The aim of the present research is to assess the beneficial and adverse effects of silicone bougie use in comparison to alternative methods of post-operative management of esophageal atresia. The study was carried on 28 patients treated at our healthcare institution for esophageal atresia during the period from May, 2017 to September, 2020. As an alternative to the use of esophageal bougie these patients were managed postoperatively, by utilizing the contrast imaging of the esophagus on the 7th and 14th days after surgery, and at the age of 1, 3, 6 months and one year. Unless the esophageal stricture has developed, the procedures are repeated annually until the age of 5. The aim of utilizing the contrast imaging technique is to prevent the stricture of the esophagus and recurrence of trachea esophageal fistula.

The search for an understanding of the genesis of tooth decay spans millennia. Streptococcus mutans have been proposed as the main etiological agents of dental cariessince the species discovery in 1924. However Omics era has provided a unique opportunity to unravel contemporary paradigms in the etiology of dental caries. Novel approaches using OMICS techniques has uncovered an extraordinarily diverse ecosystem of carious lesions where S. mutans accounts only a tiny fraction of the bacterial community. This supports the concept that caries has polymicrobial nature that do not follow classical Koch's postulates of infectious diseases and that the microbial causative agents are better described as pathobionts. Understanding the etiology of dental caries is not a mere academic exercise; it provides the basis for preventive, diagnostic, and treatment strategies and gives the dentist a theoretical framework to become a better professional.

[CARIES GENESIS IN THE ERA OF "OMICS"].

Urothelial carcinoma represents the most common type of bladder cancer (>90%) and is the most frequent malignancy of the urinary tract. Most of the urothelial carcinomas are non-invasive at the time of diagnosis, however they are characterised with the risk of recurrence after surgical treatment. The aim of our study was to investigate the characteristics of tumor heterogeneity and markers of its progression in urothelial papillary carcinomas. Study included following groups: normal urothelial epithelium, urothelial papilloma, urothelial neoplasms with low malignant potential (PUNLM), non-invasive low grade papillary urothelial carcinomas (LGPUC) and non-invasive high grade papillary urothelial carcinomas (HGPUC). In addition, study included relapsed cases of non-invasive LGPUC and HGPUC. Nuclear features and mitotic counts was assessed using digital pathology software QuPath in standard H&E stained specimens. In addition, the presence of mitosis was detected as PHH3 labelled cells by immunohistochemistry. Proliferation was measured as Ki67 labelling index by immunohistochemistry. Tumor heterogeneity was investigated by the differential expression pattern of CK5, CK7 and CK20 by immunohistochemistry. Study results showed, that Nuclear features, as well as the number of mitosis, proliferation index and intratumoral heterogeneity significantly correlate with the presence of higher grade non-invasive urothelial lesions. In addition, it is possible to distinguish two major groups of non-invasive LGPUC and HGPUC, based on nuclear and phenotypic heterogeneity and mitotic and proliferative activity, I group which is characterised with higher intratumoral heterogeneity, higher mitotic count and higher proliferative activity, represents the high risk group of non-invasive LGPUC and HGPUC recurrence.

MOLECULAR CHARACTERISTICS OF THE HETEROGENEITY OF NON-INVASIVE PAPILLARY UROTHELIAL CARCINOMAS AND THE MARKERS OF THEIR RECURRENCE.

Tuberculosis is one of the main problems of medicine in Kazakhstan. Kazakhstan is on the list of 30 countries with high rates of multidrug resistant tuberculosis in the world. Aim of this study is to conduct genotyping by MIRU-VNTR method to get preliminary data on M. tuberculosis genotypes distributed among the clinical isolates in Kazakhstan. 271 M. tuberculosis clinical isolates were gathered from new cases of tuberculosis from different regions of Kazakhstan in this study. Genotyping was done using 15 MIRU-VNTR (12 MIRU+3 ETR) loci. Obtained digital profiles of the clinical isolates were analyzed using the database on miru-vntrplus.org. Phylogenetic tree was built by UPGMA method. 97 genotypes were identified, 70 (25.8%) of them were unique and were determined in one isolate in the sample collection. The rest 201 (74.2%) isolates were grouped into 27 clusters, that contained from 2 to 102 isolates. According to genotyping results M. tuberculosis Beijing family strains were found in 65.3% cases. 121 out of 177 Beijing isolates (68.4%) were drug-resistant. Prevalence of MDR-TB was detected among drug-resistant Beijing (58.7% - 71/121) and LAM family (50% - 10/20) isolates.

