The objective of this study is to assess the validity Diagnostic Criteria for Temporomandibular joint Disorder (DC/TMD) in the diagnosis of disc displacement (DD) disorders of temporomandibular joint (TMJ).It was a prospective study in which total of 55 patients accounting to 110 TMJ; with clinical diagnosis of DD disorder as per DC/TMD criteria and all above age of 18 years were included. All the patients with clinical diagnosis of DD disorder were advised MRI. All the MRI scans were read by single radiologist. Clinician and Radiologist were blinded to each others findings. MRI diagnosis and clinical diagnosis were compared and results were formulated.The use of the Kappa statistic test indicated a good diagnostic agreement (k = 0.68) between the clinician and the radiologist. The DC/TMD criterion was found to have good validity and reliability in the diagnosis of DD disorders with sensitivity of 91.57% and specificity of 77.78%. The positive predictive value and negative predicted value calculated were 92.68% and 78%, respectively. The positive and negative likelihood ratio found was 4.12 and 0.11, respectively.DC/TMD is a simple, reliable, valid, cost-effective tool in the diagnosis of DD disorders.

Validity of diagnostic criteria for temporomandibular joint disorder in the diagnosis of disc displacement disorders of temporomandibular joint

The use of ceramic laminate veneer has considerably and successfully grown to improve anterior tooth esthetics in recent years. The removal of ceramic laminate veneers with laser is reported only in a scanty number of publications and for this reason the importance and the aim of this ex vivo study consist to verify the ability of Er: YAG laser for laminate veneers debonding with the preserving of the tooth structures (scanning electron microscopy [SEM] observations).The purpose of this study consists to verify if erbium-doped, yttrium-aluminum-garnet (Er:YAG) laser, at low fluences, is able to debond porcelain veneers, successfully used to improve anterior tooth esthetics, without damaging the tooth structures.A total of 12 freshly extracted teeth were used, and samples were decontaminated, stored, and bonded to obtain veneers adhesion. One week after, Er:YAG laser with a non-contact sapphire tip with air-water spray was used for veneer debonding at 100 mJ of energy and 30 Hz of frequency (Fluence 19.94 J/cm2).Results demonstrated that veneer debonding is possible with an Er:YAG laser and the total number of pulses seems not related to its efficiency. SEM observation confirms that residual tooth structure is not altered when using these low fluences.Low fluences with Er:YAG laser are able to debond veneers while preserving the tooth structures and SEM observation confirmed that residual tooth structure is not altered with low fluences.

Erbium-doped, yttrium-aluminum-garnet laser debonding of porcelain laminate veneers: An ex vivo study

It has been speculated that a change in cervical spine posture occurs due to forward repositioning of the mandible. Therefore, this study aimed to evaluate and compare the cervical spine posture in Class II division one patient treated with three different treatment modalities.A retrospective cohort study was conducted using pre and post-treatment lateral cephalograms of Class II Division one patients who have undergone orthodontic therapy using twin block appliance, Forsus, and bilateral sagittal split osteotomy (BSSO). This study included a total of 57 subjects comprising 19 subjects in each group. Seven cervical, three sagittal, and one vertical parameters were compared within and between each group. The data were tested using the Dependent t-test, One-way analysis of variance, and Tukey's post hoc test.A significant difference existed between the pre- and post-treatment angular measurements within the three groups showing a change in the cervical spine posture with forward positioning of the mandible. A comparison of mean changes in angular measurements between the three groups showed a significant difference in SNA, SNB, ANB, and odontoid process tangent-cervical vertebral tangent (OPT-CVT), indicating a change in the cervical posture.OPT-CVT angle predicts a change in cervical spine posture after treatment with a significant difference in the Twin Block group (P = 0.029) compared to Forsus and BSSO groups. Thus, the twin block group results in a more upright craniocervical posture than the other two treatment groups.

This article outlines the orthodontic treatment of a 21-year-old female patient with an open bite and temporomandibular joint disorders (TMDs) that developed after a severe car accident. The treatment plan utilized temporary anchorage devices (TADs) for upper molar intrusion to correct the open bite without resorting to orthognathic surgery. Over a period of 3 years, the treatment achieved a stable occlusion, normalized molar relationships, and improved esthetics. Despite the persistent of TMD symptoms, the patient experienced enhanced jaw movement and a reduction in abnormal chewing cycles. The combination of orthodontic treatment with TADs and splint therapy contributed to successfully restoring both esthetic and functional aspects. This case underscores the efficacy of TADs in addressing trauma-induced malocclusions, highlighting the potential for minimal intervention to achieve comprehensive treatment outcomes.

