Recently, biological therapies for early intervention of degenerative disc disease have been introduced and developed; however, a functional animal model that mimics slowly progressive disc degeneration of humans does not exist. The objective of this study was to establish a slowly progressive and reproducible intervertebral disc (IVD) degeneration model. The subchondral bone adjacent to the lumbar IVDs (L3/4 and L5/6) of ten rhesus monkeys was randomly injected with 4 ml bleomycin solution (1.5 mg/ml), or 4 ml phosphate buffer saline (PBS) per segment as control, respectively. The degenerative process was investigated by using radiography and T1ρ MR imaging at 1, 3, 6, 9, 12 and 15 months postoperatively. Histological scoring, Sulfated Glycosaminoglycans (GAGs) analysis and real-time PCR were performed at 15 months. The correlation between histological score, GAGs and T1ρ values were also analyzed. The results showed that the mean T1ρ values of nucleus pulposus (NP) and annulus fibrosus (AF) in the bleomycin group significantly decreased after 3 and 6 months respectively, followed by slowly decrease until at 15 months. At 15 months, the histological scores was significantly higher, and the GAGs of NP was significantly lower in the bleomycin group, compared with the control group (P < 0.05). The results of real-time PCR revealed a significant increase in matrix metalloprotease (MMP)-3, A disintegrin and metalloproteinase with thrombospondin motifs (ADAMTS)-5, tumor necrosis factor α, interleukin-1β, interleukin-6 expressions, transforming growth factor (TGF-β1) and marked reduction in aggrecan, type II collagen, von willebrand factor (vWF) expressions at the mRNA levels in the bleomycin group. Spearman correlation analysis showed a strong positive correlation between GAGs and T1ρ values of NP (r =0.740, P < 0.01), and a significant inverse correlation between histological score and T1ρ values of NP and AF (r = -0.761, r = -0.729, respectively, P < 0.01). Injection of bleomycin into the subchondral bone adjacent to the lumbar IVDs of rhesus monkeys can results in mild, slowly progressive disc degeneration, which mimics the onset of human disc degeneration. T1ρ MR imaging is an effective and noninvasive technique for assessment of early stage disc degeneration.

In vivo experimental intervertebral disc degeneration induced by bleomycin in the rhesus monkey

In current active prevention (including physical and drug prevention), the incidence of perioperative deep vein thrombosis (DVT) of the lower extremities remains high in patients with bone trauma. Risk factors need to be further optimized, and high-risk patients must be identified early. Preoperative comorbidities, especially preoperative anemia, and DVT in patients with perioperative bone trauma are not clear. The purpose of this study was to explore the causal relationship between preoperative anemia and DVT in patients with perioperative bone trauma, and further reduce the incidence of DVT in patients with bone trauma.
To analyze the relationship between preoperative anemia and perioperative DVT in patients with femoral and pelvic fractures and provide a reference for the optimization of risk factors for DVT.
The clinical data of 1049 patients with femoral and pelvic fractures who received surgical treatment from May 2018 to June 2021 were retrospectively analyzed. Propensity score matching (PSM) was performed for the covariates of DVT. Modified Poisson regression was used to analyze the relationship between preoperative anemia and DVT.
After matching 1:1 propensity scores in 1049 patients included in this study, there were 258 patients in the anemic and non-anemic groups. Preoperative anemia was statistically significant for the formation of DVT in patients with perioperative bone trauma (P= 0.000, RR = 1.567 [95% CI 1.217–2.017]). This conclusion remained true after PSM (P= 0.009, RR = 1.500 [95% CI 1.105–2.036]). Preoperative anemia has some predictive value for perioperative DVT, with DVT-associated preoperative anemia thresholds of 125 g/L and area under the receiver operating characteristic curve of 0.5877 (95% CI 0.5345 to 0.6408). On this basis, sensitivity and specificity were 89.2 and 30.3%, respectively, with a Youden index of 0.195. In addition, we conducted an E-value determination of the propensity score; the E-value analysis showed robustness to unmeasured confounding.
Preoperative anemia is highly correlated with perioperative DVT in patients with bone trauma, which is the cause of perioperative DVT in these patients.

The clinical efficacy and safety of platelet-rich plasma on frozen shoulder: a systematic review and meta-analysis of randomized controlled trials

