Clinical Child Psychology and Psychiatry

Recently, efforts have been made to better understand constructs that are associated with difficulties in emotion regulation in hopes of identifying underlying mechanisms that may be valuable targets for intervention. Against this background, the present study had two aims. Firstly, we wanted to explore the relation between emotion regulation, experiential avoidance and alexithymia by determining whether adolescents with elevated scores on a measure of alexithymia would report deficits in emotion regulation and experiential avoidance. Secondly, we sought to evaluate the role of experiential avoidance as a mediator in the relation between alexithymia and emotion regulation. The sample ( N = 64) consisted of adolescents recruited from an inpatient facility of which approximately 30% were classified as having alexithymia. The results of this study indicate that adolescents with alexithymia report deficits in emotion regulation and elevated experiential avoidance. Experiential avoidance mediated the relation between alexithymia and emotion regulation, indicating that while the inability to effectively use language to identify and describe emotional states is strongly correlated with difficulties in regulating one’s emotions, this relation is mediated by the unwillingness to tolerate aversive private experiences. Limitations and strengths of the present study are also noted.

The relation between experiential avoidance, alexithymia and emotion regulation in inpatient adolescents

Difficulties in social skills are highly prevalent in children and adolescents with attention deficit hyperactivity disorder (ADHD), however, the reasons for these social difficulties are poorly understood. This study aimed to understand social motivation in children (aged 5-8) and adolescents (aged 13-17) with and without ADHD, and the relationship between social anxiety and social motivation in youth with ADHD.204 parents of children and adolescents with and without ADHD completed online questionnaires on social motivation, social anxiety, and ADHD symptoms.Adolescents with ADHD had significantly lower social motivation than typically developing adolescents, and children with ADHD. Higher social anxiety significantly predicted lowered social motivation in participants with ADHD.These findings indicate a unique pattern of social motivation in ADHD, specifically a reduction during adolescence, reflecting social intervention inefficacy. Additionally, these findings establish a link between social motivation and social anxiety, suggesting that they may act as barriers to social intervention.

This article examines the combined roles of attachment mentalisation and diabetes distress in the psychological development of young people with Type 1 diabetes (T1D). We use these ideas to unify the evidence for psychological variables affecting young people and their families and suggest how diabetes distress and mentalisation might be part of the pathways for development of psychiatric diagnoses.Attachment theory's central hypothesis is that a secure relationship with a care-giver in the early life of a child is essential to normal emotional and relational development, whilst diabetes distress is a well recognised phenomena of burden experienced by both child and parent in relation to the condition.We extend the ideas of attachment, into the psychological adaptation processes for young people at the time of diagnosis of T1D with emphasis on the function of the parent/caregiver in mentalising the experience of the child. We also connect our current understanding of diabetes distress to the associated increased risk for disorders of eating and personality in T1D.Using principles learnt in other areas of psychotherapeutic practice we end by suggesting interventions that could impact mental health and diabetes outcomes using the mentalisation model.

Understanding developmental psychopathology in Type 1 diabetes through attachment, mentalisation and diabetes distress

Diabetes may impact physical and psychosocial well-being; the diabetes incidence has seen a drastic increase globally. There is also a rise in poor mental health and well-being in children with and without chronic illness; problems are being seen at a younger age. The objective of this review was to understand the determinants of these problems in a family context. We conducted a systematic review to investigate what lifestyle and psychological factors influence children with Type 1 diabetes and their parents. A focused literature search was performed using a combination of keywords that covered the relevant terminology for diabetes, target population, and associated emotional distress, using electronic bibliographic databases containing publications until May 2022. Methodological quality was assessed using the Quality Assessment Tools for Quantitative Studies. Twenty articles met the inclusion criteria. Quality scores were weak because of a lack of comparison groups, information about the type of therapy, or adequate sample sizes. Many of the studies included a wide age range in their sample. The majority of the studies reported that parents and their children showed depression symptoms, fear of hypoglycaemia, and higher parenting stress. We conclude that sufficiently powered studies employing appropriate control groups and measures are needed to elucidate the psychological variables associated with Type1 diabetes in children and the effects on parents, especially considering primary-age children who are increasingly reported to suffer from poor mental health, and its implications. This should help to introduce better targeted interventions and improve behavioural outcomes.

Evaluation of sluggish cognitive tempo in children and adolescents with obesity and morbid obesity

Background Ethnic disparities in cancer prevalence and health outcomes have been widely documented in adults. However, less is known about the impact of ethnic differences in young cancer patients who present with complex needs along their developmental trajectories. The present review aimed to examine psychological outcomes amongst ethnically minoritised adolescents and young adults (AYAs) with cancer. Method A systematic search was conducted on four databases using terms related to AYAs, cancer, ethnic minority and psychological outcomes. Quantitative studies of any design were included and screened against the eligibility criteria. Studies were rated for methodological quality and synthesised narratively. Results Twelve studies conducted in the United States were identified with mostly moderate to low quality and the evidence was mixed. Six studies found ethnic disparities in psychological outcomes: the majority demonstrated that ethnically minoritised AYAs experienced significantly more distress compared to White peers with cancer. Hispanic youths were highlighted as a vulnerable group that fared worse in their mental health compared to other minoritised youths. Longitudinal data showed that minoritised AYAs experienced more marked improvement in their psychological health over time compared to Caucasians. Conclusion There is emerging evidence regarding inter-ethnic differences in psychological outcomes amongst AYAs with cancer. However, the findings are inconsistent, reflecting methodological weaknesses and the complexities of intersectionality impacting on mental health. Further cross-cultural research is necessary to substantiate these findings and elucidate mechanisms behind these inequalities to promote more equitable healthcare.

Psychological outcomes in ethnically minoritised adolescents and young adults with cancer: A systematic review

Cognitive behavioral therapy (CBT) is an evidenced-based treatment for sleep concerns but may be insufficient on its own to resolve adolescent sleep problems – combining this treatment with motivational interviewing may result in more robust improvements to sleep in adolescents and young adults (AYA). This study aimed to conduct the first meta-analysis of integrated CBT and motivational interviewing (MI) for AYA sleep concerns. Following PRISMA guidelines, PsycINFO, PubMed, and Google Scholar were searched through January 2024. Cohen’s d was computed for effect sizes. Eligible articles included AYA utilized an integration of CBT and MI, reported quantitative outcomes related to sleep, and were published in peer-reviewed journals and available in English. Four studies were eligible for the meta-analysis ( N = 435). Results showed a small-medium effect size for reduced daytime sleepiness ( d = 0.39, p = .001) and increased total sleep time ( d = 0.25, p = .008) following integrated CBT and MI. Risk of bias was assessed using CMA. The findings suggest integrated CBT and MI is beneficial for AYA daytime sleepiness and sleep duration, more effective than CBT alone and other control groups. These findings contribute to the understanding of effective interventions for AYA sleep concerns, offer practical insights for practitioners, and highlight the need for further investigation into the integration of CBT and MI.

Suicide among adolescents is a serious global health concern. Previous research has highlighted that interpersonal factors are vital determinants of suicidality. However, little is known about the cumulative effects of interpersonal risks and the protective factors that buffer individuals from developing suicidal tendencies. The present study examined whether linear or nonlinear models best account for the relationship between cumulative interpersonal risk and suicidal ideation and attempts among early adolescents and the protective value of self-esteem in these associations.
