Background: Synovial fibroblasts of temporomandibular joint (TMJ) are poorly characterized, although they have important roles in progression of temporomandibular disorders (TMD). In this study, we investigated responses of synovial fibroblasts to interleukin (IL)‐1β. Methods: We examined gene expression profiles of synovial fibroblasts in response to IL‐1 β , using Affymetrix GeneChip. Regulated upon activation normal T‐cell expressed and secreted (RANTES) gene expression was confirmed by polymerase chain reaction (PCR) and real‐time PCR. RANTES protein levels were measured by enzyme‐linked immunosorbent assay (ELISA). Results: The RANTES was preferentially up‐regulated in synovial fibroblasts by IL‐1 β . The increase in RANTES gene expression in response to IL‐1 β was confirmed by PCR and real‐time PCR. Protein level of RANTES in synovial fibroblasts was also increased by IL‐1 β . Conclusions: The RANTES, a cc‐type chemokine, has chemotactic effects on lymphocytes and monocytes. Increased gene expression and protein production of RANTES in synovial fibroblasts, in response to IL‐1 β , may play an important role in recruitment of inflammatory cells into synovium and progression of synovitis in TMD.

Interleukin‐1β increases RANTES gene expression and production in synovial fibroblasts from human temporomandibular joint

Abstract Background A major challenge in the clinical management of oral cavity squamous cell carcinoma is local relapse. Even when surgical margins are tumor‐free, local relapses occur frequently, and relapse prediction by histology remains suboptimal. In leukoplakia, an oral potentially malignant disorder, the presence of architectural dysplasia is a critical risk factor for malignant transformation. This study aimed to investigate whether the presence of architectural dysplasia in oral cavity squamous cell carcinoma surgical margins is a risk factor for local relapse. Methods Hematoxylin and eosin‐stained slides of resection margins from a consecutive cohort of surgically treated patients diagnosed with stage I–IV oral cavity squamous cell carcinoma between 2008 and 2014 were assessed for the presence of architectural dysplasia ( N = 311). Five‐year local relapse‐free survival rates of oral cavity squamous cell carcinoma with architectural dysplasia were compared to those of oral cavity squamous cell carcinoma without architectural dysplasia. Results In total, 92 of 311 (29.6%) of oral cavity squamous cell carcinoma displayed architectural dysplasia in the margins. The presence of architectural dysplasia was associated with higher patient age, female sex, less pack years, lower cT‐stage, and a cohesive tumor growth pattern. In oral cavity squamous cell carcinomas with architectural dysplasia, postoperative (chemo)radiotherapy was less often indicated compared with oral cavity squamous cell carcinoma without architectural dysplasia (19.5% vs. 36.1%, p = 0.009). Five‐year local relapse‐free survival was significantly lower in oral cavity squamous cell carcinoma with architectural dysplasia than in oral cavity squamous cell carcinoma without architectural dysplasia (83.1% vs. 94.9%, p = 0.017). Conclusions Oral cavity squamous cell carcinoma arising in the background of architectural dysplasia displays relatively favorable clinical and histopathological characteristics. Nonetheless, the presence of architectural dysplasia in oral cavity squamous cell carcinoma surgical margins is associated with a higher risk of local relapse, indicating its clinical relevance.

Architectural dysplasia in surgical margins and the risk of local relapse in oral cancer

Objective The biostimulation effects of low‐level laser therapy ( LLLT ) have been demonstrated recently. This study investigated the effects of LLLT on palatal mucoperiosteal wound healing and oxidative stress status in rats. Material and method Forty‐two male Wistar rats weighing 250–300 g were used in this study. A standardized full‐thickness wound was created in the mucoperiosteum of the hard palates of the rats using a 3‐mm‐diameter biopsy punch. Treatment using a GaAlAs laser at a wavelength of 940 nm and a dose of 10 J/cm 2 was initiated after surgery and repeated on the 2nd, 4th, and 6th days post‐surgery. Seven animals from each group were sacrificed on the 7th, 14th, and 21st days after surgery. Total antioxidant status and total oxidative status were measured in serum. Results The histopathological findings revealed reduced numbers of inflammatory cells on the 7th day, increased mitotic activity of fibroblasts on the 14th and 21st day, and the same degree of collagen synthesis and vascularization on the days 7, 14, and 21 in the LLLT group compared with the control group. No significant differences in total oxidative status and total antioxidant status were observed between the groups. Conclusion LLLT using a GaAlAs laser at a wavelength of 940 nm and a dose of 10 J/cm 2 elicited a positive healing effect on palatal mucoperiosteal wounds likely via the induction of fibroblasts. The oxidative stress status was not affected by LLLT .

