BackgroundTo evaluate the morphological features of corneal endothelial cells and their relationship with the stage of retinopathy in diabetes mellitus (DM).MethodsPatients with type 2 DM and age‐matched controls were included in this prospective, cross‐sectional study. Patients with diabetic retinopathy (DR) were divided into no‐DR, non‐proliferative DR and proliferative DR based on the fundus findings. Endothelial measurements were obtained using specular microscopy (Topcon SP‐3000P, Japan). Endothelial cell density, average cell area, co‐efficient variation of cell area and percentage of hexagonal cells were evaluated. Central corneal thickness (CCT) was measured using an ultrasound pachymeter. Endothelial cell parameters and CCTs of DM and control groups were compared and subgroup analysis of the patients with DM was performed.ResultsOne hundred and twenty patients (mean age 59.5 ± 8.1-years) were in the DM group, 112 patients (mean age 57.3 ± 7.2-years) were in the control group. Both the endothelial cell density and the hexagonal cell rate were lower, while the CCT was higher in the DM group. There were no significant differences between these two groups in terms of average cell area and co‐efficient variation of cell area. Significant differences were detected between endothelial cell density values of subgroups, and endothelial cell density decreased as the stage of the DR increased. The average cell area, co‐efficient variation of cell area and CCT measurements were similar across subgroups. Hexagonality values were significantly different between subgroups, with the lowest ratio of hexagonal cells in the proliferative DR group.ConclusionAdditional precautions should be taken to reduce the risk of endothelial decompensation prior to intraocular surgery, especially in patients with proliferative DR.

Corneal endothelial changes in type 2 diabetes mellitus relative to diabetic retinopathy

The choroid is a vascular tissue which plays a range of critical roles in the normal physiology of the eye, such as supplying the outer retina with oxygen and nutrients and the regulation of intraocular pressure. There is also substantial evidence, particularly from animal studies, that the choroid plays an important role in the regulation of eye growth and the development of common refractive errors like myopia. In recent years, advances in optical coherence tomography technology have improved our ability to image and measure the choroid in the human eye. Research using this technology over the past decade has dramatically improved our knowledge of the normal choroid, and its potential role in the regulation of eye growth and refractive error development. This review aims to provide an overview of recent work examining the normal human choroid, its changes with myopia and the possible role of the choroid in the mechanism regulating eye growth. Studies have demonstrated that choroidal thinning accompanies the development and progression of myopia, and have established a close link between eye growth and choroidal thickness changes. Dramatic thinning of the choroid is seen with high myopia, and associations are also observed between choroidal thinning and reduced vision, and the development of retinal pathology associated with high myopia. In the short‐term, environmental factors known to be associated with myopia development and more rapid eye growth typically lead to a thinning of the choroid, whereas factors linked to a slowing of eye growth are typically associated with short‐term choroidal thickening. Collectively, these findings suggest that the choroid is an important biomarker of eye growth in the human eye, and additional research to better understand the human choroid is likely to further our knowledge of the signals and pathways regulating eye growth, myopia development and progression.

Current understanding of the anatomy, function and performance of the accommodative system of the young, adult human eye is outlined. Most major current models of the accommodative mechanism are based on Helmholtz's original ideas but, despite of a growing volume of related research, uncertainty continues over the relative contributions made to the overall mechanism by different ocular structures. The changes with age are then discussed. Although the amplitude of accommodation decreases steadily from later childhood, the speed and accuracy of the system within the available amplitude are little impaired until the age of about 40, when the amplitude falls below that needed for normal near work. A review of the available evidence on age‐related change in the lens, capsule, ciliary body and other relevant ocular structures confirms that geometric and viscoelastic lenticular changes play major roles in the progressive loss of accommodation. Other factors may also contribute in an, as yet, unquantified way and a full understanding of the origins of presbyopic change remains elusive.

Clinical relevance Change in intraocular pressure during acute ocular compression is related to aqueous humour dynamics. Monitoring intraocular pressure (IOP) change throughout ocular compression has potential to evaluate aqueous outflow facilities.Background Recent studies have monitored lamina cribrosa deformation using optical coherence tomography during ocular compression. IOP was measured only once immediately after ocular compression. This study aimed to evaluate IOP changes during and after ocular compression and compare the differences between low and high myopia.Methods Two groups of young, healthy adults were age-matched and underwent ocular compression. IOP was measured at baseline and monitored during a 2-min ocular compression followed by a 10-min recovery phase. Rebound tonometry was used and applied at 30-s intervals.Results Thirty low and 30 high myopes (60 right eyes) were included in the study. They had similar baseline IOP at 14.9 mmHg. IOP was elevated to 21.7 ± 3.8 mmHg and 22.3 ± 4.2 mmHg for the low and high myopic group, respectively (p = 0.877). Low myopes had faster IOP decay during ocular compression at −3.24 mmHg/min than high myopes at −2.58 mmHg/min (p = 0.0528). The IOP dropped below the baseline level after the release of the compressive force. Low myopes had IOP that returned to baseline levels faster (at 360 s) than high myopes (at 510 s).Conclusion Measuring IOP once immediately after ocular compression could under-estimate the effect of IOP elevation during ocular compression. Further studies are required regarding IOP changes from ocular compression in aqueous humour dynamics.

Effect of endonasal dacryocystorhinostomy on conjunctival morphology and visual quality

Approximately 40,000 Australians have received a donor corneal tissue transplant over the last 40 years, with the primary indications being keratoconus, Fuchs' endothelial dystrophy, bullous keratopathy, and failure of a prior corneal transplant. Although corneal cross-linking and rigid contact lenses have emerged as alternative strategies for the management of keratoconus, the demand for donor corneas is increasing in-line with the ageing population in Australia. Moreover, owing to the lack of tissue banking resources in less-developed countries, the global demand for donor corneas exceeds supply by 70-fold. These supply issues, combined with evolving tissue banking and surgical techniques, have led to the emergence of new strategies for the storage, processing and implantation of corneal cells and tissues. Organ culture techniques have been developed that support the storage of donor corneas for up to 30 days, facilitating improvements in tissue supply and surgery scheduling. Bespoke surgical methods have been developed that are tailored to the requirements of specific conditions, allowing reductions in both the volume of tissue required to be transplanted and the size of the necessary surgical incision. Further efficiencies and improvements in patient care may be achieved via exploitation of cell culture technologies as exemplified through use of cultured corneal epithelial cells for the treatment of limbal stem cell deficiency. Promising progress has also been made in developing a cultured corneal endothelial cell therapy for patients with corneal endothelial dysfunction. These evolving strategies are discussed with respect to their potential impact on the clinical presentation and management of patients who have received an implant of donor corneal tissue or cells.
