Feline lower urinary tract disease.

Objectives The aim of the study was to investigate the effect of the non-steroidal anti-inflammatory drug meloxicam on the clinical course of obstructive idiopathic cystitis in cats in a placebo-controlled clinical study. Methods Thirty-seven cats with obstructive idiopathic cystitis were enrolled. Cats received supportive treatment and an indwelling transurethral catheter for 48 h. On days 0 and 1, all cats received buprenorphine 0.01 mg/kg subcutaneously every 8 h. On day 1, cats were randomly assigned to the meloxicam (n = 18) or placebo group (n = 19) and received meloxicam (0.1 mg/kg on day 1, 0.05 mg/kg on days 2-5) or placebo orally for five consecutive days. Cats were monitored by repeated physical examinations and urinalysis, and with a 5 day questionnaire filled in by the owners after discharge and a telephone interview 3 months after presentation. Parameters for evaluation of treatment success were the occurrence of recurrent urethral obstruction, results of physical examinations and questionnaires. Results Recurrent urethral obstruction occurred in 4/18 cats (22%) in the meloxicam group and 5/19 cats (26%) in the placebo group ( P = 1.000). General demeanour and pain on abdominal palpation during hospitalisation improved significantly in both groups ( P <0.001). After discharge, with regard to general demeanour, food intake and voiding behaviour, there were no significant differences within or between groups at different time points. Conclusions and relevance Orally administered meloxicam for 5 days did not influence the incidence of recurrent urethral obstruction and the recovery from clinical signs in cats with obstructive feline idiopathic cystitis. The persistence of clinical signs in most of the cats 1 week after initial presentation indicates that symptomatic treatment for a longer period of time is warranted.

Efficacy of intravesical pentosan polysulfate sodium in cats with obstructive feline idiopathic cystitis

Practical relevance: Abdominal ultrasound plays a vital role in the diagnostic work-up of many cats presenting to general and specialist practitioners. The biliary tree encompasses the liver, gall bladder and bile ducts, although only diseases affecting the latter two are discussed here. Diseases of the bile ducts and gall bladder are more common than those of the liver parenchyma and ultrasound plays an important role in their diagnosis. Clinical challenges: Despite ultrasonography being a commonly used modality, many practitioners are not comfortable performing an ultrasound examination or interpreting the resulting images. Even differentiating between normal variation and pathological changes can be challenging for all but the most experienced. In addition, a lack of pathological change does not necessarily rule out disease; for example, absence of gall bladder and/or extrahepatic biliary distension is not sufficient to exclude the possibility of biliary obstruction, and in many cases of cholangitis the liver and biliary tree are unremarkable on ultrasound examination. Equipment: Ultrasound facilities are readily available to most practitioners, although use of ultrasonography as a diagnostic tool is highly dependent on operator experience. Aim: This review, part of an occasional series on feline abdominal ultrasonography, discusses the appearance of the normal and diseased biliary system. It is aimed at general practitioners who wish to improve their knowledge and confidence in feline abdominal ultrasound and is accompanied by high-resolution images. Percutaneous ultrasound-guided cholecystocentesis is also covered. Ultrasound examination of the liver was discussed in an article published in January 2019 and an upcoming article will cover hepatic vascular anomalies. Evidence base: Information provided in this article is drawn from the published literature and the author’s own clinical experience.