The effect of low‐level laser therapy on the healing of hard palate mucosa and the oxidative stress status of rats

Recently, there has been a momentous drive to apply advanced artificial intelligence (AI) technologies to diagnostic medicine. The introduction of AI has provided vast new opportunities to improve health care and has introduced a new wave of heightened precision in oncologic pathology. The impact of AI on oncologic pathology has now become apparent, and its use with respect to oral oncology is still in the nascent stage.A foundational overview of AI classification systems used in medicine and a review of common terminology used in machine learning and computational pathology will be presented. This paper provides a focused review on the recent advances in AI and deep learning in oncologic histopathology and oral oncology. In addition, specific emphasis on recent studies that have applied these technologies to oral cancer prognostication will also be discussed.Machine and deep learning methods designed to enhance prognostication of oral cancer have been proposed with much of the work focused on prediction models on patient survival and locoregional recurrences in patients with oral squamous cell carcinomas (OSCC). Few studies have explored machine learning methods on OSCC digital histopathologic images. It is evident that further research at the whole slide image level is needed and future collaborations with computer scientists may progress the field of oral oncology.

The use of artificial intelligence, machine learning and deep learning in oncologic histopathology

Abstract Background The objective of this study was to characterise the incidence and prognostic correlation of perineural invasion (PNI) in oral squamous cell carcinoma and determine whether nerve growth factor and its receptor tyrosine Kinase A expression could be used as biological markers for PNI. Methods A retrospective review of pathology reports of 430 patients with oral squamous cell carcinoma who were treated from 1992 to 2014 in Tayside, Scotland, was carried out. The expression of nerve growth factor and tyrosine kinase A was assessed with immunohistochemistry in 132 tissue sections of oral squamous cell carcinoma. Results Perineural invasion was identified in 17.4% of oral squamous cell carcinomas. High expression of nerve growth factor and tyrosine kinase A was seen in 84% and 92% of oral squamous cell carcinoma, respectively. Tumours with PNI expressed nerve growth factor and tyrosine kinase A with a greater frequency than tumours without PNI. PNI and high expression of nerve growth factor were significantly associated with pain. PNI was significantly associated with stage IV tumours and poor disease‐specific survival. Conclusions A higher level of expression of nerve growth factor and tyrosine kinase A may predict PNI and therefore may be considered as biological markers for PNI in oral squamous cell carcinoma. PNI and nerve growth factor overexpression may contribute to the pain generation in oral cancer patients. PNI and nerve growth factor expression can predict the aggressiveness and prognosis of oral squamous cell carcinoma patients.

Perineural invasion in oral squamous cell carcinoma: Incidence, prognostic impact and molecular insight

ABSTRACT Background The inactivation of tumor suppressor genes (TSGs) caused by abnormal DNA methylation is confirmed to be widely present in oral potential malignant diseases (OPMDs). Carotenoids like lycopene and astaxanthin can regulate DNA methylation and exert anticancer effects. Therapeutic effect of astaxanthin in OPMDs and oral squamous cell carcinoma (OSCC) models is confirmed, but the relationship between the anti‐cancer ability of astaxanthin and its DNA methylation regulation ability remains unclear. Methods Whole‐genome bisulfite sequencing (WGBS) were used to provide biological information associated with DNA methylation. Methylation specific PCR was used to detect the methylation level of specific sites. Related markers were evaluated by qRT‐PCR and western blot. CCK8 assay, cell scratch assay, flow cytometric analysis were performed to investigate the cell viability, migration, cell cycle, and apoptosis after treated with concentrations of astaxanthin. Results WGBS revealed that HOXA3 and SOX1 were the TSGs with significant differences in promoter CpG methylation of oral dysplastic keratinocytes (DOK) cells. After treatment with 8 μM astaxanthin, the promoter CpG methylation levels of the TSGs were significantly reduced, resulting in the increase in gene expression. The overall effect of astaxanthin on DOK cells is inhibiting cell viability, reducing cell migration, leading to cell cycle G 0 /G 1 arrest, and promoting apoptosis. Conclusions This study confirmed significant differences in DNA methylation patterns among oral normal, dysplastic, and cancerous cells. Astaxanthin can reduce the promoter CpG methylation level of TSGs by reducing DNA methyltransferase 1 protein expression level, upregulating mRNA and protein expression, and subsequently modulating the biological behavior of DOK.

