The British Journal of General Practice is an international journal that publishes research, editorials, debates, analyses, and clinical guidance for family practitioners and primary care researchers worldwide. It was established in 1953 as the 'College of General Practitioners Research Newsletter' and first appeared as the 'Journal of the College of General Practitioners' in 1960. Following changes in the college's status, it became the 'Journal of the Royal College of General Practitioners' in 1967. After three editors, the title was changed to the 'British Journal of General Practice' in 1990. The journal is commonly referred to as 'BJGP' and is an independent publication of the Royal College of General Practitioners.

Continuity of care has always been at the heart of general practice.Patients who receive continuity have better healthcare outcomes, higher satisfaction rates, and the health care they receive is more costeffective. 1,2The Royal College of General Practitioners (RCGP) has always advocated for continuity and has previously produced a paper on the benefits of continuity of care, and subsequently the Continuity of Care Toolkit. 3n the upcoming RCGP paper Continuity of Care in Modern Day Practice (available soon at http://www.rcgp.org.uk/policy/rcgppolicy-areas/continuity-of-care.aspx) the College asks to whom continuity is most important in modern-day general practice, and how can it be realised in the face of changing demographics, work patterns, and the introduction of new models of care.

Background Doctors can be victim-survivors of domestic abuse (DA), but how this impacts their work and wellbeing, and whether they face barriers to seeking help is not well understood. Aim To understand single doctor mothers’ lived experience of DA, barriers to seeking help, and impact on their work. Design and setting Individual qualitative interviews with female doctors in the UK who had left an abusive relationship. Interviews were conducted between August 2019 and March 2020. Method Participants were invited via a closed online forum for female doctors who are single parents. In total, 114 females expressed interest. In-depth semi-structured telephone interviews were audiorecorded and transcribed. Transcripts were uploaded to NVivo and analysed using inductive thematic analysis. Results A total of 21 participants were interviewed. The internalised stigma of DA affected participants’ sense of identity and belonging as a doctor, causing social and professional isolation. Many participants felt that the acute stress of DA had an impact on their work, yet often felt unable to take time off. Barriers to seeking help included lack of confidentiality, especially where the abusive partner was also a doctor (sometimes accusing the victim-survivor of mental illness or threatening to report them to the General Medical Council). Participants found peer support helpful, as well as consulting health professionals who were empathic towards them. After they had left the abusive relationship victim-survivors felt better equipped to support patients going through DA. Conclusion Domestic abuse impacts on the work and wellbeing of female doctors, who face unique barriers to help seeking and reporting DA. An online peer support group can help to break the sense of isolation, but specialised confidential support services are also required to help doctors experiencing DA.

Domestic abuse among female doctors: thematic analysis of qualitative interviews in the UK

Background General practice plays a critical role in the prevention, diagnosis, management, and survivorship care of patients with cancer. Mapping research outputs over time provides valuable insights into the evolving role of general practice in cancer care. Aim To describe and compare the distribution of cancer in general practice research publications by country, cancer type, area of the cancer continuum, author sex, and journal impact factor. Design and setting A bibliometric analysis using a systematic approach to identify publications. Method MEDLINE and Embase databases were searched for studies published between 2013 and 2019, which reported on cancer in general practice. Included studies were mapped to the cancer continuum framework. Descriptive statistics were used to present data from the included studies. Results A total of 2798 publications were included from 714 journals, spanning 79 countries. The publication rate remained stable over this period. Overall, the US produced the most publications ( n = 886, 31.7%), although, per general population capita, Denmark produced nearly 10 times more publications than the US (20.0 publications per million compared with 2.7 publications per million). Research across the cancer continuum varied by country, but, overall, most studies focused on cancer screening, diagnosis, and survivorship. More than half of included studies used observational study designs ( n = 1523, 54.4%). Females made up 66.5% ( n = 1304) of first authors, but only 47.0% ( n = 927) of last authors. Conclusion Cancer in general practice is a stable field where research is predominantly observational. There is geographical variation in the focus of cancer in general practice research, which may reflect different priorities and levels of investment between countries. Overall, these results support future consideration of how to improve under-represented research areas and the design, conduct, and translation of interventional research.

