Objective To evaluate the efficacy and safety of risperidone and haloperidol in treating patients with Diagnostic and Statistical Manual of Mental Disorders-Ⅳ bipolar Ⅰ disorder acute manic episodes.Methods A randomized, double-blind, controlled multicenter clinical trial was conducted. All 173 patients received 4 weeks treatment with either risperidone (n=85, averaged dosage of 4.6 mg/d) or haloperidol (n=88, 8.0 mg/d). The main efficacy measure was the change in total score of the Young Mania Rating Scale.Results The clinical response rates were 79% in risperidone and 76% in haloperidol. There were significantly higher rates of extrapyramidal symptoms in haloperidol group (58%) than that in risperidone group (44%).Conclusion The results show that risperidone has similar therapeutic effect to haloperidol in treatment of acute manic episodes, but risperidone has better tolerability.

Objective The purpose of this study was to examine the rates of major depression and suicidality and their associations with change in daily functioning in a sample of human immunodeficiency viruses(HIV) infected persons and HIV-negative controls.Methods The participants were 28 HIV-positive persons and 23 demographically matched HIV-negative controls.Major depressive disorder(MDD) and history of suicidality were determined with the Chinese version of Composite International Diagnostic Interview(CIDI),and current severity of depression and suicidal ideation were assessed with the Beck Depression Inventory(BDI),and daily functioning with the Activity of Daily Living(ADL) questionnaire.Results Almost 79%(n=22) of HIV-positive persons but just 4%(n=1) of HIV-negative controls met the criteria for MDD during their lifetime.Only one of the 22 HIV-positive individuals with MDD had a previous diagnosis of MDD, and the remaining 21 participants developed MDD within 6 months after testing HIV positive.In those 21 ones,5(24%) reported severe depressive symptom(BDI total 29),7(33%) reported moderate severe symptom(BDI =17—29),and 4(19%) had active suicidal thoughts even after the median of 16 months from learning of their HIV infection.Both depression and HIV status independently predicted worse daily functioning.In HIV-positive subjects with depression,only two had received a psychiatric evaluation and were in treatment for their depression.Conclusion Our study presents evidence of high rates of major depression and suicidality in HIV infected persons.Most developed depression within six months after learning of their HIV infection and about two-thirds of them still reported moderate to severe levels of depression symptoms after an average of two years from learning of their HIV infection.The severity of depression is significantly associated with impairment in daily functioning.

Objective To investigate the effect of chronic forced swimming stress on emotion and phospho-extracellular signal-regulated kinases of brain in rats.Methods Thirty male Sprague-Dawley rats were randomly divided into three groups with ten in each.The stressed animals were given swimming stress for 5 minutes once daily for 14 days,the apparatus controls were placed in novel environments for 5 minutes once daily for 14 consecutive days,and the controls were free of stress.The behavior was tested after stress, then the rats were decapitated.The tissues of hippocampus and prefrontal cortex were processed for P-ERK1/ 2 detection using Western blotting.Results (1) The body weight gains on the seventh [(75±22)g] and fourteenth stressed days [(70±24)g] were significantly lower in stressed group than that in controls [(101±35) g vs.(115±47)g,P＜0.05].(2) The defecation in apparatus controls[(1.4±1.9) boll] was more than that in controls [(0.4±1.0) boli] and stressed group [(0.1±0.3) boll,P＜0.05],and the locomotion [(2077±1245) cm] was significantly lower in stressed group than that in controls [(2990±1038) cm] and apparatus controls [(3110±1462) cm,P＜0.05].(3)The saccharine solution consumed [(11±6)g] and the ratio of the consumed saccharine solution to consumed total liquid [(37±16)%] in stressed group were significantly lower than that in controls [(15±4) g and (47±15) % respectively,P±0.05].(4) The P-ERK2 level in hippocampus [(46±95)%] and prefrontal cortex[(65±24)%] was markedly lower in stressed group than that in controls [(76±30)% and (99±42)% respectively,P＜0.05].Conclusions The results indicate that chronic forced swimming stress induce depression,and decrease the P-ERK2 level in hippocampus and prefrontal cortex.

