Clinical Oral Implants Research

Multiple generations of medical robots have revolutionized surgery. Their application to dental implants is still in its infancy. Co‐operating robots (cobots) have great potential to improve the accuracy of implant placement, overcoming the limitations of static and dynamic navigation. This study reports the accuracy of robot‐assisted dental implant placement in a preclinical model and further applies the robotic system in a clinical case series.
In model analyses, the use of a lock‐on structure at robot arm‐handpiece was tested in resin arch models. In a clinical case series, patients with single missing teeth or edentulous arch were included. Robot‐assisted implant placement was performed. Surgery time was recorded. Implant platform deviation, apex deviation, and angular deviation were measured. Factors influencing implant accuracy were analyzed.
The in vitro results showed that with a lock‐on structure, the mean (SD) of platform deviation, apex deviation, and angular deviation were 0.37 (0.14) mm, 0.44 (0.17) mm, and 0.75 (0.29)°, respectively. Twenty‐one patients (28 implants) were included in the clinical case series, 2 with arches and 19 with single missing teeth. The median surgery time for single missing teeth was 23 (IQ range 20–25) min. The surgery time for the two edentulous arches was 47 and 70 min. The mean (SD) of platform deviation, apex deviation, and angular deviation was 0.54 (0.17) mm, 0.54 (0.11) mm, and 0.79 (0.22)° for single missing teeth and for 0.53 (0.17) mm, 0.58 (0.17) mm, and 0.77 (0.26)° for an edentulous arch. Implants placed in the mandible had significantly larger apex deviation than those in the maxilla.
Cobot‐assisted dental implant placement showed excellent positional accuracy and safety in both the in vitro study and the clinical case series. More technological development and clinical research are needed to support the introduction of robotic surgery in oral implantology. Trial registered in ChiCTR2100050885.

Prevalence and risk/protective indicators of peri‐implant diseases: A university‐representative cross‐sectional study

OBJECTIVE
Aimed to evaluate the long-term clinical and radiographic outcomes of short implants supporting single crowns in the posterior regions.


MATERIAL AND METHODS
A retrospective study design was adopted. The clinical and radiographic data of 231 short implants (intra-bony length ≤8 mm) supporting single crowns in 168 patients, were collected after 5-10 (mean 7.22) years' follow-up. Implant and prosthesis failures, peri-implant marginal bone loss, biological and technical complications were evaluated. The influence factors on implant failure were studied.


RESULTS
In total 4 implants and 11 prostheses failed. The 10-year (5-year) cumulative survival rate was 98.3% (98.7%) for implant-based analysis and 97.6% (98.2%) for patient-based analysis. The short implants placed in type IV bone yielded more failures than in type I-III and presented a survival rate of 94.0%. The 10-year survival rate of the prostheses was 95.2%. The mean marginal bone loss between implant installation and the 10 years' follow-up visit was 0.63 ± 0.68 mm. The marginal bone loss between the first and 5th year was minimal (0.05 ± 0.10mm and not statistically significant). 18 (7.8%) implants were exposed to biological complications, whereas 29 (12.6%) implants were involved in technical complications.


CONCLUSIONS
High survival rates for both the implants and the prostheses could be achieved after 5-10 years for short implants supporting single crowns, without severe marginal bone loss and complications. One may conclude that a single crown supported by a short implant is a predictable treatment modality. However, short implants in type IV bone sites should be applied with caution.

Long-term outcomes of short dental implants supporting single crowns in posterior region: a clinical retrospective study of 5-10 years

To evaluate the long-term clinical and radiographic outcomes of dental implant placed using osteotome sinus floor elevation (OSFE) without bone grafts, and to analyze the potential influence factors of implant survival and endo-sinus bone remodeling.A retrospective study design was adopted. The clinical and radiographic data of 96 implants in 80 patients were collected after 4-9 (mean 5.40) years follow-up. Implant failures, peri-implant marginal bone loss (MBL), and endo-sinus bone remodeling on the radiographs were evaluated. A life-table analysis was used to assess the implant survival. Statistical models were established to investigate the potential influence factors of implant survival and endo-sinus bone gain (ESBG).In total, nine implants in seven patients failed, giving the 9-year cumulative survival rates of 90.6% and 91.3% for implant-based analysis and patient-based analysis, respectively. The mean MBL between implant installation and the 4- and 9-year follow-up visit was 0.46 ± 0.88 and 0.50 ± 1.69 mm, respectively. The average ESBG on radiographs was 2.95 ± 1.25 and 2.16 ± 1.13 mm at the 4- and 9-year follow-up. The final ESBG was found to be positively correlated to implant protrusion length after surgery without any other factors related. The implant survival rate was significantly lower in severe atrophic site (residual bone height <5 mm).Osteotome sinus floor elevation without bone grafts is a predictable treatment modality in the long run. But it should be used with caution when the initial bone height of the edentulous site is lower than 5 mm. The final endo-sinus bone height was found to be positively correlated to implant protrusion length measured on radiographs immediately after implant installation.

