Purpose: Dental caries is a non-communicable, preventable disease that disproportionately affects low-income children in the United States (US). The purpose of this systematic review was to describe oral health education and promotion activities designed to prevent early childhood caries (ECC) provided by Early Head Start (EHS) programs in the United States. Methods: Five databases were searched including CINAHL, Dentistry & Oral Sciences Source through EBSCO, PubMed, Google Scholar, and the Wiley Online Library, to identify peer-reviewed quantitative studies published in English on oral health education and promotion activities within EHS programs from 2000 to 2019. Studies were assessed for eligibility using the Preferred Reporting Items for Systematic Reviews and Meta-Analyses flow diagram (PRISMA). Two researchers independently evaluated the included studies. Results: The initial search yielded a total of 363 articles. Following the screening process, five studies met the inclusion criteria (observational, n=2; quasi-experimental, n=3). The main outcome measures included oral health knowledge, attitudes and behaviors, oral health education, oral health promotion, and oral health activities. Three studies investigated the effectiveness of oral health education and promotion interventions among EHS staff and parents. Two studies examined oral health activities such as education, toothbrushing instructions, toothpaste use, dietary education, and dental assessment. Conclusion: Studies that focused on increasing pediatric oral health knowledge and practice behaviors among both EHS staff members and parents reflected positive outcomes. Ongoing research is needed to examine the effectiveness of oral health education and promotion activities as they relate to the oral health outcomes of children enrolled in EHS programs.

Oral Health Education and Promotion Activities by Early Head Start Programs in the United States: A systematic review.

Purpose: Orofacial manifestations of Lyme disease can affect head and neck anatomical structures that are frequently examined by dental professionals. The purpose of this systematic review was to examine the literature for types and frequencies of orofacial manifestations documented in populations in the United States (US) with Lyme disease.Methods: Four electronic databases (Dentistry and Oral Sciences, PubMed, Cinahl Plus, and Medline) were systematically searched during the summer of 2019 using keywords and MeSH terms to identify relevant studies. Search term alterations and synonyms were cross-checked using the US National Library of Medicine Unified Medical Language System Metathesaurus. Full-text, English language studies were included if they reported on US populations with Centers for Disease Control and Prevention confirmed cases of Lyme disease. The review followed guidelines set forth in Preferred Reporting Items for Systematic Reviews and Meta-Analysis (PRISMA). Quality was assessed with a modified version of the Cochrane Data Collection Form for Randomized Control Trials and Non-randomized Control Trials. Extracted data was organized by themes of manifestations and the frequencies were calculated.Results: An initial search extracted 217,381 articles; 43 met the inclusion criteria and were further reviewed for quality. Twelve articles published from 1992-2017 were deemed appropriate for inclusion. All were from non-dental journals and fewer than half (n=6) reported on Lyme disease endemic states. Eight incidences of orofacial manifestations within head/neck regions were documented in Lyme disease patients (n=951) and included: headache (39.5%), facial palsy (42.5%), temporomandibular joint arthralgia (42.0%), altered taste (11.0%), stiff neck (13.6%), sore throat (3.0%), neck pain/arthralgia (7.5%), and erythema migrans rash (5.2%).Conclusion: Eight orofacial manifestations of Lyme disease were revealed by this systematic review. Future research regarding the orofacial manifestations of Lyme disease is needed so this medical condition can be better understood by oral health care providers and result in improved health outcomes for infected patients.

Purpose: Dental hygienists perform precision instrumentation tasks repetitively throughout the workday, placing them at increased risk for developing a musculoskeletal disorder. The purpose of this pilot study was to determine differences in muscle activity and pinch force generation between the traditional scaling technique and a modified scaling technique.Methods: A convenience sample of dental hygienists (n=12) acted as their own controls in this counterbalance-designed pilot study. Muscle activity and pinch forces were assessed while participants performed traditional and modified scaling techniques with designated instruments on artificial calculus applied to the lower left quadrant of a typodont, for a period of five minutes. Surface electromyography was used to measure muscle activity; sensors attached to the instrument handle measured pinch forces. Participants were surveyed regarding the instruments used and scaling technique preferences at the conclusion of the session. Parametric and non-parametric tests were used to analyze the data. Descriptive statistics were used to analyze the exit survey.Results: The modified scaling technique required less muscle activity than the traditional technique while scaling, however results were not significant (p>0.05). The traditional scaling technique required greater overall pinch force during scaling (p=.00). Pairwise comparisons revealed significant differences between pinch force generation in the thumb for the two scaling techniques (Z = -2.401, p= 0.016) and in the index finger (Z = -2.223, p= 0.026). The traditional scaling technique generated more pinch force (thumb x=7.25±4.99, index finger x=2.86±2.14) when compared to the modified scaling technique (thumb x=4.52±2.32, index finger x=1.65±1.28). Participants had a slightly higher preference for the instrument utilized for the modified scaling technique in terms of balance, maneuverability, overall comfort and the associated scaling technique as compared to the instrument utilized for the traditional scaling technique.Conclusion: Use of a modified scaling technique may reduce muscle activity and pinch force generation as compared to the traditional lateral pressure scaling technique during instrumentation. Future research on ergonomic scaling techniques is needed to determine their efficacy and impact on musculoskeletal disorders.

The Effects of the Traditional Scaling Technique Versus a Modified Scaling Technique on Muscle Activity and Pinch Force Generation: A pilot study.

Purpose: Workplace burnout in academia is a problem that affects career satisfaction and longevity. The purpose of this study was to determine the prevalence of burnout among entry-level dental hygiene program directors.Methods: The Copenhagen Burnout Inventory (CBI) survey was used to determine prevalence of burnout in 325 dental hygiene program directors from across the United States. The survey was disseminated electronically. The CBI contains 19 questions that measure overall, personal, work-related, and client/student-related burnout on a five-point Likert type scale. The survey also included nine demographic and three open-ended questions related to burnout. Descriptive statistics, one sample t-tests, and one-way ANOVA tests were used to analyze the data.Results: One hundred twenty-seven dental hygiene program directors completed the survey for a 39.1% response rate. Most participants (62.2%, n=79) indicated moderate to high burnout on the personal burnout subscale, approximately one half (51.2%, n=65) on the work-related burnout subscale, and one third (33.1%, n=42) on the client/student-related burnout subscale. No statistically significant differences were found when comparing mean scores between directors of two-year and four-year program or between participants under age 50 and those 50 years of age and older (p-values>0.05). Program directors with teaching workloads of 51-60% had significantly lower burnout on the work-related burnout subscale when compared to participants with teaching workloads of 31-40% (p=0.045). Participants with the lowest workload allocations for administrative duties had higher overall mean burnout scores.Conclusion: Results from this study suggest one out of two dental hygiene program directors have symptoms of some type of burnout with the highest prevalence rate in the personal burnout subscale. Findings underscore the need for further research to identify stressors that lead to burnout as well as identify prevention strategies that promote a healthier work climate for dental hygiene program administrators.