Objective To investigate the effects of clozapine on Dizocilpine maleate (MK-801)induced hypoglutamatergic schizophrenia model in mice.Methods (1) To investigate the effects of clozapine on explorative behavior and spontaneous activity, three doses of clozapine (1.0, 1.5, 2.0 mg/kg, i.p.) or vehicle was injected 30 min before the test. Locomotor activity was recorded for 30 min with an automated video tracking system, in which the components of the locomotor activity were divided into exploration (the first 10 min) and spontaneous activity (the second 20 min). (2) To examine the effects of clozapine on MK-801-induced hyperlocomotion and stereotyped behavior, mice were administered with clozapine (1.0, 1.5 and 2.0 mg/kg) or vehicle 5 min before administration of MK-801 (0.25 mg/kg). After the second injection, locomotor activity was recorded for 90 min by the video tracking system; stereotyped behavior was evaluated for 90 min with a ranked intensity scale by two raters who were blind to the drug treatment.Results Clozapine (1.5 and 2.0 mg/kg) significantly inhibited the explorative behavior and spontaneous activity (P0.001). Clozapine at 1.0 mg/kg did not affect exploration and spontaneous activity (P0.05). Clozapine (1.0-2.0 mg/kg) dose-dependently antagonized MK-801-induced hyperlocomotion. (2) Clozapine(1.5-2.0 mg/kg)dose-dependently reduced MK-801-induced stereotypy. The lowest dose (1.0 mg/kg) had no effect on stereotypy induced by MK-801. Conclusion Clozapine has the selective action on the brain region. At lower dose, clozapine selectively inhibited hyperlocomotion mediated by the mesolimbic/mesocortical system while at higher doses it inhibited hyperlocomotion and stereotypy mediated by both mesolimbic/mesocortical and nigrostriatal systems.

Effects of clozapine on the hypoglutamatergic schizophrenia model in mice

Objective To investigate brain functioning during attention task in patients with schizophrenia by using functional magnetic resonance imaging.Methods Fourteen paranoid schizophrenic patients and sixteen healthy volunteers underwent fMRI when performing Stroop test.Results (1) Left inferior frontal,left superior temporal and inferior temporal gyrus was activated significantly in patients (the activated number was 11/3,6/8,6/8 respectively) than that in controls (the activated number was 6/10, 1/15,1/15 respectively) during Stroopl test (P＜0.05).Right middle frontal,left anterior eingulate gyrus (ACC) in schizophrenic patients (both activated number was 4/10 ) was activated poorly than that in controls (the activated number was 12/4,14/2 respectively) during Stroop2 test (P＜0.05).However,left superior frontal gyrus of patients (the activated number was 9/5 ) was activated significantly than that of controls (the activated number was 3/13) during Stroop2 test (P＜0.05).(2) Activation volume of right DLPFC,left ACC was significantly less during Stroopl test in patients (the activated volume was 256±579, 18±59 respectively) than that of controls (the activated volume was 298±597,67±87 respectively) (P＜0.05).Activation volume of bilateral DLPFC,left ACC during Stroop2 test was less in patients (the activated volume was 73±190,80±245,17±28 respectively) than that in controls (the activated volume was 425±800,414±703,76±98 respectively) (P＜0.05).Conclusion The findings of dysfunction in prefrontal lobe,left ACC and temporal lobe in schizophrenia give further support that these dysfunction brain areas (circuits) may be related to schizophrenic patients' attention deficit.

