Computer Methods in Biomechanics and Biomedical Engineering

For the first time in history, on March 17, 2020, the European Union closed all its external borders in an attempt to contain the spreading of the coronavirus 2019, COVID-19. Throughout two past months, governments around the world have implemented massive travel restrictions and border control to mitigate the outbreak of this global pandemic. However, the precise effects of travel restrictions on the outbreak dynamics of COVID-19 remain unknown. Here we combine a global network mobility model with a local epidemiology model to simulate and predict the outbreak dynamics and outbreak control of COVID-19 across Europe. We correlate our mobility model to passenger air travel statistics and calibrate our epidemiology model using the number of reported COVID-19 cases for each country. Our simulations show that mobility networks of air travel can predict the emerging global diffusion pattern of a pandemic at the early stages of the outbreak. Our results suggest that an unconstrained mobility would have significantly accelerated the spreading of COVID-19, especially in Central Europe, Spain, and France. Ultimately, our network epidemiology model can inform political decision making and help identify exit strategies from current travel restrictions and total lockdown.

Outbreak dynamics of COVID-19 in Europe and the effect of travel restrictions

Since the discovery of X-rays, medical imaging has played a major role in the guidance of surgical procedures. While medical imaging began with simple X-ray plates to indicate the presence of foreign objects within the human body, the advent of the computer has been a major factor in the recent development of this field. Imaging techniques have grown greatly in their sophistication and can now provide the surgeon with high quality three-dimensional images depicting not only the normal anatomy and pathology, but also vascularity and function. One key factor in the advances in Image-Guided Surgery (IGS) is the ability not only to register images derived from the various imaging modalities amongst themselves, but also to register them to the patient. The other crucial aspect of IGS is the ability to track instruments in real time during the procedure, and to portray them as part of a realistic model of the operative volume. Stereoscopic and virtual-reality techniques can usefully enhance the visualization process. IGS nevertheless relies heavily on the assumption that the images acquired prior to surgery, and upon which the surgical guidance is based, accurately represent the morphology of the tissue during the surgical procedure. In many instances this assumption is invalid, and intra-operative real-time imaging, using interventional MRI, Ultrasound, and electrophysiological recordings are often employed to overcome this limitation. Although now in extensive clinical use, IGS is often currently perceived as an intrusion into the operating room. It must evolve towards becoming a routine surgical tool, but this will only happen if natural and intuitive human interfaces are developed for these systems.

Image-guided surgery: From X-rays to Virtual Reality

The aim of the present study is to examine the impact absorption mechanism of the foot for different strike patterns (rearfoot, midfoot and forefoot) using a continuum mechanics approach. A three-dimensional finite element model of the foot was employed to estimate the stress distribution in the foot at the moment of impact during barefoot running. The effects of stress attenuating factors such as the landing angle and the surface stiffness were also analyzed. We characterized rear and forefoot plantar sole behavior in an experimental test, which allowed for refined modeling of plantar pressures for the different strike patterns. Modeling results on the internal stress distributions allow predictions of the susceptibility to injury for particular anatomical structures in the foot.

Foot internal stress distribution during impact in barefoot running as function of the strike pattern

The objectives of this study were to evaluate the tooth movement tendency during space closure in maxillary anterior teeth by various combinations of retraction force and intrusive force in a double-archwire lingual orthodontic system. Mini-implant-double slot lingual orthodontics system models of the bilateral maxillary first premolar extraction cases were constructed. Three-dimensional finite element models of the maxilla were constructed with definite position mini-implants (8 mm) and power arms (6 mm). Different retraction forces(50gf、100gf、150gf)were applied with the help of a nickel-titanium closed coil spring on the plate side. Intrusive forces(0gf、50gf、100gf)were applied with the help of the mini-implant between the two central incisors, and the initial displacements of the maxillary anterior teeth were analyzed. Variable amounts of displacements like controlled tipping, uncontrolled tipping, lingual crown tipping, labial root tipping, extrusion and distal crown tipping were observed in all the models, and these tendencies increased as the magnitude of retraction force increased, and these tendencies decreased as the magnitude of intrusive force increased. When the intrusive force was greater than or equal to the retraction force, the maxillary central incisors showed the trend of lingual crown tipping and labial root tipping, resulting in uncontrolled tipping movement. In terms of horizontal changes, the increasing width of bilateral anterior teeth was observed, with canines showing the least increasing trend. Various combinations of retraction force and intrusive force in a double-archwire lingual orthodontic system provide a new choice for torque control of the anterior teeth. Although anterior mini-implants and elastics can achieve incisor intrusion and lingual root torque, they cannot achieve the expected torque without additional torque control methods.

