Food Policy, Systems, and Environment: Strategies for Making Healthful Food the Easiest Choice

Background: Patients undergoing surgical treatment for obesity are likely to experience changes in their health-related quality of life (HRQOL). The purposes of this study were to investigate HRQOL before and after laparoscopic gastric bypass (LGB) surgery, and to describe the influence of weight loss surgery on HRQOL in morbidly obese patients. Methods: Patients with morbid obesity who underwent weight loss surgery at a hospital in Taiwan were recruited. The standard survey was administered before surgery and again 3 months after LGB to patients with a body mass index (BMI) of >40 kg/m2 or a BMI ≧35 kg/ m2 when combined with diabetes/hypertension. Results: The sample included 40 patients; 62.5% were women and the average age of participants was 30.1 years. Scores on the subscales of the Medical Outcomes Study SF-36 demonstrated significant improvements between baseline and follow-up. The physical health domain was most affected. There were also significant improvements related to physical functioning, physical-role limitations, bodily pain, general health perceptions, emotional-role limitations, and general mental health at 3 months after LGB. BMI explained 26.5% of the variance in HRQOL change after LGB. The higher the BMI level, the poorer the HRQOL after LGB. Marital status and body weight explained 26.8% of variances of mental component summary after LGB. Conclusions: Significant weight loss was achieved as early as 3 months after LGB and was associated with substantial improvement in HRQOL in patients. The results of the study foster knowledge of and support research about improving the HRQOL and lives and nursing care of those suffering from obesity.

Early Health Status and Health-Related Quality of Life after Laparoscopic Gastric Bypass Surgery in Morbidly Obese Patients

Obesity prevention is a national initiative with the attention of First Lady Michelle Obama and is being addressed on many levels with a multifactorial approach. However, many children and adolescents are beyond the benefit of prevention efforts and need treatment. Bariatric surgery in the adolescent population is a broadly debated topic that generates valuable discussion about the management of childhood obesity. This article seeks to describe the unique and pertinent developmental aspects of preparation and support given to obese adolescents and their families, and to present the experience at the University of Illinois Medical Center Adolescent Weight Management Program in Chicago, Illinois. There is a lack of scientific evidence in the current literature addressing what factors influence an adolescent's success with weight loss surgery, or clinical practice guidelines that outline recommendations for preparation of adolescents for bariatric surgery. It is the hope that reporting this experience will stimulate more scientific inquiry in this area. The purpose of this article is to encourage the provider to focus toward assisting the adolescent through the educational and psychosocial stages, with a goal of creating a solid foundation for success with bariatric surgery. The following stages and themes are noted from our clinical and management experience with adolescents in a weight management program that included the option of bariatric surgery. Erikson's Psychosocial Theory of Human Development1 was the construct for the developmental care provided in this novel program at the University of Illinois Medical Center.

Healthcare reform continues to generate changes that impact all facets of patient care. One of these impending changes is Value-Based Purchasing (VBP). VBP will create an incentive pool that will allow participating organizations to earn a specified amount based upon core measures and patient satisfaction, specifically measured by HCAHPS (Hospital Consumer Assessment of Healthcare Providers and Systems). For the first time, reimbursement will be linked to nursing care. One healthcare organization's experience reveals distinct patient satisfaction differences between bariatric and non-bariatric surgical patients.

It has taken many years for RNs to achieve recognition as healthcare professionals possessing the exceptional skill and knowledge necessary to provide high-quality care. RNs need to be aware that the care they give and the documentation of that care may be reviewed by others from a legal perspective, and protect themselves accordingly by being as accurate and attentive as possible throughout each and every shift.1 Exposure to malpractice liability is an unfortunate consequence of modern-day nursing practice. An understanding of malpractice will assist nurses in identifying situations that may expose them to legal liability, and will hopefully lead to improved patient care.2

In the 2000 publication, To Err is Human, experts reported that as many as 98,000 people die in any given year from medical errors that occur in U.S. hospitals. In 2012, William Charney argues that, to make matters worse, this has been a growing problem for more than a decade, with little to no systemic approach to address the problem. Charney explains that, in 2009, 788,558 deaths in the United States occurred as a result of medical errors and hospital acquired infections, representing one-third of all deaths in America that year.1 This article explores the role of the bariatric nurse in developing moral courage to address the threat of workplace bullying on patient safety.

Background: The rise in obesity in the United States has led to an increase in the number of bariatric procedures performed annually to treat obesity. It has become the only treatment method that provides significant and sustained weight loss in obese patients with resultant improvement in obesity-related comorbidities.1 Objective: The aim of this project was to evaluate the knowledge and attitudes of the nurses regarding aspects of bariatric surgery at a hospital that is a Center for Excellence for bariatric surgery. Methods: A voluntary survey was conducted using SurveyMonkey® to assess the nurses' current knowledge of bariatric surgical procedures and attitudes toward patients undergoing bariatric surgery. Results: The results of the survey showed that 66.7% had no previous experience of caring for bariatric surgical patients, 3.3% did not understand gastric bypass surgery, 6.7% did not understand the sleeve, and 26.7% did not understand the duodenal switch surgery. In addition, 43.3% felt that bariatric surgery improves the quality of life for some obese patients who are compliant with diet and postoperative instructions, but 6.7% of nurses surveyed did not agree with bariatric surgery as a means for improving quality of life. Conclusion: Since bariatric surgery has become more popular in recent years to treat obesity, it is imperative that nurses understand the procedures and how to care for this patient population. The results of this survey were used to educate the nurses further at this facility about bariatric surgery and the duodenal switch procedure.
