Facial transplantation adheres to principles analogous to those of autologous reconstruction of composite facial defects. The aesthetic subunits of the face are comprised of skin and the underlying soft and bony tissues. Their appearance is derived from the thickness and contour of the underlying tissues, with critical support provided by the skeletal buttresses in each facial region.1 Combinations of aesthetic incisions including coronal, preauricular, and posterior neck may be used to conceal scars.

The skeletally mature adult skull is not simply a larger version of the infant or adolescent skull (Fig. 1).1,2 The differential growth of the skull and craniomaxillofacial complex depends on several factors that occurs on a continuum from embryologic development, infancy, adolescence, and into young adulthood, and the synergistic relationship between presence of soft/hard tissues and function (Fig. 2).3–6 The theory states that “any given bone grows in response to functional relationships established by the sum of all the soft tissues operating in association with that bone.” Furthermore, it states that bone and its osteogenic membranes do not genetically regulate the rate and directions of their own growth.

Pediatric Mandibular Reconstruction

Immediate nerve allograft reconstruction of the inferior alveolar nerve (IAN) results in functional sensory recovery (FSR)1–4 and improved quality of life5–8 for patients who undergo mandibular resection for benign and malignant disease. Historically, mandibular resection involving the IAN resulting in a nerve defect was left without reconstruction for a variety of reasons, including a focus by the surgeon on surgical resection of the specific pathologic condition without the additional operative time required for nerve grafting, lack of availability of an ideal nerve autograft (sural nerves9 and/or greater auricular nerves were used10,11) or allograft (not available before 2008), and a lack of microneurosurgical expertise.

Neurosensory Reconstruction of the Mandible

The ideal flap for head and neck reconstruction is a ‘composite tissue constituent flap’ that can replace all components of an ablative defect and is easy to harvest with minimal morbidity to the patient. The DCIA can fulfill these requirements and be amenable for a two-surgeon approach thus reducing operative time. As a donor unit, the DCIA can be harvested as an osseous or an osseous with the soft tissue component of the over lying muscle with or without skin. The flap is based on the deep circumflex iliac artery and vein with an average pedicle length of 8 cm with vessels between 1.5 and 3 mm in diameter.