Finite element analysis of tooth movement under different retraction force and intrusive force in double-archwire lingual orthodontics system

In the invisible orthodontic treatment, composite thermoforming film materials have become the focus of orthodontic clear aligners. The orthodontic efficacy of clear aligners which consisted of multi-layers materials remains unclear. This study aims to evaluate the biomechanical effects of various multi-layers of clear aligners on en-mass retraction of maxillary anterior teeth.A patient-specific 3D non-linear finite element model numerical analysis was constructed to simulate the en-mass retraction of maxillary anterior teeth with clear aligner after extraction of the first premolars. Four kinds of multi-layers clear aligners with different proportion of film materials were simulated. The biomechanical responses of four different clear aligners on invisible orthodontics were calculated. The tooth displacement in all directions, the hydrostatic pressure of periodontal ligament, the orthodontic deformation of clear aligner, and the stress distribution of alveolar bone were compared and investigated.In all experimental models, the maximum equivalent deformation of alveolar bone, the vector displacement of tooth and the compressive/tensile stress of periodontal ligament decreased with the increase of soft layer thickness. The elastic strain of clear aligners also decreased with the increase of the ratio of soft/hard layers.The multi-layers clear aligner is better than the single-layer clear aligner in tooth movement, stress distribution of periodontal ligament and mechanical loading of alveolar bone, especially when the ratio of soft layer to hard layer is more than 50%. Moreover, the side effects of the multi-layers clear aligner are significantly less than those of the single-layer one.

Early cancer detection using T-cell receptor sequencing (TCR-seq) and multiple instances learning methods has shown significant effectiveness. We introduce a multiple instance learning method based on convolutional neural networks and self-attention (MICA). First, MICA preprocesses TCR-seq using word vectors and then extracts features using convolutional neural networks. Second, MICA uses an enhanced self-attention mechanism to extract relational features of instances. Finally, MICA can extract the crucial TCR-seq. After cross-validation, MICA achieves an area under the curve (AUC) of 0.911 and 0.946 on the lung and thyroid cancer datasets, which are 7.1% and 2.1% higher than other methods, respectively.

A rapid, sensitive, and low-damage method for isolating circulating tumor cells (CTCs) is crucial for cancer research. This study, based on dielectrophoresis (DEP) and finite element modeling, investigates multitarget cell separation from blood on microfluidic chips. The effects of electrode shape, dielectric conductivity, and flow rate on cell movement and separation efficiency were analyzed. The results showed optimal separation with a 90° electrode angle, 1.5 V applied voltage, and a 1:3 inlet flow rate ratio. This study provides valuable insights for optimizing DEP-based microfluidic devices to improve multitarget cell separation efficiency and purity.

Uterine fibroids are common benign gynecological tumors that are observed in up to 80% of premenopausal women. It is understood that as the fibroid size increases, the surrounding tissues will be subject to greater loads. However, the effect of fibroid region on the uterine structure is not as clear. To better understand the mechanical loading of the endometrium due to the presence of a uterine fibroid, we developed a finite element model of the uterus to examine the effect of both fibroid depth and region in relation to the endometrium. The finite element model of the uterus, endometrium, and a uterine fibroid were created from a 3D segmentation of a patient's magnetic resonance images. This model was then loaded into ANSYS Mechanical 2023 R1, and then deformation, stress, and strain of the endometrium was measured for 24 fibroid positions (8 regions × 3 depths). The highest endometrial loads (deformation, stress, strain) were observed when the fibroid region was superior to the uterus and the depth was deep. Superior regions generated 10–20% higher loads on the endometrium in comparison to other regions, while deep locations had 5–10% higher endometrium loads when compared to superficial depths across almost all regions. A simple uterus model was used to show the effect of fibroid position on loads acting on the endometrium. This can provide insight into mechanisms of abnormal uterine bleeding and infertility and better inform clinical decision making.
