El resultado estético de la reconstrucción mamaria depende de varios factores difíciles de evaluar, y lleva por tanto a una variabilidad muy significativa en los resultados obtenidos. La valoración habitual es subjetiva. De ahí la inquietud por disponer de una herramienta de valoración objetiva.Crear una metodología para medir resultados estéticos de la simetría frontal en la reconstrucción mamaria mediante el uso de un software.En el Instituto Nacional de Oncología de Cuba, en el período entre diciembre de 2011 y diciembre de 2017, se aplicó una nueva metodología para medir la simetría mamaria. El software fue desarrollado íntegramente por los autores. Toma como entrada una imagen frontal. En cada medición se evaluaron las medidas aportadas por el software. Se aplicó a un grupo control de 74 mujeres, de una población sana, a un grupo de 56 pacientes tributarias de reconstrucción mamaria inmediata y a un grupo de 45 pacientes con reconstrucción mamaria diferida. Se analizaron variables sociodemográficas y oncorreconstructivas. En el análisis estadístico se usó la mediana.La mayoría de las pacientes estaban en percentiles mayores del 75, por lo que los resultados estéticos fueron muy satisfactorios, con mejores resultados en los casos en que se practicó cirugía inmediata.Se desarrolló un nuevo método cuantitativo para medir simetría frontal en pacientes reconstruidas por cáncer de mama. Los mejores resultados fueron en las pacientes reconstruidas de forma inmediata. El método es de fácil aplicación y económico.The aesthetic result of breast reconstruction depends on several factors that are difficult to evaluate, leading to highly significant variability in the results. Evaluation is usually subjective and consequently there is strong interest in a tool that could provide objective valuation.To create a method to measure the aesthetic results of frontal symmetry in breast reconstruction, by means of software.A new method was applied to measure breast symmetry in the National Institute of Oncology of Cuba between December 2011 and December 2017. The software was developed entirely by the authors. First, a frontal image was taken. Each subsequent measurement provided by the software was evaluated. The method was applied in a control group of 74 women from a healthy population, a group of 56 patients suitable for immediate breast reconstruction, and a group of 45 patients with delayed breast reconstruction. Sociodemographic variables and the technique surgical were analysed. In the statistical analysis, the median was used.Most of the patients were in percentiles higher than the 75th percentile, indicating highly satisfactory aesthetic results. Better results were obtained in patients undergoing immediate reconstruction.A quantitative new method was developed to measure frontal symmetry in patients undergoing reconstruction due to breast cancer. The best results were obtained in patients undergoing immediate reconstruction. The method is easy to apply and inexpensive.

The effects of di-(2-ethylhexyl) phthalate (DEHP) at five different doses from 10 to 1000 mg kg−1 soil on biological properties were investigated over a period of 56 days. Meanwhile, the dissipation of DEHP was also monitored. The results indicated that the microbial biomass C (Cmic) fluctuated at around 70 mg kg−1 soil for the control, whereas the Cmic varied significantly for the soil samples contaminated by DEHP. The catalase activities in all five treatments were stimulated at most time, and the activities of phosphatase in the soils treated by DEHP with 500 mg kg−1 or 1000 mg kg−1 were significantly higher than the other treatments from the 20th day. Urease was more sensitive and inhibited significantly during the initial period of incubation. Additionally, the dose–response relationship of invertase was presented in the later phase of incubation. The activities of urease and invertase might indicate soil perturbations caused by the introduction of DEHP. The dissipation of DEHP was found to follow the pseudo first-order kinetics behavior.

The prevalence of overweight and obesity in children and adolescents is still too high and represents a pool of latent morbidity. Data from Spain reveal an alarming increase in the prevalence of obesity in children and youth. Treatment of obesity seems easy, that is, just advice children and their families to eat less and to exercise more. In practice, however, treatment of childhood obesity is time-consuming, frustrating, difficult, and expensive. According to evidence, what seems to be the most successful approach is intensive lifestyle modification that includes guidance on dietary aspects, food-related factors, physical activity, and behavioral strategies. Family support is of utmost importance. All of these considerations are essential for developing and implementing preventive measures specifically directed toward children and adolescents. La prevalencia del sobrepeso y la obesidad en niños y adolescentes aún es excesivamente alta y representa una reserva de morbosidad latente. Datos procedentes de España demuestran un aumento inquietante de la tasa de obesidad en niños y jóvenes. El tratamiento de la obesidad parece sencillo, es decir, sólo hay que aconsejar comer menos y hacer más ejercicio a los niños y sus familias. En la práctica, no obstante, el tratamiento de la obesidad infantil lleva mucho tiempo, y es frustrante, difícil y costoso. Según los indicios, lo que parece ser el enfoque de más éxito es modificar intensamente el estilo de vida, que incluye una orientación sobre aspectos alimentarios, factores relacionados con la alimentación, la actividad física y estrategias de la conducta. El apoyo de la familia es de suma importancia. Todas estas consideraciones son imprescindibles para desarrollar e implantar medidas preventivas que estén dirigidas especialmente a los niños y adolescentes.