Objectives The aim of this retrospective cohort study was to compare two groups of cats with lower respiratory tract disease, one with Mycoplasma felis detected by PCR in the bronchoalveolar lavage fluid (BALF) (M+) and the other without (M–), with regard to signalment, clinical signs, diagnostic results, treatment response and survival. Methods All cats for which M felis was investigated by PCR in BALF between 2016 and 2023 were included. Cats with evidence of oropharyngeal contamination, or for which PCR results were under the quantification level, or without follow-up information were excluded. Cats that had received antibiotics effective against M felis before BALF collection were excluded if PCR results were negative. Follow-up information was retrieved from the medical records and by contacting referring veterinarians and owners. Results A total of 55 cats were included (19 in the M+ group and 36 in the M– group). Significant differences were detected between the two groups in the prevalence of systemic signs (M+: 0%, M–: 28%; P = 0.01), bronchial collapse on bronchoscopy (M+: 28%, M–: 6%; P = 0.03), radiographic alveolar lesions (M+: 57%, M–: 24%; P = 0.04), and percentage of neutrophils (M+: 65%, M–: 35%; P = 0.002) and eosinophils (M+: 9%, M–: 25%; P = 0.03) in the BALF. Antibiotics were used more frequently in M+ cats (M+: 90%, M–: 42%; P = 0.001) than in M– cats. No significant difference was found in treatment response (short term: P = 0.94, long term: P = 0.28) and risk of death ( P = 0.42) between the two groups. Conclusions and relevance The presence of radiographic alveolar lesions and neutrophilia in BALF was significantly associated with the detection of M felis in BALF. This association might be causal, consequential or contextual (ie, sharing the same cause). The detection of M felis in BALF did not negatively impact prognosis but the necessity to treat M felis using targeted antibiotics remains to be determined.

Clinical and prognostic relevance of <i>Mycoplasma felis</i> PCR detection in feline lower respiratory tract disease

Objectives The aim of the present study was to evaluate the differences in intraoperative nociception, incision size and operative time between midline (OVE m) and flank ovariectomy (OVE f) in feral or stray cats. Methods Two groups of animals, the OVE m group (n = 19) and the OVE f group (n = 19), were evaluated at six intraoperative time points. Cats assigned to both groups were premedicated with dexmedetomidine (20 μg/kg IM) and methadone (0.2 mg/kg IM). General anaesthesia was induced with intravenous propofol to effect and maintained with isoflurane in 100% oxygen. The data collected included heart rate, non-invasive systolic and median blood pressure, respiratory rate, weight, body condition score, surgical time, incision size and rescue analgesia. Results Significant differences were observed between surgical approaches, with larger incision size ( P &lt;0.001) and longer surgery time ( P = 0.04) in the OVE m group. No significant differences between surgical approaches were detected regarding intraoperative nociception ( P &gt;0.05). Conclusions and relevance Based on intraoperative nociception, there is no strong reason to advocate a preferred surgical approach for feline OVE in sterilisation programmes; however, considering surgery time and incision size, the OVE f approach may contribute to the sterilisation of more animals and reduce the risk of wound dehiscence as it allows for smaller incisions to be made in animals where postoperative wound control is difficult or impossible.

Intraoperative nociceptive and clinical comparisons between ventral midline and flank ovariectomy in feral and stray cats

Objectives Window entrapment in cats can lead to reduced blood flow to the spinal cord, muscles and nerves, resulting in ischaemic neuromyelomyopathy. The severity and duration of entrapment greatly influence clinical and neurological outcomes, as well as prognosis. The aim of the present retrospective multicentric study (2005–2022) was to describe clinical, neurological and selected clinicopathological findings, as well as the outcome of cats trapped in bottom-hung windows, presented to both first-opinion and referral-only clinics. Methods The study included cats with detailed clinical and neurological evaluations at admission, along with at least one of the following biochemical parameters: creatine kinase (CK), aspartate transaminase (AST), alanine aminotransferase (ALT) activities, urea and/or creatinine. Clinical and neurological parameters evaluated in the study included rectal temperature, femoral pulse, gait, urinary bladder function, tail function and survival to discharge. Odds ratios (ORs) were calculated for survival and each clinical, neurological and biochemical variable. Results Of the 70 cats that met the inclusion criteria, only seven (10%) died or were euthanased during hospitalisation. Nevertheless, with the available data, we found evidence of an association between clinical and neurological status and survival, with tail function being the strongest association. Cats lacking tail sensation, motor function and/or tonus were more likely to die than cats with normal tail function or only mild abnormalities (OR = 24). Similarly, cats with severe hypothermia or an absent femoral pulse were less likely to survive (OR = 12.75 and 7.5, respectively). In this sample (with a relatively low number of deaths), we did not find evidence of an association between CK, AST and ALT activity with survival. However, the only two cats with severe increases in creatinine died. Conclusions and relevance Assessment of gait, urinary bladder function, femoral pulse, rectal temperature and particularly tail function is promising for predicting outcomes in cats with window entrapment trauma.

