The Hellenic Journal of Cardiology (International Edition, ISSN 1109-9666) is the official journal of the Hellenic Society of Cardiology, aimed at publishing high-quality articles on various aspects of cardiovascular medicine. A primary goal is to feature original research articles related to clinical and basic research in each issue, many of which will include invited editor comments. Hot topics such as molecular cardiology, innovative cardiac imaging, and electrophysiological mapping techniques will frequently appear in the journal in the form of articles or special reports from invited experts. The editorial board also places a strong emphasis on topics related to continuing medical education, guideline implementation, and the cost-effectiveness of cardiology.

VA ECMO has emerged as an effective rescue therapy in patients with cardiogenic shock refractory to standard treatment protocols and its use is rising worldwide in the last decade. Although experience and availability are growing, outcomes remain poor. There is need for evidence to ameliorate clinical practice and improve outcomes.

Central versus Peripheral VA ECMO for cardiogenic shock: an 8-year experience of a tertiary cardiac surgery center in Greece

– Tricuspid regurgitation (TR) adversely affects prognosis following mitral transcatheter edge-to-edge repair (TEER). We aimed to derive a risk stratification tool for patients undergoing TEER for mitral regurgitation while exhibiting significant TR. – This is a single-center, retrospective analysis of 217 consecutive individuals referred to an isolated mitral TEER who had moderate-to-severe or greater TR at baseline. The primary outcome was the 1-year composite of all-cause mortality or heart failure hospitalizations. The cohort was randomly split in a 75%-to-25% ratio, creating train (n=163) and test (n=54) datasets. Model development, discrimination and calibration were based on the train dataset. Internal validation was applied to the test dataset. – Overall, 81 (37.3%) patients experienced the primary outcome. After multivariable analysis, a score for predicting the primary outcome was constructed that utilized a 0-to-3 scale, in which each point represented one of three baseline variables independently associated with this combined endpoint: serum B-natriuretic peptide (BNP) level >1,000 pg/mL, qualitative right ventricular (RV) dysfunction on transthoracic echocardiogram, and cardiac implantable electronic device (CIED). C-statistic of the model was 0.66 (95% CI, 0.57-0.75, p=0.002) and 0.75 (95% CI, 0.61-0.89, p=0.004) in the train and test datasets, respectively – representing comparable performance to current, more complex tools. Neither this BNP-RV-CIED (BRC) score nor other models were prognostically meaningful in 32 patients excluded from the main analysis who underwent a combined mitral-tricuspid TEER. – The BRC score is a simple clinical prediction tool that may aid in the triage of isolated mitral TEER candidates with significant pre-existing TR.

Outcome Prediction score for mitral transcatheter edge-to-edge repair in patients with concomitant significant tricuspid regurgitation

Left atrial appendage (LAA) closure (LAAC) procedures are conventionally performed using empirical fluoroscopic viewing angles. However, because the LAA is a highly variable anatomical structure, these angles cannot depict the LAA in the optimal position. The present study aimed to assess the efficiency of using a novel optimal fluoroscopic projection angle (OPA) for LAAC and to validate its feasibility. The OPAs of the derivation cohort were acquired using cardiac computed tomography angiography (CCTA) to assess its superiority for depicting LAA depth versus traditional working angles (TAs) of RAO 30°, CAU 20°. The practicability of OPA-guided LAAC was demonstrated by comparison between clinical data from the validation cohort and those from a propensity-score matched (PSM) control group, as well as randomized controlled studies investigating LAAC. Of 705 patients in the derivation cohort, the median OPA was RAO 46°, CAU 31°. Compared with TA, the OPA depicted a longer mean (± SD) LAA depth (5.1±4.4) mm and a larger orifice diameter (1.1±1.1 mm), (P<0.0001 for both). All 38 OPA-guided LAACs were successful, with a shorter mean procedure duration (42.9 ± 12.3 min versus [vs.] 107.2 ± 41.5 min; P<0.0001) and reduced device consumption (1.08 vs. 1.5 per case), compared with the PSM control group. At the 3-month follow-up, the incidence of peri-device leak was 52.6% (20/38) detected by CCTA, with a mean leakage of 1.6±0.8 mm. By unfolding the LAA depth and orifice diameter to a better view, OPA demonstrated the potential to optimize LAAC procedural efficiency, although further larger-scale studies are required to confirm this.

Epicardial stenosis and coronary microvascular dysfunction (CMD) may coexist in patients with chronic coronary syndrome (CCS). Microvascular resistance reserve (MRR) has been demonstrated to be a valid cross-modality metric using continuous saline infusion thermodilution and intracoronary Doppler flow velocity methods. This study aimed to investigate the prevalence and diagnostic concordance of CMD defined by MRR using two methods-stress transthoracic Doppler echocardiography (S-TDE) and the invasive bolus thermodilution method (B-Thermo)-in patients with functionally significant epicardial stenosis.

The integration of computational technologies into cardiology has significantly advanced the diagnosis and management of cardiovascular diseases. Computational cardiology, particularly through cardiovascular imaging and informatics, enables precise diagnosis of myocardial diseases by utilizing techniques such as echocardiography, cardiac magnetic resonance imaging, and computed tomography. Early-stage disease classification, especially in asymptomatic patients, benefits from these advancements, potentially altering disease progression and improving patient outcomes. Automatic segmentation of myocardial tissue using Deep Learning (DL) algorithms improves efficiency and consistency in analyzing large patient populations. Radiomic analysis can reveal subtle disease characteristics from medical images and can enhance disease detection, enable patient stratification, and facilitate monitoring of disease progression and treatment response. Radiomic biomarkers have already demonstrated high diagnostic accuracy in distinguishing myocardial pathologies and promise treatment individualization in cardiology, earlier disease detection, and disease monitoring. In this context, this narrative review explores the current state of the art in DL applications in medical imaging (CT, CMR, echocardiography and SPECT), focusing on automatic segmentation, radiomic feature phenotyping, and prediction of myocardial diseases, while also discussing challenges in integration of DL models in the clinical practice.