USE OF 15 MIRU-VNTR GENOTYPING FOR DISCRIMINATING M. TUBERCULOSIS CLINICAL ISOLATES FROM KAZAKHSTAN.

The aim of the study was to investigate effectiveness of early rehabilitation after two types of osteosynthesis on the functional results of surgical treatment of patients with extra-articular fractures on the distal part of the humerus. In the period of 4 to 6 months after the operation (an average of 3.5 months), 47 patients in two clinical groups were examined. 26 patients (the comparison group) underwent surgical treatment by the method of bone fixation with plates and screws. 21 patients (the study group) have been treated with transosseous osteosynthesis. Postoperative management in the comparison group was of a traditional nature. In the study group, methodological principles of rehabilitation have been developed, such as early onset, continuity and complex nature. The analysis of observations of patients with injuries of this localization showed that it is possible to improve the results of treatment by functionally oriented management of patients in the postoperative period. Transosseous osteosynthesis in the study group allowed achieving the predominance of positive treatment results (86% vs. 77%) and becoming an effective means of preventing and eliminating elbow joint contractures in the early postoperative period (125±12.1° vs. 97.7±9.7°).

EARLY REHABILITATION OF PATIENTS AFTER SURGICAL INTERVENTIONS ON THE DISTAL HUMERUS

Recent studies have revealed a strong association between Coronary Artery Disease (CAD) severity and arterial stiffness parameters, such as Pulse Wave Velocity (PWV). The majority of studies mainly using carotid-femoral PWV (cfPWV) or brachial-ankle PWV (baPWV). The emerging data have indicated that oscillometrically PWVao proved to be an independent marker of atherosclerotic cardiovascular disease (ASCVD), as well. The main goal of the present study has been to compare the oscillometrically measured parameters of aortic stiffness between patients with Acute Coronary Syndrome (ACS) and patients with stable ischemic heart disease (SIHD). In 100 patients with Acute Coronary Syndrome (Group 1) and 91 patients with Chronic Coronary Syndrome (Group 2) arterial stiffness was assessed by oscillometrically measured complex arterial function. The highest strength of the association revealed between Aortic Pulse Wave Velocity, PWVao (m/s), and incidence of Acute Coronary Syndrome (ACS) with Odds Ratio (OR) of 9.41; 95% CI (4.86, 18.2). Next was Augmentation Index, AIx (%) with OR=5.11; 95% CI (2.65, 9.86), and last Central Systolic Blood Pressure SBPao (mmHg) with OR=3.15; 95% CI (1.63, 6.1). An oscillometrically measured parameters of arterial stiffness, such as Aortic Pulse Wave Velocity (PWVao), Augmentation Index (AIx), and Central Systolic Blood Pressure (SBPao) may be useful in terms of early risk stratification and prevention of Acute Coronary Syndrome (ACS).

COMPARISON OF THE OSCILLOMETRICALLY MEASURED AORTIC PULSE WAVE VELOCITY, AUGMENTATION INDEX AND CENTRAL SYSTOLIC BLOOD PRESSURE BETWEEN PATIENTS WITH ACUTE CORONARY SYNDROME AND CHRONIC CORONARY SYNDROME.

The purpose of the study was to identify the psychological peculiarities of patients with disorders of the musculoskeletal system in order to identify in the future the targets of medical and psychological intervention, correction and complex treatment of this contingent. The study methods: Inventory form SF-36 - short form of the nonspecific Inventory for determination of a life quality (The 36-Item Short Form Health Survey, Coping Inventory for Stressful Situations, CISS), S. Norman, D. F. Endler, D. A. James, M. I. Parker, adopted by T. L. Kryukova, A. Buss and A. Durkee Hostility Inventory, BDHI, adopted by A. L. Osnitsky), Test of self-assessment of stress resistance of S. Kouhen and G. Willianson The study showed a significant difference (p<0,05) in the quality of life of the examined main group and comparison group. In the course of the study it was confirmed that the significant reasons for the decrease in the quality of life of the patient are age-related, individual-psychological differences and social ones. Correlation analysis revealed the relationship of aggression with indicators of the quality of life of the subjects, a direct relationship between indicators of physical health and physical functioning and between indicators of pain and physical functioning, an inverse relationship between indicators of stress resistance and physical health. Identification of the targets of medical and psychological correction will make it possible to develop a program of prevention, rehabilitation of people of mature age with with disorders of the musculoskeletal system, taking into account the factors of influence and the involvement of relevant specialists while maintaining an individual approach.