Long‐term outcomes of osteotome sinus floor elevation without bone grafts: a clinical retrospective study of 4–9 years

To retrospectively compare multilevel volumetric changes in both hard and soft tissues between antral pseudocyst (AP) removal and retainment before maxillary sinus floor augmentation (MSFA) and immediate implant placement.Twenty-six patients with 38 implants placed from 2016 to 2021 were included and divided according to a cohort design as follows: 13 removing the cyst (RC group) and 13 "leaving alone" the cyst (LC group). 3D radiographic parameters (primary outcome), 2D parameters and clinical records (secondary outcome) involving both hard and soft tissues were evaluated for four periods (T1: immediate postoperative, T2: 6-month, T3: 12-month, and T4: 2- to 5- year follow-up). Possible confounding factors, including sinus anatomical features and implant distribution, were also analyzed to eliminate their disturbance.The 3D volumetric change rate of bone grafts in the RC group (-9.32% ± 10.01%) from T2 to T3 was significantly lower than that in the LC group (-19.8% ± 10.59%) (p < .05). The change rate of apical bone height (ABH), endo-sinus bone gain (ESBG) and other 2D parameters were not significantly different between the two groups. 5.3% implants in RC group and 9.1% implants in LC group failed during follow-ups. 0% postoperative complications were observed in RC group. The Schneiderian membrane of RC group was significantly thinner than that of LC group at two measuring points in sinus.The present study demonstrated that compared to AP retainment, AP removal before MSFA and immediate implant placement can obtain higher bone graft volumetric stability and favorable prognosis.

Influence of Implant Component Materials on Peri‐Implant Soft Tissue Healing: A Comparative Histological and Immunohistochemical Study in Humans

ABSTRACT Objectives Primary stability (PS) is a key factor for promoting osseointegration and long‐term success of dental implants particularly for immediate loading protocols. Beyond the current assessments of PS, an accurate pre‐operative evaluation of PS would contribute to the improvement of surgical planning and treatment outcome. This study used biomechanical testing and homogenized finite element (hFE) analysis to objectively measure PS in the laboratory, and digitally estimate PS from prior μCT reconstructions. Material and Methods Thirty‐five bone samples extracted from the jaws of two donors were examined. Twenty‐two were finally evaluated for PS. After scanning of the samples with μCT, implants were inserted by two experienced surgeons, and various metrics such as μCT‐based bone volume fraction (BV/TV), insertion torque (IT), and resonance frequency analysis (RFA) were assessed to determine PS. Mechanical tests were conducted to measure ultimate force (UFexp) as an objective indicator of PS while the hFE simulations were performed to estimate this same ultimate force (UFsim). Results Higher correlation was found between UFsim and UFexp ( R 2 = 0.85) than between BV/TV and UFexp ( R 2 = 0.61), IT and UFexp ( R 2 = 0.50), and RFA and UFexp ( R 2 = 0.38). All variables demonstrated a statistically significant linear correlation with UFexp ( p &lt; 0.01). Conclusion UFsim turns out to be a more reliable and objective indicator of PS than IT and RFA. The hFE analysis requires prior μCT reconstructions and is currently limited by numerical convergence problems. Despite these limitations, pre‐operative hFE analysis emerges as a promising tool with a higher accuracy for estimation of PS than state of care techniques.

Primary Stability of Dental Implants in Human Jawbones: Experiments &amp; <scp>FE</scp> Analyses

