目的 研究阻断泛素-蛋白酶体通路与人肝癌细胞药物敏感性的关系及可能机制.方法 以不同浓度的特异性泛素-蛋白酶体抑制剂MG-132处理人肝癌细胞株HepG2作为实验组,化疗药物为氟尿嘧啶、表阿霉素、丝裂霉素.MTT法检测细胞的代谢活性(IC50值),流式细胞术和Hoechst荧光染色检测细胞凋亡,半定量逆转录-聚合酶链反应(RT-PCR)和Western blot检测caspase-3基因的转录水平和蛋白水平.结果 实验组的IC50值明显低于对照组,细胞凋亡率、caspase-3的转录水平及蛋白水平明显高于对照组.结论 阻断泛素-蛋白酶体通路能增加人肝癌细胞的药物敏感性,其机制可能与caspase-3基因的表达水平升高有关。

Experimental Study the on Relationship between Inhibiting the Ubiquitin-Proteasome Pathway and Drug Sensitivity in Human Liver Carcinoma Cell

人肝癌细胞中抑制泛素-蛋白酶体途径与药物敏感性关系的实验研究

Objective:To systematically evaluate the efficacy and clinical value of early enteral nutrition (EEN) combined with microecological agents in the treatment of severe acute pancreatitis (SAP).Methods:China National Knowledge Infrastructure, Chinese biomedical literature database, Wanfang Database, VIP, Cochrane Library, PubMed, Embase, and Web of Science were analyzed, and the retrieval time range is from the establishment of the datebase to November 1, 2019. To compare the clinical efficacy of EEN combined with microecological agents (experiment group) and single EEN treatment (control group) in SAP patients, and to compare the main outcome indicators: serum C-reactive protein level, incidence of multiple organ dysfunction syndrome, incidence of pancreatic infection necrosis, incidence of other complications, mortality, and length of hospital stay. And secondary outcome measures: plasma interleukin-8 (IL-8), tumor necrosis factor-α level, gastrointestinal score, and incidence of surgical intervention. The quality of the included literature was evaluated by using the Cochrane systematic evaluator's manual 5.1.0 risk of bias assessment tool, and meta-analysis was performed by using Stata16.0 software.Results:A total of 762 patients were enrolled in 9 RCTs. The results of meta-analysis showed that: among the main outcome indicators, C-reactive protein level [Mean Difference (
 MD)=-7.58, 95%
 CI: -23.71-8.55, P>0.05], incidence of multiple organ dysfunction syndrome [Logarithm Risk Ratio (Log
 RR)=-0.30, 95%
 CI: -0.71-0.10, P>0.05], incidence of pancreatic infection and necrosis (Log
 RR=-0.21, 95%
 CI: -0.57-0.16, P>0.05) and mortality rate (Log
 RR=0.13, 95%
 CI: -0.36-0.62, P>0.05) the differences were not statistically significant. The incidence of complications in the experimental group was significantly lower than that in the control group (Log
 RR=-0.29, 95%
 CI: -0.51-0.07, P<0.05), and the length of hospital stay in the experimental group (
 MD=-4.45, 95%
 CI: -7.47--1.43, P<0.05) was significantly shorter than that in the control group. Plasma IL-8 levels (
 MD=-7.43, 95%
 CI: -14.28--0.57, P<0.05), TNF-α level (
 MD=-38.96, 95%
 CI: -72.96--4.95, P<0.05)and gastrointestinal score (
 MD=-0.15, 95%
 CI: -0.17--0.13, P<0.05)were significantly lower in the experimental group than in the control group, and the incidence of surgical intervention was significantly lower than that of the control group (Log

 CI: -8.96-0.57, P>0.05) no statistical significance.
 Conclusion:EEN combined with microecological preparations can reduce the length of hospital stay in SAP patients and the incidence of complications. Therefore, EEN combined with microecological agents may be beneficial for SAP patients.

The progress and advance on diffusion-weighted imaging in locoregional interventional treatment of hepatocellular carcinoma

肝细胞癌局部介入治疗中扩散加权成像的进展与进展

Objective:To study the influencing factors in predicting conversion of laparoscopic liver tumor resection to open surgery to provide references for selection of patients for laparoscopic hepatectomy.Methods:The clinical data of 222 patients who were planned to undergo laparoscopic liver tumor resection at Ningbo Medical Center Lihuili Hospital from December 2015 to November 2018 were analyzed retrospectively. There were 146 males and 76 females, with an average age of 58.1 years. These patients were divided into the conversion group (
 n=24) and the laparscopic group (
 n=198) according to whether intraoperative conversion to open surgery was carried out. Chi-square test was used to evaluate relevant factors of conversion. Logistic regression analysis was used for multivariate analysis and to find out independent risk factors of conversion.
 Results:The conversion rate was 10.8% (24/222). Univariate analysis showed that hypertension, history of abdominal surgery, liver cirrhosis, tumors located at posterosuperior segments and major liver resection were significantly associated with conversion (all P<0.05). Multivariate analysis showed that the risk factors which were independently associated with conversion were liver cirrhosis (
 OR=2.611, 95%
 CI: 1.018-6.701), tumors located at posterosuperior segments (
 OR=6.115, 95%
 CI: 2.207-16.941), and major liver resection (
 OR=3.361, 95%
 CI: 1.150-9.825)(all P<0.05).
 Conclusion:Patients who were planned for laparoscopic liver tumor resection with liver cirrhosis, tumors at posterosuperior segments or major liver resection were at higher risks of conversion to open surgery.

Influencing factors in predicting conversion of laparoscopic liver tumor resection to open surgery

预测腹腔镜肝肿瘤切除术转为开腹手术的影响因素

Objective
To analyze the clinical characteristics, diagnosis, treatment and prognostic factors of primary hepatic neuroendocrine tumor (PHNET).


Methods
To analyze the clinical data of 21 patients with PHNET who were admitted to the First Affiliated Hospital of Zhengzhou University from January 2012 to January 2019. There were 11 males and 10 females, with ages which ranged from 36 to 75 years. Log-rank test was used for single-factor analysis of the relationship between clinical and pathological indicators and prognosis. Cox proportional risk model was used for multi-factor analysis.


Results
Among the 21 PHNET patients, 10 presented with central and upper abdominal pain, 3 abdominal pain accompanied by nausea and vomiting, 5 abdominal distension, and 3 were asymptomatic. Carbohydrate antigen (CA) 19-9 was elevated in 6 patients and CA125 in 7 patients. Abdominal enhanced CT showed solid liver space occupying lesions. Four patients had solitary and 17 had multiple lesions. The mean diameter was 58 mm. Fourteen patients had lymph node metastasis. Five patients underwent radical hepatectomy, 2 transcatheter hepatic arterial chemoembolization + chemotherapy, 12 chemotherapy, and 2 supportive treatment. Factors which were associated with prognosis of PHNET patients were surgery, tumor grading, cytokeratin positivity and lymph node metastasis. Multivariate Cox regression analysis showed that inoperability (HR=8.99, 95%CI: 1.13-71.80) was an independent risk factor of prognosis of PHNET patients. The prognosis in patients who underwent surgical resection was better.


Conclusion
Patients with PHNET had no specific clinical manifestations. Surgical resection gave the best results in treatment. As surgical resection affected prognosis, it should be carried out if technically feasible.
