Critical Strategies: Psychotherapy in Managed Care

Summary Using one practice whose history parallels the history of independent psychological practice in the U.S. as a case example, the article chronicles the major changes in practice patterns that have occurred in response to changes in reimbursement. It attempts to show how changes in reimbursement practices have altered and shaped the scope of professional practice as well as the nature of psychological care that is available to society. It is felt that the case example has heuristic value for understanding and documenting what is happening to the profession and where it may be going.

Summary Since World War II the scope of our professional services has grown. The sense of an expanding, increasingly recognized profession came to an end with the emerging dominance of managed care. Psychologists are now attempting to adjust to the impact that managed care has on the delivery of psychological services. New areas of practice and new forms of treatment will continue to emerge as psychologists struggle to survive. However, this article focuses on the problems of the profession itself. Solutions are suggested.

Summary The advent of managed care has raised many ethical issues for social workers. These issues relate to social workers' ethical responsibilities to their clients in the areas of self-determination, informed consent, competence, conflicts of interest, privacy and confidentiality, and the interruption and termination of services. Using the newly revised Code of Ethics of the National Association of Social Workers as a basis for discussion, this article identifies the issues and dilemmas facing social workers delivering services in a managed care environment. Formal and informal safeguards are provided for practitioners which will help them avoid ethical violations while protecting the integrity of their mental health treatment.

Summary Confidentiality represents an important obligation to patients of mental health professionals. It typically invokes an explicit promise to reveal nothing about a patient without that individual's expressed, informed consent (McQuire, Toal, & Blau, 1985). The advent of the Tarasoff case has changed the scope of confidentiality both ethically and legally since the birth of the psychology profession. Consideration for the welfare of the individual patient was weighed against consideration for the welfare of society as a whole. How confidentiality law affects patients' self-disclosures, the therapeutic relationship, and ultimate therapy outcome is not yet clear. This paper explores the effects of confidentiality law on these issues.
