Abstract Aims This study aimed to explore the change trend and group heterogeneity of psychosocial adjustment level and to determine its influencing factors among young and middle-aged patients with first-episode acute myocardial infarction (AMI). Methods and results The Psychosocial Adjustment Scale of Illness was used to assess the psychosocial adjustment level of the patients at 1, 3, and 6 months after discharge, respectively. Data were analysed using Pearson correlation analysis, generalized estimating equations, and growth mixed models. A total of 233 patients were included, and their psychosocial adjustment scores at the three time points were 57.18 ± 15.50, 36.17 ± 15.02, and 24.22 ± 12.98, respectively. The trajectories of changes in patients’ psychosocial adjustment levels were divided into three latent categories: moderate adjustment improvement group (72.5%), low adjustment improvement group (16.3%), and persistent maladjustment group (11.2%). Among them, predictors of the persistent maladjustment group included no spouse, low monthly family income per capita, normal body mass index, never smoking, never exercising, combined with hyperlipidaemia, low social support, submission coping, and high perceived stress. Conclusion The psychosocial adjustment level of young and middle-aged patients with first-episode AMI showed an upward trend within 6 months after discharge, and there was group heterogeneity in the change trajectory of psychosocial adjustment level. It is suggested that a multicentre, large-sample longitudinal study should be carried out in the future, and the time of follow-up investigation should be extended to further clarify the change trajectory and influencing factors of psychosocial adjustment of patients with different subtypes, to provide the theoretical basis for formulating targeted intervention programmes.

Psychosocial adjustment changes and related factors in young and middle-aged patients with first-episode acute myocardial infarction: a longitudinal study

Approaches in the secondary prevention of heart disease have witnessed a steady evolution in the past decades from educational leaflets to standardized face-to-face therapies to harnessing technology to deliver individualized telehealth-based rehabilitative initiatives. 1 Yet, heart disease remains the leading cause of mortality globally. 2Given the chronicity and morbidity that accompanies heart diseases, and in light of an aging population, this is an especially important concern for healthcare systems worldwide. 2There is a growing interest in exploring alternative approaches to supplement existing preventive strategies in a way that is effective and sustainable to meet the holistic needs of patients with heart disease. 3,4This is mirrored in the inclusion of Traditional Chinese medicine (TCM) in the 11th revision of the World Health Organization's International Statistical Classification of Diseases and Related Health Problems (ICD). 5raditional Chinese exercises (TCEs) has been practiced in China for over 2000 years and focuses on promoting physical health and mental well-being through the restoration and balance of qi. 6Qi is defined as the 'the circulating life force whose existence and properties are the basis of much Chinese philosophy and medicine'. 7Common TCEs like Tai Chi, Qigong and Baduanjin share similar exercises philosophies of low-to moderate-intensity movements performed in combination with muscle stretching and relaxation, coordinated breathing, and a meditative state of mind. 6However, owing to the nature of its practice and roots in TCM, reception to TCEs has been previously met with scepticism and contention. 8,9While mainstream Western medicine seeks to draw conclusions from large samples based on statistical methods to explain and generalize well-tested disease states and treatment choices, this perspective is too simplistic in TCM where factors determining health are viewed as specific to an individual and priority is given to holistically understanding and managing the individual as a whole. 8,9In this editorial, we discuss the recent efforts made to legitimize and validate the efficacy of TCEs in heart disease management and the opportunities that exist to integrate these two practice perspectives for improved outcomes for patients and healthcare systems.