Astaxanthin Increases Tumor Suppressor Gene Expression and Affects Cellular Biological Behavior in Oral Dysplastic Keratinocytes by Regulating <scp>DNA</scp> Methylation

ABSTRACT Background Despite recent advancements, women still encounter significant challenges in various fields, including dentistry. However, the increasing interest in female participation in science acknowledges its fundamental role in the advancement of knowledge. This study aims to assess indicators of women's involvement in Brazilian research in the areas of Oral Pathology and Oral Medicine. Methods This cross‐sectional study evaluated 197 professionals affiliated with the Brazilian Society of Stomatology and Oral Pathology in 2023. Data were collected from publicly available Lattes curriculum and organized into three sets of information: researcher profile, scientific production and human resources formation. Both the data from the researcher's entire career and from the last 5 years (2019–2023) were assessed separately. Descriptive analyses of categorical variables were performed, while the Mann–Whitney test was employed to compare the numerical variables regarding researchers' gender. Results Of 197 professionals, 117 (59.4%) were female. Although there was no significant difference in scientific production between genders, men had more publications, received approximately twice as many citations, and exhibited higher H‐index values compared to women. Notably, women surpassed men in undergraduate student supervision, while men predominated in supervising master's and PhD students. Conclusions This study highlighted the relevance of female participation in Oral Pathology and Oral Medicine research in Brazil. However, disparities persist regarding women participation, especially in scientific article citations and postgraduate students' supervision.

ABSTRACT Background An altered blood lipid profile has been considered as a diagnostic and/or prognostic marker for cancer. Since oral cancer is usually preceded by oral potentially malignant disorders (OPMDs) and share common etiopathogenesis, thus researchers have tried to explore the role of blood lipid profile as a marker for OPMDs; however, no consensus has been made regarding the utilization of serum lipid profile as a biomarker for oral submucous fibrosis (OSMF). Thus, the present article aimed to validate serum lipid profile as a biomarker for OSMF. Methodology PubMed, Scopus, Google Scholar and Clinical key databases were searched for relevant articles. Thirty‐six studies that met the eligibility criteria were included for qualitative review, however, out of these, 27 studies with specific data for OSMF and the control group were included in the meta‐analysis. Results A significant reduction in very low‐density lipoprotein ( p = 0.042), low density lipoprotein ( p = 0.006), high density lipoprotein ( p = 0.020), triglyceride ( p = 0.049) and total cholesterol ( p = 0.009) levels in blood were observed in OSMF patients in comparison to healthy controls whereas no significant difference was seen in contrast to oral squamous cell carcinoma patients. Conclusion Although a significant alteration was observed in lipid levels in OSMF patients, considerable heterogeneity in all the studied parameters implies that blood lipid profile could not be used as a reliable biomarker for OSMF and require further investigation.

ABSTRACT Background The terminology surrounding developmental lesions in the oral cavity is widely applied, often leading to confusion in differentiating between developmental malformations and neoplasia. Odontogenic tumor classification includes both true neoplasms and malformations which make it very complex and dynamic. Method and Conclusion In this brief report, we will first discuss the concepts of malformations and neoplasia and then focusing on their relevance to odontogenic tumors, which impacts their classification and treatment, particularly mixed odontogenic lesions.
