Invasive fungal sinusitis is still a life-threatening infection in immunocompromised patients. Its onset is rapid and leads to severe complications. We describe a neutropenic patient with acute myeloid leukemia who developed invasive sinus aspergillosis despite receiving prophylactic itraconazole and empirical micafungin. The patient demonstrated apparent clinical findings such as nasal discharge, nasal congestion, and skin and mucosal necrosis. Computer tomography scans strongly demonstrated invasive maxillary sinusitis. Serological examinations for (1-3)-β-d-glucan and Aspergillus galactomannan antigen were positive, suggesting Aspergillus infection. The diagnosis of invasive sinus aspergillosis was confirmed by in situ hybridization using Aspergillus-specific probe in formalin-fixed, paraffin-embedded tissue samples. The causative organism was identified as Aspergillus fumigatus by fungal culture. Despite surgical treatment with drainage and intensive antifungal administration of voriconazole and liposomal amphotericin B, the infection rapidly disseminated to the lungs, resulting in a fatal progression of the condition.

Denture fragment inhalation is uncommon. Denture fragment impaction at the glottis is rare. A glottis impaction without incurring airway symptoms is even rare. The populations of elderly wearing denture are significant and abundant. It is important as not to miss the possibility of denture inhalation as the illustrated in this case report. In the event of broken or swallowed denture, all patients warrant a proper upper airway assessment for foreign body until proven otherwise.

Unusual glottic denture impaction: A case report

喉部义齿嵌塞：一例报告

This report is the case report of a 48-year-old woman who referred to our hospital complaining of a mass at the left maxillary tuberosity and slight trismus. CT and MRI imagings showed a tumor like image with bone absorption of the left maxillary tuberosity. Under general anesthesia, the lesion was excised. Histopathological examination showed low grade mucoepidermoid carcinoma. There is no sign of recurrence or metastasis of the tumor in 5 year after the surgery.

Low grade mucoepidermoid carcinoma of the maxillary tuberosity: A case report

This study evaluated the reliability of using local pedicled mandibular osteomuscular and facial-cervico-pectoral flaps to fill soft-tissue defects of both the mandible and cheek following debridement. Six patients with refractory osteoradionecrosis of the mandible underwent radical resection and reconstruction of type H defects with combined pedicled mandibular osteomuscular and facial-cervico-pectoral flaps. The patients consisted of four men and two women with an average age of 50.5 years (range 48–58). All of the flaps survived. Function and aesthetics were satisfactory for both the patient and surgeon, and no donor-site problems occurred. The patients were followed for an average of 20.6 months (range 16–24) and recurrence was never encountered. The pedicled mandibular osteomuscular and facial-cervico-pectoral flaps are technically simple, fast to harvest, and have exceptionally long arcs of rotation. These flaps enabled reconstruction of both composite segmental defects of the mandible and soft-tissue defects of the cheek following debridement.

Juvenile psammomatoid ossifying fibroma (JPOF) is a rare, slowly progressive tumor of the extragnathic craniofacial bones, with a tendency towards locally aggressive behavior and recurrence. The pathognomonic histopathologic feature is the presence of spherical ossicles, which are similar to psammoma bodies. Very few cases in association with secondary aneurysmal bone cyst (ABC) formation have been reported in the literature. Treatment consists of complete surgical removal and incomplete excision has been associated with a high local recurrence rate. The prognosis is good because malignant change and metastasis have not been reported. Authors report a case of JPOF of the mandible with secondary ABC in a 17-year-old male patient.

A case of fracture of the genial tubercles in a 68-year-old woman is reported. This is an uncommon diagnosis which is usually associated with a severely atrophied edentulous mandible with hypertrophied genial tubercles. Clinical complaints include swollen floor of mouth and dysphagia. A complete denture is usually worn. The diagnosis will normally be revealed with a true occlusal radiograph. However, a computed tomogram may be necessary in cases which are less typical. This case was managed conservatively with a good outcome.

Isolated fracture of the genial tubercles: Report of a case

A 45-year-old Japanese woman presented complaining of swelling of the right mandible. Initial histopathological diagnosis was cemental dysplasia. Because of rapid enlargement of the lesion, surgical resection was performed. The post-surgical second histopathological diagnosis was osteosarcoma. In spite of additional treatment with radiotherapy and chemotherapy, the patient died 32 months after the initial visit. Autopsy revealed osteosarcoma with metastases infiltrating of a number of organs, including the lungs, heart, and vertebrae.

Spindle cell carcinoma is a rare malignant tumour that shows biphasic proliferation of squamous cell carcinoma components and sarcomatoid components of spindle-shaped cells in the same tissue. In addition, cases that a malignant tumour of the head and neck region metastasizes to the thyroid gland are extremely rare. We report an extremely rare case that spindle cell carcinoma of the tongue metastasized to the thyroid gland. A 59-year-old female presented with a polypoid mass in the right margin of the tongue. Based on histopathological diagnosis of spindle cell carcinoma of clinical staging II, a partial glossectomy was performed. Thyroid mass appeared at 5 months after the surgery. She was diagnosis with thyroid gland cancer and a total thyroidectomy was performed. A histopathological diagnosis revealed thyroid gland metastasis. However, a thyroid tumour recurred in the neck at 4 months after the thyroid surgery. The tumour was untreatable, and the patient expired 13 months after the primary resection.