Management of overweight and obesity in adolescents: an integral lifestyle approach

Foods contain biologically active substances with various physiological effects. Among them, some components such as dietary fibers, polyunsaturated fatty acids and antioxidants exert multiple activities, including intestinal regulation, anti-cancer, anti-lipidemia, anti-diabetes, immuno-regulatory and anti-oxidative activities. In addition, combinational uses of some components such as sesamin and α-tocopherol produce synergistically enhanced biological effects that are greater than the sum of individual effects. The expression of multiple effects enables production of multifunctional foods, which can prevent the occurrence of various diseases. In addition, synergic effects between biologically active substances allow us to decrease the dose of each component. This leads to improvement of safety and reduction of production costs of multifunctional foods.

La desnutrición en el paciente oncológico tiene un origen multifactorial, que depende del paciente, el tipo de tumor, el tratamiento recibido y la aparición del síndrome de caquexia-anorexia cancerosa, que supone el grado máximo de desnutrición. La caquexia aparece como consecuencia de la interacción entre el tumor y el paciente; esta da lugar a la activación de distintos mediadores que producen cambios importantes en el metabolismo del paciente. En el niño, a diferencia del adulto, una mayor pérdida de peso no condiciona peor pronóstico, pero sí empeora su calidad de vida. Malnutrition in cancer patients has a multifactorial origin. Will depend on the patient, tumour type, treatment received and the anorexia-cachexia syndrome onset, which is the maximum degree of malnutrition. Cachexia occurs as a consequence of the interaction between the tumour and the patient, leading to activation of different mediators that will produce important changes in the metabolism of the patient. Children with severe weight loss are not associated to lower survival, but their quality of life will be worse.

Mecanismos de la desnutrición en oncología pediátrica

La valoración nutricional es muy importante en los pacientes renales (estén o no en programa de diálisis) porque la desnutrición determina no sólo su morbimortalidad, sino que además tiene una gran repercusión en la calidad de vida de estas personas. Aportamos nuestra experiencia en esta valoración, con la novedad de incluir de forma sistemática la bioimpedancia como un instrumento más (útil y barato) para poder aproximarnos al conocimiento de estos pacientes tan inestables hemodinámicamente. Nutritional assessment is very important in kidney patients (whether they are on a dialysis programme or not), because undernourishment not only determines their morbimortality, but it also has a great effect on these people's quality of life. We lend our experience to this assessment with the novelty of systematically including Bioimpedance as another (useful and cheap) instrument for being able to get close to the knowledge of these patients who are so haemodynamically unstable.

El cáncer infantil está considerado como la segunda causa de mortalidad en la edad infantil. El progreso en la investigación, los métodos diagnósticos y la eficacia del tratamiento permiten mejorar el índice de supervivencia. La propia neoplasia, como su tratamiento, puede ocasionar la aparición de desnutrición, lo que repercute en la supervivencia de los pacientes, la tolerancia y la respuesta a los tratamientos oncológicos y aumenta la mortalidad. Es necesario realizar una valoración del estado nutricional, conocer los síntomas asociados y el grado de riesgo del tratamiento antineoplásico para poder calcular las necesidades nutricionales. Asimismo, se deben concretar unos objetivos específicos y realizar la intervención nutricional adecuada, con la finalidad de aumentar la supervivencia, disminuir las complicaciones y mejorar la calidad de vida. Childhood cancer is considered to be the second cause of mortality in children. Progress in the research, diagnostic methods and effectiveness of treatment can improve the survival rate. The neoplasm itself, as well as the treatment can give rise to malnutrition. This can affect patient survival, tolerance and the response to cancer treatments and increased mortality. An assessment of the nutritional state is required, with knowledge of associated symptoms and degree of risk of antineoplastic treatment, in order to calculate the nutritional needs. Likewise, there must be specific goals and appropriate nutritional intervention, in order to increase survival, reduce complications and improve the quality of life.