Spinal deformities affect the overall alignment of the spine and thus the vectors of loading on the lumbar region and intervertebral discs. Due to wedging of the disc or vertebrae of unbalanced spinal segments, alignment change may affect the range of motion (ROM) of individual spinal segments or the global spine. This is particularly important in adolescent idiopathic scoliosis (AIS) patients who may suffer from early degeneration, back stiffness and pain. Hence, this study aimed to determine the correlation between spine range of motion (ROM) and adolescent idiopathic scoliosis (AIS) curve magnitude.Consecutive recruitment of all AIS patients with Lenke 5 (thoracolumbar/lumbar) curves within one month was performed with ROM assessments in the coronal, sagittal and axial planes using the change in C7-S1 distance on standing upright, active flexion and extension positions, change in finger-floor distance on forward bending position and lateral bending, lateral bending angles, modified Schober's test, and trunk rotation in seating position. Patients were further stratified into two groups based on their lumbar spine curve magnitude: Group A with curves of 10 to 39 degrees and Group B with 40 degrees or greater. Univariate and multivariate analyses were conducted, with lumbar curve magnitude severity being the dependent variable.In total, 58 patients (n = 12 males, n = 46 females; mean age: 15.7 years) were recruited. The mean curve magnitudes were 25 ± 6.5 degrees in Group A and 48 ± 10.6 degrees in Group B. Mean axial rotation (Group A: 90 ± 21.7 degree; Group B: 76 ± 19.6 degrees; p = 0.038) and lateral bending ROM (Group A: 67 ± 13.4 degrees; Group B: 58 ± 14.3 degrees; p = 0.045) decreased in more severe curves. These two parameters continued to remain significant irrespective of the curve severity cut-off values.This is the first study to determine associations between spinal ROM parameters with the lumbar curve magnitude in AIS patients. We found that the coronal curve severity is associated with reduced axial and coronal ROM. This is a platform for future studies assessing lumbar spine biomechanics in AIS and to determine the effects of altered spine motion in this context and its implication in patient management and outcomes.

The association of lumbar curve magnitude and spinal range of motion in adolescent idiopathic scoliosis: a cross-sectional study

This study aims to determine the learning curve for lumbar discectomy and nerve root decompression using the unilateral biportal endoscopic (UBE) technique on fresh frozen cadavers through linear regression and cumulative sum (CUSUM) analyses. The analysis was conducted on three spine surgeons without UBE experience. They performed lumbar discectomy using the unilateral endoscopic technique on fresh frozen cadavers. Each physician performed operations on 40 lumbar spine segments, recording the time it took each physician to complete a segment of lumbar discectomy by UBE. The learning curve was assessed by linear regression and CUSUM analysis using SPSS 27.0 software and GraphPad Prism 8.0 software. According to the CUSUM analysis of the learning curve, the number of segments required to complete the learning phase for the three surgeons was 20, 16, and 20, respectively. With the increased number of operated segments, the operation time for each lumbar vertebral segment gradually decreased and stabilized. In our study, spine surgeons without experience in UBE require 20 segments of surgery to overcome the UBE learning curve. Additionally, fresh frozen cadavers can effectively simulate real surgeries. Whenever possible, we should reasonably use fresh frozen cadavers for early UBE training of doctors, thereby better protecting the health and safety of patients.

Analysis of the learning curve for unilateral biportal endoscopic technique using CUSUM method on fresh frozen cadavers

The COVID-19 pandemic had a significant impact on healthcare systems and the general population. However, it remains unclear to what extent these major societal changes have had an impact on the management of hip fractures in older people in Japan. Therefore, we investigated the effect of the COVID-19 pandemic on the number of patients with hip fractures, their characteristics, and their perioperative management as a retrospective multicentre study. We included 1894 patients aged ≥ 65 years who underwent surgery for hip fracture at three hospitals between January 2019–December 2021. Patients were classified according to the time of injury; patients treated between January–December 2019, January–December 2020, and January–December 2021 were divided into the pre-COVID-19 group, early COVID-19 group, and late COVID-19 group, respectively. We compared age, sex, body mass index, preadmission residence, surgical procedure, length of hospital stay, waiting time for surgery, in-hospital complications, and in-hospital death. Our findings suggested that the early COVID-19 and late COVID-19 groups showed a 6.8% and 7.5% reduction in the number of HF patients, respectively, compared to the pre-COVID-19 group. Waiting days for surgery, length of hospital stay, and in-hospital mortality or complication rates did not significantly change before and after the pandemic. However, infection was increased in the early COVID-19 group regarding the subgroup of complications. The COVID-19 pandemic altered the characteristics of hip fractures in older individuals.

Changes of hip fracture in older patients before and after the COVID-19 pandemic: a retrospective multicentre study in Japan

