative sample of the population of the city of São Paulo, Brazil. Most of our sample in both groups were morning-type, and in the stroke group, ischemic stroke was the most common type. There was an increased risk for sleep apnea among evening-types, and a higher prevalence of diabetes in the evening- and intermediate-types. We found no association between circadian preference and the time of stroke onset, or with wake-up stroke. We found a low prevalence of evening-type participants in our sample of controls (2.9%) and in the stroke group (1.7%). This investigation showed a similar circadian preference (morning-types) in stroke outpatients and the general population.

Circadian preference and stroke characteristics: A descriptive study

Shift workers frequently experience alterations in their circadian rhythms, which are correlated with variations in hematological parameters. Changes in blood cells may be related to an individual's health status. Therefore, this study aimed to compare the relationship between shift work and changes in blood cells among a group of healthcare workers in Sri Lanka. A comparative cross-sectional study was conducted among healthcare workers, recruited by a stratified random sampling technique. Socio-demographic data were collected using a structured questionnaire. Venous blood samples were obtained and analyzed for the determination of total and differential blood cell counts. Descriptive statistics were used for the analysis of sociodemographic and hematological parameters. A sample of 37-day workers and 39 shift workers were included in the analysis. The mean ages (years) were not significantly different between the groups (36.8 ± 10.8 vs 39.1 ± 12.0; P = 0.371). Shift employees showed a significantly higher total mean white blood cell count (WBC) of 7548.75 mm-3 compared to day workers' 6869.19 mm-3 (P = 0.027). They also had higher mean absolute counts for all different WBC types (Neutrophils: 3949.2 vs 3557.7 , Lymphocyte: 2756.5 vs 2614.2 , Eosinophil: 317.6 vs 233.4 , Monocytes: 491.63 vs 432.51 , Basophils: 31.68 vs 29.22 ). Shift employees exhibited higher WBC counts than day workers at the same level of work experience. The length of shift work exposure revealed a positive link with neutrophil (r = 0.225 ) and eosinophil counts (r = 0.262 ), whereas these correlations were negative for day workers. Shift workers were associated with higher WBC counts in healthcare workers compared to their day-working counterparts.

The effect of shift work on different hematological parameters among healthcare workers

Shift workers may experience disrupted sleep-wake or circadian cycles which, in turn, may lead to major health problems such as hypertension, obesity, and diabetes. Research indicates that shift workers have increased levels of leukocytes. Data from the National Health and Nutrition Examination Survey (2005-2010) examined the relationship between self-reported current shiftwork and leukocytes with adjustment for important confounders. Using linear regression, we found no association between shiftwork and leukocyte counts. Those not working had elevated levels compared to day workers but values in both groups were still within normal ranges. Additional research is needed to fully understand how shiftwork influences changes in leukocyte counts.

Circadian misalignment and disrupted rest-activity rhythms have been linked to adverse health and educational outcomes, yet few studies have considered their relationships with economic outcomes. We investigate associations between multiple accelerometer-measured circadian misalignment traits (Composite Phase Deviation and the Sleep Regularity Index) and rest-activity rhythm traits (Inter-daily Stability, Intra-daily variability and relative amplitude), with occupational attainment, measured using the average wage paid to an individual's occupation. We use data from 20 356 UK Biobank participants who wore an accelerometer (2013-16), provided employment data for the year they wore the accelerometer, and supplied covariate data at recruitment (2006-10). We use linear regression, with incremental adjustments for four sets of covariates, and stratify our analysis by sex. Our findings provide mixed evidence on the relationship between circadian misalignment and occupational attainment, varying by the measure of circadian misalignment used, and sex. We find fragmented rest-activity rhythms to be associated with higher occupational attainment, which is counterintuitive. Since circadian misalignment is a modifiable trait, our results suggest potential benefits of policies aimed at reducing circadian misalignment, such as altering work schedules and reducing bright light exposure in the evening. Further research is needed to elucidate the mechanisms through which rest-activity rhythms may impact economic outcomes.