精神分裂症患者注意任务期间的脑功能障碍：一项功能性磁共振成像研究

目的 探讨舞蹈治疗对慢性精神分裂症患者的辅助作用.方法 在药物治疗的同时,对30 例慢性精神分裂症患者辅以舞蹈治疗( 治疗组) ,30 次为一个疗程;采用简明精神病评定量表( B P R S) 和阴性症状量( S A N S) 表于治疗前后分别进行评定,并与单纯药物治疗的30 例慢性精神分裂症患者( 对照组) 进行对照.结果 经舞蹈治疗后,治疗组 B P R S 和 S A N S评分中位数分别为26 和17 ,均低于治疗前( B P R S和 S A N S 分别为31 和27 ; P＜ 0?05 ~0?01) ,亦低于对照组( 分别为28 和19 ; P＜0?05 ~0?01) .结论 辅以舞蹈治疗对于慢性精神分裂症患者精神症状的缓解、社会功能恢复,及康复治疗均有积极意义.

【Abstract】 After the epidemic of COVID‑19 has been basically under control interiorly, China is under the situation of regular epidemic prevention and control. Hence a reflection on the problems about the risk of gaming disorder in the epidemic period can be helpful to mental health professionals who prevent and control gaming disorder. The authors clarified the improper publicity of the gaming industry and false media report, and also reviewed the trend of game using was rapidly rising and game market was rapidly expanding. Consequently, as the author addressed, the health risks due to excessive gaming patterns were increasing. This review also pointed out and analyzed some new trend of the expansion of female game users during the epidemic, and the further in-depth integration of games and gambling. Finally, some advice was provided to the mental health workers to cope with the increasing risk of gaming disorder during a post-epidemic era. © Chinese Journal of Psychiatry 2021.

COVID‑19 and the risk of gaming disorder: a reflection in the situation of regular epidemic prevention and control

COVID-19与游戏障碍风险：常态化疫情防控下的反思

Three patients diagnosed with COVID-19 were all young women in their thirties who have suffered from Internet violence in their personal life after hospitalization. They showed significant emotional distress such as, depression state, acute stress disorder, and dissociative disorder. The current study adopts short-term, individualized and comprehensive psychological interventions, including psychological support, encouragement, listening, safety confirmation, catharsis, psychological suggestion, and stimulation of internal potential to treat patients. The third case was provided with psychological interventions combined with antipsychotic treatment. After timely psychological interventions all three patients achieved sound results. © 2021 Chinese Medical Journals Publishing House Co.Ltd.

Psychological intervention for three COVID patients who suffered online violence

三名遭受网络暴力的COVID患者的心理干预

Objective
To investigate the differences of socio-demographics and smartphone use features among Chinese college students between problematic smartphone user (PSU) and non-PSU, and to clarify the link between PSU and psychopathological symptoms (anxiety and depression).


Methods
A total of 1 205 undergraduates from 35 classes of 8 universities or colleges, stratified with different school levels, were selected using the cluster random sampling strategy. The Problematic Cellular Phone Use Questionnaire (PCPUQ) was used to identify problematic use of smartphone use, and Zung Self-Rating Anxiety Scale (SAS) and Zung Self-Rating Depression Scale (SDS) were used to measure anxiety and depression symptoms.


Results
The prevalence of PSU among undergraduates was estimated to be 20.6%(207/1 006). There were significant differences between PSU and non-PSU on their major of either science category or humanities category (χ2=11.424, P<0.01), grade (χ2=5.868, P<0.05), monthly living expense supported by their family (χ2=12.713, P<0.01), daily time spent on their smartphones (χ2=20.052, P<0.01),frequency of changing the mobile phones (χ2=14.511, P<0.01) and monthly smartphone bill (χ2=13.107, P<0.01). Furthermore, the self rated anxiety (34.8% (72/207) vs. 14.6% (117/799), χ2=43.704, P<0.01) and depression (36.2% (75/207) vs. 15.4% (123/799), χ2=45.159, P<0.01) were both found more elevated in PSU than non-PSU.


Conclusion
Some common sociodemographic variables and smartphone usage patterns were identified in undergraduates displaying PSU. Moreover, PSU is associated with more elevated anxiety and depression symptoms.
