Prostatitis is a common disorder which remains poorly understood. The causes of nonbacterial prostatitis and prostadynia are unclear, but new molecular methods show promise in uncovering some infectious agents. Bacterial prostatitis constitutes 5 to 10% of the total cases of prostatitis, but causative organisms are well-delineated. Unfortunately the high initial therapeutic success rates using antibiotics directed against these susceptible organisms are too often associated with high relapse rates. Quinolones, such as ciprofloxacin, ofloxacin, and trovafloxain, are the agents of choice for antibacterial therapy of prostatitis. More information is needed concerning the antibiotic levels and activity in infected prostatic tissue and secretions, optimum duration of therapy, and benefit of long-term suppressive therapy.

A minimum dataset (MDS) has the potential for many uses in the complementary and integrative medicine (CIM) setting. Methods such as comparative effectiveness (CER) are conducted in real-life clinical settings using data sourced from clinical records and patient reported outcomes (PROs), which is then collated into a MDS of high quality to provide information both immediately and over time. Other uses of a CIM-MDS include surveillance and monitoring CIM use.Strategies for establishing a CIM-MDS in Australia were explored. The focus was data sources, especially the role of PROs. The findings drew on a body of research that included a case study of an IM primary care clinic; interviews with patients, practitioners, and staff from the clinic and a systematic literature review of patient questionnaires for use in the IM setting.Aside from basic information, automated data extraction of clinical data from Australian CIM clinics is very limited. A small battery of patient (and possibly practitioner) questionnaires may the best way to begin obtaining data. Patient and practitioners may well seek benefits other than contributing to research from the exercise of data collection, such as accessing individual patient results to track outcomes and inform clinical care. The format of the questionnaires matters as well. Although electronic formats are acceptable to many patients, paper questionnaires are still preferable to some.A bottom-up approach that involves all stakeholders and builds on other national and international initiatives is recommended for developing a CIM-MDS. The final choice of data for a CIM-MDS will be informed by its intended uses. The lack of any standardised nomenclature for CIM coding is an important obstacle to building a robust dataset; however, in establishing a CIM-MDS there is the opportunity to collect data that could help inform a CIM coding system.

Processes for the credentialing and privileging of health professionals are of increasing importance and value to the U.S. health care system and to society. As efforts continue to provide and reward more efficient, affordable, and higher quality health care (the “triple aim” described by Berwick et al.1), the ability to ensure the capabilities and competence of the health professionals, including pharmacists, who practice within an increasingly complex and sophisticated system has become both more relevant and essential.Currently, all U.S.-educated pharmacists attain a fundamental set of credentials to qualify to enter practice—an accredited professional pharmacy degree and a license awarded upon successful completion of a national postgraduation examination administered by the National Association of Boards of Pharmacy on behalf of state boards of pharmacy. This process provides an established framework to assure stakeholders of the ability of pharmacists to provide care and services that reflect sound, entry-level practice. However, evolving patient care and health system needs and demands have heightened the requisite skills needed by pharmacists to deliver more complex services. Ongoing professional development and competency assessment are integral parts of health professionals’ expectations to maintain a contemporary practice. This resource guide on the credentialing and privileging of pharmacists has been developed to supplement the Council on Credentialing in Pharmacy (CCP)1 Guiding Principles for Post-licensure Credentialing of Pharmacists2 and to assist those who are introducing or enhancing a credentialing and privileging system for pharmacists within their health care settings. CCP does not provide the guide for use as a standard of practice nor intends to represent the content as best or expected practices.

A case of recurrent Staphylococcus lugdunensis central nervous system (CNS) infection associated with a ventricular peritoneal (VP) shunt is reported. A total of five S. lugdunensis isolates were isolated from the patient (four from ventricular fluid and one from a VP catheter tip). All five isolates share an indistinguishable pulsed-field gel electrophoresis profile. A full range of conventional biochemical tests and 16S rRNA gene nucleotide sequence analyses were performed to confirm identification of the organism. This is the first report of S. lugdunensis in catheter-related VP fluid infection. The isolate was capable of colonizing the CNS and was difficult to treat.