Previous research has established connections between the acromion index (AI), critical shoulder angle (CSA), acromiohumeral distance (AHD), and lateral acromion angle (LAA) with the incidence of rotator cuff tears (RCTs). Despite numerous international studies exploring the variability of shoulder joint morphology and parameters, research incorporating these parameters has yet to be conducted in Somalia. This study aims to analyze and describe the radiological parameters of acromion morphology in the Somali populations. Furthermore, to investigate the relationship of these parameters with rotator cuff tears within the Somali population. The data and physical examination of 188 patients who had a CT scan of the shoulder from 2018 to 2023 were retrospective analyses, including 107 patients (49 females, 58 males) with RCT and 81 patients (20 females, 61 males) with non-RCT. Using Three-dimensional computed tomography (3D-CT), parameters such as the AI, CSA, AHD, and LAA were compared between the RCT and non-RCT groups. Two independent assessors measured Each parameter from anterior views of the scapula. The inter- and intra-observer reliability was evaluated using the intraclass correlation coefficient (ICC). Additionally, the relationship between these parameters and the presence of rotator cuff tears was explored, and the predictive value of each parameter was assessed through receiver operating characteristic (ROC) analysis. The results showed excellent intra- and inter-observer reliability, with all ICC values above 0.75. Among these, the CSA exhibited the greatest measurement stability, with an intra-observer ICC of 0.929 and an inter-observer ICC of 0.911. Significant positive correlations were identified between the AI, CSA, and the presence of RCTs, with CSA exhibiting the strongest correlation (r = 0.629; P < 0.001). Additionally, significant negative correlations were found between the presence of RCTs and both AHD (–0.247) and LAA (–0.338; P < 0.001 for both). The ROC analysis revealed that the CSA is the most effective parameter for identifying the presence of RCT (area under the ROC curve. 0.857). The outcome of this study shows that AI, CSA, AHD, and LAA are independent factors associated with rotator cuff tears in the East African, particularly the Somali population. Specifically, larger AI, CSA, and smaller AHD and LAA are correlated with an increased likelihood of RCTs in this population. In patients with shoulder disease suspected of RCTs, using a 3D-CT scan to measure the CSA may be helpful, as it is the best predictor measurement. Not Applicable.

Association between acromion morphological parameters and rotator cuff tears in Somali population—a three-dimensional computed tomographic study

The relationship between thromboelastogram (TEG) hypercoagulation status and perioperative deep vein thrombosis (DVT) in patients with femoral and pelvic fractures is not well understood. We aimed to investigate the relationship between hypercoagulation status identified by thromboelastography and postoperative DVT formation in patients with femoral and pelvic fractures, as well as to evaluate the role of thromboelastography in assessing hypercoagulation status and predicting postoperative DVT formation. Data from 2,065 patients with femoral and pelvic fractures who underwent surgical treatment at a hospital in China between May 2018 and December 2023 were retrospectively analysed. Hypercoagulable TEG was defined as reaction time (R) < 5 min, coagulation time (K) < 1 min, alpha angle (α) > 72 degrees, maximum amplitude (MA) > 70 mm, and/or coagulation index (CI) > 3. The correlation between preoperative hypercoagulability identified by TEG and postoperative DVT formation was assessed using multivariate logistic regression. Propensity score matching (PSM) was performed to control for confounding factors. Compared to the non-DVT group, the DVT group had decreased R and K values, while the α, MA, and CI values significantly increased (P < 0.05). Multivariate logistic regression analysis demonstrated that hypercoagulable TEG findings were predictive of postoperative DVT formation. PSM, using a 0.1 calliper value, matched 296 patients from the hypercoagulation and non-hypercoagulation groups in a 1:1 ratio. Before PSM, hypercoagulable TEG was associated with DVT in femoral and pelvic fractures (P < 0.001, odds ratio [OR]:1.860, 95% confidence interval: 1.389–2.492). After PSM, these two variables remained correlated (P = 0.001, OR = 1.878, 95% confidence interval:1.301 − 2.709). The hypercoagulable state identified by TEG can predict thromboembolic events in patients with femoral and pelvic fractures. Association of thromboelastogram hypercoagulability with postoperative deep venous thrombosis of the lower extremity in patients with femur and pelvic fractures: a cohort study

Musculoskeletal health among the elderly emerges as a pivotal public health concern against the backdrop of a globally aging population. Creatine, popular within fitness circles, has emerged as a subject of scientific inquiry for its potential benefits on elderly' musculoskeletal well-being. Despite extensive documentation of its effects on athletic performance, investigations into creatine's long-term contributions to the musculoskeletal health of the elderly are comparatively limited. Utilizing publicly accessible genetic datasets, this study aimed to explore the potential causal link between creatine levels and a variety of musculoskeletal health markers in the elderly population, such as osteoarthritis (OA), rheumatoid arthritis (RA), osteoporosis (OP), bone-specific alkaline phosphatase, bone mineral density(BMD), serum calcium levels, and reduced hand grip strength in individuals aged 60 and above. Our methodological approach was grounded in Mendelian Randomization (MR) analysis, with a primary emphasis on the Inverse Variance Weighted (IVW) technique, to explore potential causal links under specific assumptions and limitations inherent to MR analysis. A significant inverse relationship was observed between creatine levels and reduced hand grip strength in individuals aged 60 and above (OR = 0.64, b=-0.44, p = 0.01), suggesting higher creatine levels might be beneficial in maintaining hand grip strength. Conversely, analysis of other bone health parameters, including bone-specific alkaline phosphatase, bone mineral density, serum calcium levels, and conditions such as osteoarthritis, rheumatoid arthritis, and osteoporosis, yielded p-values exceeding 0.05, denoting no statistically significant associations. This study provides preliminary evidence indicating a link between creatine levels and decreased hand grip strength among the elderly, notably in individuals aged 60 and older. This finding is significant for understanding the potential impact of creatine supplementation on elderly Musculoskeletal health and underscores the need for evidence-based decision-making in nutritional supplementation. To robustly validate these observations, it is essential to conduct future randomized controlled trials with large sample sizes.