Orthodontic Treatment of Anterior Open Bite Caused by a Car Accident

Background: The choice of sealing material is a crucial factor that influences the outcome of endodontic treatment. Aim: The purpose of this study was to compare the sealing ability of mineral trioxide aggregate (MTA) Angelus, Endocem MTA, and NeoPutty MTA when used as furcal perforation repair materials. Materials and Methods: A total of 45 mandibular molars were used. Root canal treatment was carried out following which standardized furcal perforations were made. The specimens were randomly divided into three groups of 15 teeth each. Group A-Furcal perforation repair by means of MTA Angelus, Group B-Furcal perforation repair using Endocem MTA, and Group C-Furcal perforation repair by NeoPutty MTA. Following the repair procedure, the pulp chambers and access openings were filled with temporary restorative material and immersed in 1% basic fuchsin dye for 24 h. The teeth were sectioned longitudinally and the linear dye penetration was measured using a stereo-microscope. Results: The one-way analysis of variance (ANOVA) revealed a statistically significant difference among the groups ( F = 16.15, P &lt; 0.001). On calculating the depth of leakage to the total length of the perforation, it was observed that the mean leakage was 32.83% in Group I, 52.92% in Group II and the lowest, 16.35% in Group III. The ANOVA test reveals a statistically significant difference among the groups ( F = 15.92, P &lt; 0.001). Comparing the depth of dye penetration of dye, 33.3% of the Group I samples showed ≥50%, 49%–25%, and &lt;25% dye penetration. Although 56.7% of Group II samples showed ≥50% dye penetration, whereas 63.3% of Group III showed &lt;25% dye penetration. Conclusion: The present study indicated that NeoPutty MTA had the least dye penetration followed by MTA Angelus and Endocem MTA.

Stereomicroscopic Evaluation of Sealing Ability of Three Different Furcal Perforation Repair Materials: An In vitro Study

Introduction: There is a scarcity of studies in which both alveolar bone remodeling and root resorption are simultaneously assessed and compared against possible influencing factors such as rate of retraction and change in inclination. The aim of our study was to assess the changes in the alveolar bone thickness and root length of the maxillary anterior teeth after retraction and correlate it to the potential influencing factors, namely rate of retraction and change in inclination. Subjects and Methods: Ten patients requiring upper premolar extraction as part of their treatment were included in the study. Alveolar bone and root length changes were measured using cone beam computed tomography and the rate of retraction and change in inclination were assessed using lateral cephalograms. Two readings: Preretraction and postretraction were taken. Statistical analysis was done using Paired rank t -test and Spearman’s rank correlation tests. Significance level was set at P &lt; 0.05. Results and Conclusions: The labial alveolar bone thickness showed an increase, whereas the palatal and total alveolar bone thickness showed a decrease. Root resorption was observed in all teeth. Alveolar bone changes showed a positive correlation to the rate of retraction and a negative correlation to the change in inclination. The root length change showed a positive correlation to the change in inclination.

Evaluation of the Changes in the Alveolar Bone and Root Length after Maxillary Anterior Tooth Retraction and the Factors Influencing Them

上颌前牙内收后牙槽骨及牙根长度变化的评估及其影响因素

Regenerative endodontic therapy (RET) of young permanent teeth with necrotic pulps and apical periodontitis in young people, deciduous tooth pulp may be utilized as a natural, biologic scaffold. Recent developments in stem cell biology and material sciences are beneficial for new treatment methods. Previously traumatized and necrotic young permanent tooth was treated with RET protocol. In the first visit, irrigation, and placement of triple antibiotic paste dressing were done. After that in the second visit, intracanal medication was removed and whole pulp tissue harvested from the deciduous tooth was transplanted in the young permanent tooth. Following placement of Biodentine ® coronal barrier, access cavity was restored with acid-etch resin composite. This case report would highlight favorable outcome of RET using deciduous pulp autotransplantation in young permanent tooth using cone-beam computed tomography for 6 months’ follow-up.

Critical alveolar ridge defects can result from several causes. Ridge expansion is a technique that aims to increase the width of the bone crest. A male patient presented an edentulous segment which leads to be careful concerning the adjacent teeth. Through this case report, the used technique was explained with all steps and was combined with guided bone regeneration (GBR). The particularity of this case was the membrane exposure during the healing period before placing the crowns, resulting in bone resorption. This resorption was stable after restauration and follow-up. Ridge split is indicated in cases where the width of the ridge is at least 3.5 mm. The procedure demonstrated a safe and predictable alternative for implant placement in a knife-edge ridge, with a high survival rate of ridge expansion. The patient presented good healing at the follow-up with a little resorption that remained stable in time after crown placing.