East meets West: making the case for traditional Chinese exercises in the management of heart disease

Abstract Aims In-hospital telemetry monitoring has been an integrated part of arrhythmia monitoring for decades. A substantial proportion of patients require arrhythmia monitoring during stays in non-intensive care units. However, studies exploring patients’ experiences of telemetry monitoring are scarce. Therefore, the aim was to explore and describe patients’ experiences of in-hospital telemetry monitoring in a non-intensive care setting. Methods and results Twenty face-to-face, semi-structured interviews were conducted. Interviews were conducted before discharge at two university hospitals in Norway. The patients were purposively sampled, resulting in a well-balanced population comprising 11 men and nine women, mean age 62 years (range 25–83). Average monitoring time was 9 days (range 3–14). Data were audiotaped, transcribed verbatim, and coded using NVivo software. Qualitative content analysis using an inductive approach was performed. Patients expressed a need for individualized information during telemetry monitoring. Their feelings of safety were related to responses from nurses from the central monitoring station when alarms from the telemetry were triggered. Despite perceived physical restrictions and psychological limitations associated with telemetry monitoring, they found monitoring to be beneficial because it facilitated the diagnosis of arrhythmia. Moreover, they expressed a need for improvements in wearable monitoring equipment. Patients expressed ambivalent feelings about discontinuing the telemetry and their readiness for discharge. Conclusion Patients need individualized information about the results of their telemetry monitoring in order to better understand the arrhythmia management and to increase their experience of safety after discharge. The limitations patients experienced should be taken into consideration in further upgrades of telemetry monitoring equipment.

The patient experience of in-hospital telemetry monitoring: a qualitative analysis

Having nurses to perform point-of-care ultrasound (POCUS) when physically examining patients to assess for early signs of decompensation and fluid overload has been proven to be feasible, provide extra information, and improve quality of care. To date, few studies have analyzed whether it is feasible for intensive care nurses to perform POCUS on critically ill patients. This study aimed to evaluate whether the findings of briefly trained intensive care nurses' ultrasound examinations agreed with conventional examinations performed by physicians.This comparative cross-sectional inter-rater agreement study comprised 50 patients admitted to a cardiac intensive care unit (CICU), with signs and symptoms of acute dyspnoea and suspected cardiac disease. After a brief training session, the study nurses performed standardized examinations of each patient's inferior vena cava (IVC) and the pleural and pericardial cavities using a handheld ultrasound device. A blinded physician repeated the same examinations using conventional ultrasound. Analysis using Gwet's agreement coefficient with quadratic weights showed moderate agreement for the IVC respiration variation [0.60; 95% confidence interval (CI): 0.38-0.82], and substantial agreement for the IVC diameter (0.70; 95% CI: 0.50-0.90) and right-sided pleural effusion (0.70; 95% CI: 0.52-0.88). For left-sided pleural effusion (0.85; 95% CI: 0.75-0.95) and pericardial effusion (0.95; 95% CI: 0.90-1.01), the agreement was almost perfect.Briefly trained intensive care nurses in a CICU can perform POCUS examinations of the IVC and pleural and pericardial cavities with moderate to almost perfect agreement with identical examinations performed by physicians.

Point-of-care examinations using handheld ultrasound devices performed by intensive care nurses in a cardiac intensive care unit

Informal caregivers play a pivotal role in the care of people living with advanced heart failure, however, carers' needs have not been clearly identified.The aim of this study is to explore the evidence on palliative care needs expressed by carers of people with heart failure.Five electronic databases (CINAHL PLUS, EMBASE, Medline, PsychInfo and SCOPUS) were systematically searched and articles published January 2003-June 2014 with a qualitative methodology focusing on the palliative care needs of carers of people living with heart failure were included.Data was systematically extracted from 15 articles using an inductive methodology for the thematic analysis. Ten broad categories emerged from which three key areas of support needs were identified; psychosocial support to maintain a sense of normalcy; support with daily living; support navigating the healthcare system. The articles were predominantly published in the UK and USA with a total sample size across all articles of 270, the majority of which were older female spouses. Results included a combination of carers, patients and professionals thoughts, however data was extracted for carers only.Carer's needs initiate when the patient is diagnosed and continue throughout the disease into bereavement. These needs are continuously prioritised and reprioritised depending on the patients' medical stability. A holistic approach is needed to support these carers, incorporating heart failure and palliative care specialties. Further research is warranted to explore different methods of delivering support and information and to evaluate whether these reduce carer burden.