PSYCHOLOGICAL PECULIARITIES OF PATIENTS WITH DISORDERS OF THE MUSCULOSKELETAL SYSTEM

It is not so easy to understand the essence of the definition of health, although this phrase consists of two words that have an important semantic meaning, that is, is a generalized characteristic, and physical is related to the capabilities of a person. The purpose of this study was to study the relationship between some physiological indicators of the health of the Armenian population. A cross - sectional study of 2000 residents aged 11 to 79 years (men n=429, average age - 33.7±13.2 and women n=1171, average age-34.6±12.1) of the Republic of Armenia was conducted. Physiological indicators (wellness and its component indicators-body composition, endothelium and stress resistance (stress scores) and) were measured using the Mutiscan BC-OXi PLUS Version 2.9 RU monitoring system to study indicators with acceptable reproducibility and a significant difference in values. Studies of the general sample showed that among the population of Armenia, the indicators of endothelium and wellness are within the normal range, the indicator of body composition is slightly below the norm, and the indicator of stress resistance is on the border between the norm and the violation. The average indicators of stress resistance, endothelium and wellness are significantly higher in women compared to men and a reliable direct proportional relationship of average strength between the indicators of stress resistance-wellness, endothelium-wellness and body composition-wellness in both groups are revealed. It was found that the correlation infrastructure (according to Spearman and Pearson) of the above indicators is subject to significant changes depending on gender, while the level of these indicators (in points) in the studied samples is significantly less susceptible to changes.

PECULIARITIES OF RELATIONSHIP BETWEEN SOME PHYSIOLOGICAL INDICATORS OF PHYSICAL HEALTH AMONG THE POPULATION OF ARMENIA

Hoffmann-Clayton procedures appears to be promising surgical treatment in severe rheumatoid forefoot deformities. It has been reported that 80% to 90% of foot deformities in adults are due to rheumatoid arthritis. Despite of various surgical approaches, early functional and cosmetic results have been the greatest concern among patients. Thus, optimal surgical approach in correction of severe rheumatoid forefoot deformities is of vital importance for better subjective and clinical results. Clinical study was conducted on 56 painful chronic rheumatoid foot who were treated by arthrodesis of 1st metatarsophalangeal (MTP) and lesser metatarsal head resections. They were divided into 2 groups based on surgical approach in lesser metatarsal head resections. 1st group had 25 feet with dorsal approach (Clayton) and 2nd group - 31 feet with plantar approach (Hoffmann). Subjective and clinical outcomes were evaluated in both groups. The mean post-operative AOFAS scores were 67.82 (range: 32 - 82) and mean post-operative Foot Function Index (FFI) was 0.51 (range: 0.23 to 0.63) in both groups. Eighty seven percent (48/56 feet) reported early pain relief, improved cosmetic appearance, and improved footwear comfort in Hoffmann group. The mean hallux valgus angles improved from 37 to 15 degrees and the 1st intermetatarsal angle from 17 to 8 degrees in both groups. Four feet had non-union of the 1st MTP joint arthrodesis and three among them were re-operated. Hoffmann and Clayton procedures are optimal methods for excision arthroplasty of lesser metatarsal heads. However, Hoffmann (plantar approach) serves to be more convenient resulting in early recovery, adequate functional stability, rehabilitation and better cosmetic results.

SUBJECTIVE AND CLINICAL OUTCOMES OF SURGERY FOR CORRECTION OF RHEUMATOID FOREFOOT DEFORMITIES.

The article describes the effectiveness of the treatment of chronic apical periodontitis in one visit compared to multiple visits. A systematic review of the literature was conducted analyzing articles published on PubMed, Google scholar, ResearchGate and ScienceDirect between 2011 and 2021, treatment effectiveness was the selected outcome variable. A total of 21 articles were selected, 9 of which were selected for detailed review. Chief question in this article was: which treatment approach is more effective? Cleaning, Shaping, disinfecting and obturating in one visit? Or multiple (two) visit protocol? Chronic Apical Periodontitis is treatable in one visit, if done properly. Chronic Apical Periodontitis is treatable in one visit, if done properly, with proper shaping, disinfection, and proper hermetic obturation of the root canal. Bioceramic sealers showed the highest success rate out of all sealers used, the usage of 3-5.25% sodium hydrochloride and 17% ethylenediaminetetraacetic acid showed favorable results, although photodynamic therapy and ultrasonic activated irrigation showed outstanding results.
