Abstract Type 2 diabetes mellitus (T2DM) remains a major global health threat, claiming millions of lives annually. Despite recent advancements in managing T2DM, the need for innovative treatment options persists. Sodium-glucose cotransporter 2 (SGLT2) inhibition has proven to be an effective therapeutic strategy against T2DM, offering benefits in lowering hemoglobin A1c (HbA1c) and plasma glucose levels as well as treatment for patients with diabetes who develop chronic kidney disease (CKD). Bexagliflozin is a recently approved, orally administered SGLT2 inhibitor for treating T2DM and exhibits promise in combating CKD stages 3a and 3b. Structurally, bexagliflozin differs from other SGLT2 inhibitors in that it has a cyclopropyloxyethoxy group at the para position of the peripheral phenyl ring. This review article provides an overview of bexagliflozin’s discovery, mechanism of action, binding site interactions, metabolism, pharmacokinetics, and clinical applications. Emphasis is placed on the significant contribution of SGLT2 inhibitors, particularly bexagliflozin in glycemic control and treating associated comorbidities such as heart failure and chronic kidney disease in T2DM patients.

Bexagliflozin: a comprehensive review of a recently approved SGLT2 inhibitor for the treatment of type 2 diabetes mellitus

The acid-base properties and the Cu(II) binding processes of methotrexate (MTX) were characterized by selected spectroscopic techniques and potentiometric measurements. The pH titration data showed that MTX behaves as a triprotic ligand. The deprotonation constants were determined for α-COOH and γ-COOH groups and (N1)H+ from the pteridine ring. Taking all the obtained results into consideration, a coordination pattern was proposed. The DNA-cleaving activity and reactive oxygen species (ROS) generation were investigated for both MTX and the Cu(II)-MTX system. The complex displayed a promising nuclease activity toward plasmid DNA in the presence of hydrogen peroxide. Interestingly, the induction of ROS, such as hydroxyl radicals, superoxide anions or singlet oxygen, was excluded and a different mechanism of DNA degradation was proposed. As MTX is now commonly used in anticancer therapy i.e. against lung cancer, basic cell-based studies were carried out to establish if its Cu(II) complex exhibits higher cytotoxic properties than the ligand alone. Activities of both compounds were also tested against colon carcinoma. Moreover, the determined values of IC50 were confronted with the cytotoxic activity of cisplatin.

Abstract Periodontal disease begins with bacterial plaque buildup in the oral cavity, inciting an inflammatory response that results in subsequent tissue damage. Even after standard treatment like scaling and root planning (SRP) to remove plaque and biofilm, the host immune response can remain hyper-active, perpetuating further tissue destruction. In these cases, aggressive periodontitis is resistant to SRP and the inflammatory response may persist, even in the absence of plaque, presenting a significant clinical challenge. Previous experiments have provided a validated model of periodontal inflammation by exposing murine macrophages to lipopolysaccharide (LPS) from Aggregatibacter actinomycetemcomitans ( Aa ), a pathogen linked to aggressive periodontitis. Using this model, we have previously demonstrated that the periodontal disease microenvironment triggers epigenetic changes, notably heightened lysine-specific demethylase 4B (KDM4B) activity. Data indicate that the KDM4B inhibitor ML324 can reverse the macrophage-mediated pro-inflammatory response induced by Aa LPS in vitro, providing compelling evidence for KDM4B as a rational therapeutic target for periodontal disease. In the present studies, a cohort of compounds was developed as potential KDM4B inhibitors. Synthesis and characterization of derivatives led to the discovery of compound 14 with an IC 50 of 170 nM against KDM4B and immunosuppressive activity in the Aa LPS challenge model. These results suggest KDM4B inhibitors as potential therapeutic agents for modulating the immune response for periodontal disease.
