Emergency Room (ER) can be considered as a High Reliability Organization expected to guarantee reliability despite facing hectic and unpredictable events. To achieve this status, ER should invest on the management style of Organizational Mindfulness, focused on anticipation and containment of errors. The aim of this exploratory research was to investigate how Organizational Mindfulness occurs in Italian ERs and what is the role of Safety Culture and Work Engagement in predicting Organizational Mindfulness measured with an ad hoc questionnaire. Results show how the communal sphere of Safety Culture seems a better predictor of Organizational Mindfulness than the individual sphere of Work Engagement.

An explorative study of antecedents of organizational mindfulness in emergency room

The application of biomarkers in TBI management remains underutilised with paucity of data in Asian populations. This study investigated the correlation between UCH-L1 and GFAP with TBI severity and patient outcomes in a Malaysian tertiary centre. The study was conducted at Universiti Malaya Medical Centre in Kuala Lumpur, Malaysia, from February 1, 2017, to November 30, 2019. GFAP and UCH-L1 were measured in 61 TBI cases and 19 controls. Correlations between biomarkers and TBI severity, as well as patient outcomes, were assessed using Spearman's rank correlation coefficient. GFAP/UCHL1 showed significant correlation with Marshall CT classification (r=0.437, p&lt;0.001), Glasgow Coma Scale on arrival (r=-0.444, p&lt;0.001), and Acute Physiology and Chronic Health Evaluation II (APACHEII) score (r=0.501, p&lt;0.001). GFAP demonstrated fair-to-good accuracy in predicting TBI severity and outcomes. A consistent cut-off value of 0.01845 ng/mL for GFAP and 0.01960 for GFAP/UCHL1 predicted TBI severity, with high sensitivity (72.2-100%) and acceptable specificity (38.8-80.0%). GFAP and GFAP/UCHL1 showed promising utility in predicting TBI severity and patient outcomes in the Asian population. The findings underscore the potential clinical significance of biomarker assessment in TBI management, though further validation in larger cohorts is warranted.

Correlation of UCH-L1 and GFAP in predicting severity of traumatic brain injury in an Asian population

Data on patients with poisoning requiring ICU (Intensive Care Unit) &amp; HD (High Dependency) care in Singapore is inadequate. In this study, we describe the epidemiological, clinical characteristics, implicated agents, interventions, and outcomes of patients with toxic exposures admitted to HD &amp; ICU. We conducted a retrospective and observational study. Hospital records of all cases referred to CGH Toxicology Service from 1 January 2015 to 31 December 2022 were reviewed. A total of 245 (9.6%) out of 2547 patients reviewed required HD or ICU care. 131 patients were admitted to ICU, while 114 patients were admitted to HD beds. Amongst those admitted to ICU, male to female ratio was approximately 60:40, and the commonest age group was 31-40 years old (19.9%), while deliberate self-harm poisoning accounted for 52.7 % of these cases. Benzodiazepines, anti-psychotics, anti-depressants, and opioids were most involved.The fatality rate was 3.2 %. Cardiotoxic drugs, anti-psychotics, and anti-depressants were associated with most fatalities. Benzodiazepines, anti-psychotics, anti-depressants, and opioids were the most associated with ICU admissions, while cardiotoxic drugs, anti-psychotics, and anti-depressants were associated most with mortalities.

Emergency department poisoning and toxicology patients admitted to the high dependency and intensive care units: an eight-year analysis

Acute Respiratory Distress Syndrome (ARDS) is a complex and progressive lung injury that results in respiratory failure, often caused by pulmonary or systemic disorders. We present a 32-year-old male who developed ARDS following intranasal inhalation of cocaine. Diagnosis was established thorough clinical evaluation, identification of characteristic ultrasonographic patterns, and careful exclusion of other potential causes. The patient was managed with conservative medical therapy, including non-invasive ventilation, bronchodilators, antibiotic therapy, and comprehensive supportive care. Despite initial concerns about the severity of his condition, the patient's outcome was favourable, reflecting the diverse presentations and potential reversibility of ARDS. This case highlights two main aspects: i) the critical role of emergency physicians in recognizing and managing ARDS, even when the underlying cause is not immediately apparent; and ii) the importance of considering illicit drug use as a potential trigger during the initial assessment of respiratory complications.

An unusual case of acute respiratory distress syndrome in a young man

In Italy, over 86% of patients with mental disorders have one emergency department visit per year, with 6–9% having two visits and approximately 2% having three visits. It is not significant to include those with four or more visits in this data set. In the context of the COVID-19 pandemic, there has been a notable increase in the utilisation of emergency services for psychiatric concerns. In our emergency department (ED), we observed a 10% increase in admissions for psychiatric problems, psychomotor agitation, behavioural disorders, and drug intoxication between 2022 and 2023. We present an organisational management model that has been found to be effective for patients with minor psychiatric problems, those who repeatedly present to the emergency department, and/or those with discontinuous referrals to the relevant mental health centre or acute episodes. One possible strategy could be to consider directing psychiatric emergencies that can be deferred to territorial crisis management from the Trento emergency department to the mental health centre.

A modern management model for patients with acute and subacute psychiatric problems in the emergency department of Trento (Northern Italy)

This case report from England describes an 8-year-old girl who suffered severe health complications culminating in cardiac arrest due to profound neglect and malnutrition. The patient presented with cardiac arrest, which was attributed to her severe wasting. Upon admission, her body mass index measured 12.8 kg/m2 (z-score ranging between -2 and -3), indicative of severe malnutrition. The patient presented with respiratory, renal, and hepatic failure, in addition to disseminated intravascular coagulation. Resuscitative measures were administered, and she underwent 162 hours of ventilation in the paediatric intensive care unit. Despite the gravity of her initial presentation, the patient exhibited a notable recovery. Paediatric cardiac arrest stemming from extreme nutritional neglect is a rare occurrence in England. This case underscores the paramount importance of prompt intervention and comprehensive medical oversight. The patient's relatively favourable recovery underscores the potential for positive outcomes when timely and appropriate medical interventions are employed.
