Journal of Medical and Biological Engineering

Purpose: From the myofibrils to the whole muscle scale, muscle micro-constituents exhibit passive and active mechanical properties, potentially coupled to electrical, chemical, and thermal properties. Experimental characterization of some of these properties is currently not available for all muscle constituents. Multiscale multiphysics models have recently gained interest as a numerical alternative to investigate the healthy and diseased physiological behavior of the skeletal muscle. Methods: This paper refers to the multiscale mechanical models proposed in the literature to investigate the mechanical properties and behavior of skeletal muscles. More specifically, we focus on the scale transition methods, constitutive laws and experimental data implemented in these models. Results: Using scale transition methods such as homogenization, coupled to appropriate constitutive behavior of the constituents, these models explore the mechanisms of ageing, myopathies, sportive injuries, and muscle contraction. Conclusion: Emerging trends include the development of multiphysics simulations and the coupling of modeling with the acquisition of experimental data at different scales, with increasing focus to little known constituents such as the extracellular matrix and the protein titin.

Optimization of Pulsed Electromagnetic Field Parameters for Knee Osteoarthritis Treatment and its Clinical Validations

Abstract Purpose Breast cancer, the most prevalent cancer among women, is closely linked to abnormal glucose and lipid metabolism, leading to radioresistance by upregulating survival-signaling pathways. Overexpression of fatty acid synthase (FASN), a key enzyme in lipogenesis, results in excessive lipid synthesis in breast cancer. This study evaluated whether FASN inhibition enhanced the radiosensitivity of breast cancer cells and inhibited their progression, potentially uncovering mechanisms for new therapeutic strategies. Methods MCF-7 breast cancer cells were treated with the FASN inhibitors orlistat and TVB-3166, and cytotoxicity was assessed using the MTT assay. Protein expression changes, migratory ability, and responses to radiotherapy were analyzed by the Western blotting, Transwell, and MTT assays, respectively. To confirm FASN dependence, MCF-7 cells were infected with shFASN lentivirus to verify the specificity of the observed effects to FASN inhibition. Results Both orlistat and TVB-3166 treatments induced significant cell death. Reduced FASN, HKII, pERK, and pAKT expression levels, along with an increased BAX/p-BCL2 ratio, indicate that FASN inhibition disrupted cell proliferation and promoted apoptosis by altering tumor metabolism. Furthermore, decreased MMP9 expression correlated with reduced cell migration after FASN inhibition. Importantly, FASN inhibition significantly and dose-dependently enhanced the radiosensitivity of MCF-7 cells. These findings were validated using shFASN lentivirus, confirming that the observed effects were FASN-dependent. Conclusion FASN inhibition limited survival and migration and enhanced radiosensitivity in MCF-7 cells. These findings indicate the potential efficacy of FASN inhibitors as standalone therapies or as adjuncts to radiotherapy for breast cancer.

Abstract Purpose Cervical total disc replacement (TDR) is commonly performed to treat degenerative cervical spondylosis, but it often faces challenges such as implant wear and migration. This study aimed to develop and evaluate a novel 3D-printed TDR featuring a titanium endplate and a lattice-structured poly-ether-ether-ketone (PEEK) design. The primary objective was to replicate the natural motion of the cervical disc while addressing complications associated with conventional TDRs. Methods The novel implant was created using additive manufacturing techniques, incorporating three lattice-structured PEEK designs (Cross, Octet, and Ventiles) for the nucleus pulposus and annulus fibrosus components. A finite element analysis was conducted to compare the biomechanical performance of the novel TDR with an intact cervical disc and a commercially available TDR (Baguera®C, Spineart SA, Geneva, Switzerland). Key parameters, including maximal von Mises stresses, range of motion, paths of the instantaneous center of rotation, and facet joint stresses, were evaluated under physiological loads (100 N follower load and 1.5 Nm pure moments). Results The novel 3D-printed TDR maintained structural integrity, with stresses remaining within the yield strength of PEEK. The biomechanical performance closely resembled that of an intact cervical disc, demonstrating similar ranges of motion, instantaneous center of rotation paths, and facet joint stress distributions. Conclusion The findings indicate that the innovative 3D-printed TDR may restore normal cervical spinal kinematics more effectively than existing commercial options, potentially reducing the risk of post-operative facet joint syndrome. Further experimental and clinical studies are recommended to validate these results.
