Our experiences color our practice lens, motivating and guiding our efforts. They define our need to renew and refocus interventions and energies. The unique nature of these efforts prompts us to validate the individuality of issues faced with clients. My respect for this principle of self-determination drives my case management practice in how I approach that intimate world of client systems during their death and dying processes. From the earliest phase of awareness to anticipatory grief and mourning, through death itself and then bereavement, the critical importance of examining the course of themes from an intergenerational perspective sets a tone for practice-a tone that embraces age-specific concepts for our clients and their support community, including the professionals involved.

For more than 75 years, the University of Wisconsin Hospital and Clinics (UWHC) has provided care for its community. Today, UWHC has 476 licensed beds served by a staff of 893 physicians and 495 residents and fellows. The hospital shares its facility with UW-Madison School of Nursing, UW Medical School, and UW Comprehensive Cancer Center. With 6185 employees, UWHC has grown significantly over the years. For fiscal year 2004, inpatient admissions were 20,617, outpatient visits were 518,841, and emergency department (ED) visits were 33,543.

Patient safety has become a key issue in health care since the Institute of Medicine (IOM)1 released the report To Err Is Human: Building a Safer Health System. This landmark report examined the extent of preventable patient injuries and deaths occurring in US hospitals. It was estimated that between 44,000 and 98,000 people die annually as a result of medical errors and that nearly half were preventable. Subsequent studies2 suggest that the medical error rate is even higher. These statistics are a call to action for case managers to explore creative ways to implement patient safety practices in their systems and procedures.

Case management (CM) has developed an increasingly high profile as a means to improve care, given the demographic realities of an aging population coupled with an increasingly expensive, complex, and fragmented health care delivery system. Case management can create the bridges necessary to provide the level of coordination needed to help patients and their families get the best health outcomes possible. Although they may seem to be conceptually clear, the definitions, structure, organization, scope of services, and goals for CM vary widely among organizations, locations, and payers.

The concept of the "older population" is changing as increasing numbers of baby boomers begin to age, bringing more and more clients to the attention of case managers. Alcohol use and abuse can mimic symptoms of illness and have negative consequences for the older population across physical, social, and psychological parameters. It is therefore necessary in good case management to consider the impact of substance abuse and addiction in the older and geriatric populations. The unique nature of this problem gives us much to consider, and a systematic approach can promote a timely resolution.