Window entrapment trauma in cats: clinical, neurological and clinicopathological findings and outcome (70 cases)

Objectives The objective of the study was to investigate the prevalence of hyperthyroidism in Spain, including its geographical distribution and prevalence across different age groups. Methods A cross-sectional study was conducted. Samples submitted to a reference laboratory to evaluate serum total thyroxine concentration (TT4) during a 3-year period were evaluated (n = 27,888). A cat was considered hyperthyroid if the TT4 concentration was greater than 60.4 nmol/l (4.7 μg/dl). Hyperthyroid cats were classified based on their TT4 concentration at diagnosis as mildly (TT4 60.4–124.8 nmol/l [4.7–9.7 μg/dl]), moderately (TT4 124.8–249.68 nmol/l [9.7–19.4 μg/dl]) or severely (TT4 &gt;249.6 nmol/l [&gt;19.4 μg/dl]) hyperthyroid. The samples were also grouped by life stage, into young adults (1–6 years old), mature adults (7–10 years old) and seniors (&gt;10 years old). The study included samples from 17 regions of Spain and geographical mapping was based on seven domains. Results One thousand seven hundred and twenty-two out of 27,888 cats (6.2%, 95% confidence interval [CI] = 5.9–6.5) were classified as hyperthyroid. Within the hyperthyroid group, ages ranged from 2 to 25 years old, with a median of 14 years. Within the hyperthyroid population, 14/368 (3.8%), 27/368 (7.3%) and 327/368 (88.8%) were young adults, mature adults and seniors, respectively. The prevalence of hyperthyroidism in this laboratory-based sample in Spain was 2.7 (95% CI = 0.4–5.7) to 6.9% (95% CI = 6.4–7.4), depending on the geographical region. During the study period, 8.2% of cases that were initially non-hyperthyroid were later diagnosed with hyperthyroidism. Conclusions and relevance Prevalence of hyperthyroidism in Spain is 6.2% (95% CI = 5.9–6.5) overall and 7.9% (95% CI = 7.2–8.8) in cats older than 10 years. The distribution of the disease is heterogeneous between different geographical regions. Further large-scale prospective studies are required to determine the risk factors in each region and understand the reasons for this variation.

Prevalence of feline hyperthyroidism in a laboratory-based sample of 27,888 cats in Spain

Objectives The aim of this study was to evaluate ground reaction forces (GRFs) in cats after unilateral total hip replacement (THR) and compare them with cats after femoral head and neck ostectomy (FHO). Methods The databases of the Small Animal Clinic of the Veterinary University in Vienna and three referral clinics were searched for cats that had undergone unilateral THR with the BioMedtrix Micro total hip system or FHO more than 6 months previously. Owners were invited to complete a survey and bring their cats for re-examination, inlcuding clinical and orthopaedic examinations, hip radiography and a gait analysis using a pressure-sensitive plate. Results Nine cats were included in each group. Cats after THR showed larger GRF values (peak vertical force [PFz] and vertical impulse [IFz] normalised to total force [%TF]) on the operated limb. The resulting symmetry indices (SIs) were lower in terms of vertical force in 7/9 (78%) cats and vertical impulse in 6/9 (67%) cats between the hindlimbs in cats after THR compared with FHO – SI (PFz) = 3.31% ± 2.19% (THR) vs 4.84% ± 2.99% (FHO) and SI (IFz) = 5.17% ± 3.66% (THR) vs 8.27% ± 3.12% (FHO). Cats after FHO showed significantly lower muscle circumference and range of motion (ROM) at the operated hindlimb compared with the contralateral side, whereas cats after THR showed no statistically significant differences between their hindlimbs. Owner surveys revealed significant differences in their subjective assessment of activity and change in gait between the two groups, with better values for cats after THR. Conclusions and relevance This was the first study that measured GRFs in cats after THR. PFz (%TF) and IFz (%TF) values were higher in the operated limb of the THR group than in those of the FHO group, resulting in lower symmetry indices (indicating better symmetry) and better loading of the corresponding hindlimb. This finding is clinically relevant and can help in making decisions regarding the treatment of hip joint pathologies in cats.