General practice-based cancer research publications: a bibliometric analysis 2013–2019

Anal fissures are tears of the anal mucosa. They can cause extreme pain (often up to 1–2 hours post-defaecation) and in many cases bleeding. Acute anal fissures are classified as lasting 6 weeks. Primary fissures have no clear underlying cause. This is in contrast to secondary fissures, which are thought to be caused by another principal condition.

If not treated effectively, anal fissures can lead to recurrence, infection, or abscesses. Continuity of care in general practice and secondary care: retrospective cohort study

Background: effectiveness of single disease management programs (DMP) in general practice may be limited for patients with low socioeconomic status (SES), as DMPs insufficiently take into account the specific problems and needs of this population. A Person-Centred Integrated Care (PC-IC) approach focusing on patient’s needs could address these problems. Aim: to explore experiences of patients with (multiple) chronic diseases with regard to the acceptability of a general practice-based PC-IC approach, with a focus on patients with low SES, and to establish which modifications are needed to tailor the approach to this group. Design and setting: in 2021, a 6 month feasibility study in seven general practices in the Netherlands was carried out. The healthcare professionals provided care based on a PC-IC approach to patients with diabetes, chronic respiratory diseases and/or cardiovascular disorders. Method: a qualitative study using focus group discussions, in-depth interviews, and semi-structured telephone interviews in a combined total of 46 patients with chronic diseases and multimorbidity, including 31 patients with low SES. Results: an overall positive experience of participants with the PC-IC approach was observed. Discussing their health made patients feel being taken more seriously, and provided the opportunity to discuss their life and health concerns. Recommended adaptations of the PC-IC approach for patients with low SES include creating comprehensible materials and offering communication training for healthcare professionals. Conclusion: the PC-IC approach seems helpful for chronic disease patients, provided that it is tailored to their skills and abilities. Several modifications for patients with low SES were suggested.

Background:Direct oral anticoagulants (DOACs) are commonly co-prescribed with statins. Although biologically plausible, whether there is a drug interaction between DOACs and atorvastatin/simvastatin is unclear. Aim:To investigate the association between co-prescribed DOACs and atorvastatin/simvastatin and bleeding, cardiovascular disease and mortality. Design and Setting:Clinical Practice Research Datalink Aurum(1/1/2011-31/12/2019). Method: We used a cohort design to estimate hazard ratios for clinically relevant pharmacological interaction safety outcomes (intracranial bleeding, gastrointestinal bleeding, other bleeding) comparing DOACs+atorvastatin/simvastatin with DOACs+other statins (fluvastatin, pravastatin and rosuvastatin which are not anticipated to interact with DOACs). Effectiveness outcomes (ischaemic stroke, myocardial infarction, venous thromboembolism, cardiovascular mortality, and all-cause mortality) were also included. A case-crossover design comparing odds of exposure to different drug initiation patterns in hazard window versus referent window within an individual was also conducted. Results:Of 397,459 DOAC users, we selected 70,318 people co-prescribed atorvastatin, and 38,724 co-prescribed simvastatin. The cohort analysis showed no difference in risk of all outcomes comparing DOACs+atorvastatin/simvastatin versus DOACs+other statins. In case-crossover analysis, ORs for other bleeding (OR:4.04; 99%CI:3.07–5.31) amongst those initiating DOACs while taking atorvastatin, and the ORs for gastrointestinal bleeding (OR:5.16; 99%CI:3.66–7.28) and other bleeding (OR:5.12; 99%CI:3.61–7.26) amongst those initiating DOACs while taking simvastatin were greater than those initiating DOAC monotherapy. Similar patterns were also observed for cardiovascular mortality and all-cause mortality. Conclusion:This study shows no evidence of interaction between DOACs and atorvastatin/simvastatin. However, people starting a DOAC whilst taking atorvastatin/simvastatin, were at high risk of bleeding and mortality, likely due to temporal clinical vulnerability.