ABSTRACT Objective This university‐based retrospective study aimed to assess the performance of the implant disease risk assessment (IDRA) in predicting peri‐implantitis. Material and Methods Patients with implants loaded for at least 1 year were included. Peri‐implantitis development was the outcome, while the IDRA score and its eight vectors were the predictors. The IDRA score was calculated using an online tool. Data were analyzed using Cox proportional hazards models and ROC curve (AUC). Results Among 480 implants in 235 patients, 7.9% of implants and 9.4% of patients developed peri‐implantitis. Implants at high risk for the “number of sites with PD ≥ 5 mm” vector had an increased risk (HR = 9.8, p = 0.004) of peri‐implantitis, compared to those at low risk for this parameter. Implants at moderate (HR = 4.8, p = 0.04) and high (HR = 10.0, p = 0.01) risk for the “distance from the restorative margin (RM) to bone crest (BC)” vector exhibited a higher risk of peri‐implantitis than implants at low risk for this parameter. The IDRA tool demonstrated an AUC of 0.66 (sensitivity = 0.80; specificity = 0.24) when estimated at implant level and an AUC of 0.61 (sensitivity = 0.91; specificity = 0.32) when calculated at patient level. The mixed‐effects Cox model did not reveal a significant association between the overall IDRA score and the development of peri‐implantitis (HR = 7.2, p = 0.18). Conclusion IDRA demonstrates good sensitivity but low specificity and suboptimal discriminatory capacity in predicting peri‐implantitis. The “number of sites with PD ≥ 5 mm” and “distance from RM to BC” emerged as the most effective predictors for peri‐implantitis.

Performance of the Implant Disease Risk Assessment in Predicting Peri‐Implantitis: A Retrospective Study

ABSTRACT Purpose To evaluate the performance and clinical outcome of vertical and horizontal bone augmentation (VHBA) in posterior maxillary regions combining lateral window sinus floor elevation (LWSFE) with a horizontal bone shell technique applying the maxillary facial sinus wall as a bone plate. Materials and Methods In 18 patients, LWSFE was combined with a horizontal bone shield augmentation procedure utilizing the maxillary facial sinus bone wall as a lateral bone plate. Both the sinus cavity and the lateral bone box created were grafted with a mixture of autogenous bone/venous blood and bovine bone mineral. The primary aim was to assess the performance of combined techniques enabling subsequent implant placement. Using radiographic measurements (preoperative, after VHBA, at implant placement, and at follow‐up), bone gain/reduction of augmented horizontal ridge width (HRW) and vertical bone height (VBH) were evaluated. Additionally, clinical outcome assessing implant survival/success rate, marginal bone loss (MBL), and implant health (mucositis/peri‐implantitis) was evaluated. Results For the combined VHBA techniques, HRW and VBH increased significantly ( p &lt; 0.001) from preoperative 3.5 ± 1.4 mm/3.6 ± 2.1 mm to 9.7 ± 1.9 mm/18.0 ± 1.6 mm post‐augmentation. However, HRW and VBH dimensions decreased up to 8.9 ± 1.8 mm/17.1 ± 1.4 mm at implant placement and 8.6 ± 1.7 mm/16.7 ± 1.3 mm at follow‐up evaluation (3.8 ± 1.8 years; p &lt; 0.001, respectively). Augmented bone reduction was significantly higher (−7.7%) between the augmentation procedure and implant placement than in the post‐implant‐placement period (−2.5%). All implants survived (100%) representing peri‐implant MBL of −0.9 ± 0.7 mm, pocket depth of 3.4 + 1.8 mm, and prevalences of 5%/0% for peri‐implant mucositis/peri‐implantitis. Conclusion The combination of horizontal bone augmentation using local bone shield transfer from the maxillary facial sinus wall with LWSFE enables sufficient reconstruction of maxillary posterior ridge.

The Use of the Facial Sinus Wall as Bone Shell Onlay Graft for Maxillary Posterior Ridge Reconstruction: A Retrospective Case Series

ABSTRACT Objectives Variations in transmucosal abutment contour design may affect the outcomes of implant therapy. This randomized controlled trial was primarily aimed at testing the effect that CAD/CAM zirconia abutments with either a concave or linear divergent transmucosal morphology have on peri‐implant mucosal dynamics and indicators of peri‐implant health at 1 year after final implant‐supported prosthesis insertion in the anterior maxilla. Materials and Methods Following computer‐guided implant placement and osseointegration, eligible subjects were randomized into either the experimental (concave morphology) or the control (linear divergent morphology) group. A comprehensive set of outcomes of interest related to peri‐implant soft tissue dynamics, phenotypical features, and indicators of peri‐implant health were assessed at different time points over a 1‐year period after insertion of the final restoration. Results Out of 60 initially recruited subjects, a total of 54 completed the study ( n = 29 in the experimental group concave/ n = 25 in the control group). Overall implant survival and restoration rates between master impression and 12 months were 100% and 98.2%, respectively. Although a trend for coronal migration of the buccal mucosa zenith, gain in mucosal thickness, and increased probing depth and bleeding on probing was observed in both groups, these changes were clinically negligible, and no substantial differences were observed between study groups regardless of variations in transmucosal abutment morphology. Conclusion The use of either linear divergent or concave custom CAD/CAM zirconia abutments in a screw‐retained, delayed loading approach yielded no significant differences.