Cross-sectional relationships of circadian misalignment and rest-activity rhythms with occupational attainment in UK Biobank participants

This study investigated the impact of age on morningness-eveningness (ME) and its inter-individual variability, with a focus on sex-specific patterns. A sample of 2890 participants aged 12-94 years (55.85% female) completed the Composite Scale of Morningness. Multiple linear regression analyses revealed a significant Age × Sex interaction, indicating distinct age-dependent patterns for males and females in both ME and its inter-individual variability. We conducted segmented regression analyses to explore these dynamics further and identify breakpoints. Eveningness increased across adolescence, with both males and females reaching peak lateness at 20 years. Morningness increased thereafter until 64.94 years for females and until 59 years for males. Following these sex-specific breakpoints, morningness remained constant for females and increased at a reduced rate for males. After the age of 48, males exhibited greater morningness than females. Inter-individual variability in ME (VME) changed with age, increasing until 33 years for men and 36.80 years for women, followed by a decrease for both sexes. From 50.30 years onward, females showed greater VME compared to men. This study suggests that ME and VME shift across age, and differences between sexes were observed from middle age onwards. Diurnal preferences are likely influenced by internal and environmental variables.

Examining sex differences in morningness-eveningness and inter-individual variability across years of age: A cross-sectional study

Physical inactivity may exacerbate the adverse health effects associated with shift work. We investigated the association of shift work with leisure-time physical inactivity (LTPI). A cross-sectional analysis included 33 983 adults from National Health Interview Survey who self-reported their work schedule and leisure-time physical activity. Participants were classified in to two groups as either (a) inactive or (b) insufficiently/sufficiently. Prevalence ratios were derived using SUDAAN. Analyses were stratified by sex. In this sample of U.S. workers, 27% were shift workers, 26% were physically inactive, and 47% did not meet recommended levels of leisure-time physical activity. Inactivity level was 23% higher in women than in men among shift workers but did not differ by sex among daytime workers (PR = 1.02, 95% CI: 0.96-1.07). Evening or night work was associated with higher LTPI among women but not men. LTPI was 17% higher in women working the night shift, and 24% higher in those on the evening shift compared to those working the daytime shift. These findings suggest that shift work may contribute to increased LTPI, with effects that may be particularly pronounced in women. Interventions addressing LTPI among shift workers may help mitigate the adverse health effects that have been linked to shift work in prior studies.

Aberrant light/dark (LD) cycles are prevalent in modern society due to electric light usage, leading to mood disorders from circadian disruption or misalignment. However, research on the physiological and behavioral effects of LD variations on brain neurotransmitters is limited. We investigated the effects of extreme LD cycles on body weight (BW), core body temperature (Tcore), locomotor activity (ACT), emotional behaviors, and monoamine levels (noradrenaline [NA], dopamine [DA], and serotonin [5-HT]) in male Wistar rats that were exposed to 1 month of either long light phase (20 L:4D), long dark phase (4 L:20D), or normal (12 L:12D) LD cycles. The 20 L:4D rats exhibited blunted rhythms, with decreased amplitude and advanced/delayed acrophase in Tcore and ACT, alongside increased BW. The 4 L:20D rats showed circadian misalignment, with increased/decreased amplitude in Tcore or ACT and delayed acrophase in Tcore and ACT, also gaining BW. In the 20 L:4D group, NA and 5-HT levels decreased in the suprachiasmatic nucleus and amygdala, respectively, while the 4 L:20D group had increased DA and 5-HT levels in the caudate putamen and dorsomedial hypothalamus, respectively. Open field and social interaction tests indicated anxiety-like behaviors in both test groups. Overall, each extreme LD cycle affected Tcore, ACT amplitude, acrophase, and monoamine levels differently, inducing anxiogenic responses.

Influence of extreme light/dark cycles on monoamine levels, physiological indices, and emotional behaviors in rats

This study investigated the quality and duration of sleep among shift-working nurses. In this cross-sectional survey, nurses from eight units across two tertiary hospitals in South Korea's capital region participated. We collected data on sleep quality and duration from 117 nurses. We used the Mann-Whitney test, analysis of variance, or the Kruskal-Wallis test to analyze differences in sleep quality and duration across different shifts. The nurses generally reported low sleep quality, with sleep disturbances being the most common issue. The worst subjective sleep quality was associated with the night shift, while the shortest sleep duration was reported for the day shift. Nurses working two consecutive night shifts reported the poorest sleep quality and shortest sleep duration. The quality and duration of sleep for shift-working nurses varied depending on the shift type. There were also differences based on the arrangement of multiple consecutive shifts. The findings of this study suggest that planning for nurses' shift work should consider sleep quality and duration. Specifically, nurses working the night shift reported poor sleep quality and duration, indicating a need for strategies to ensure they get sufficient rest after their shifts.
