Writing a scientific article: A step-by-step guide for beginners

Abstract Purpose Hypothermia is a serious condition in older adults. Knowledge of a priori chances of underlying diseases may affect initial management, hence prognosis. This systematic review provided an overview of existing literature on the incidences of underlying causes of hypothermia in older patients at the emergency department. Methods MEDLINE, The Cochrane Library, and Embase were searched up to February 1st, 2022. Inclusion criteria were age ≥ 65 years, emergency department setting, and body temperature &lt; 36.0 degrees Celsius. Exclusion criteria were iatrogenic hypothermia, no underlying cause reported, and patient selection based on specific diseases. Title/abstract and full-text were screened and quality was assessed using the Joanna Briggs Institute Critical Appraisal Tool. Data were presented using descriptive statistics and narrative analyses. Results Forty-one reports were included, including 6 cohort studies and 35 case reports. The 6 studies involved 2173 hypothermic patients, whose age varied from a mean of 67 to a median of 79 years and temperature from a median of 30.8 to a mean of 33.7 degrees Celsius. One study reported about primary hypothermia (incidence of 44%). Acute medical illness was often reported as underlying cause of secondary hypothermia (49–51%). Reported incidences of infection and sepsis ranged from 10 to 32%, of trauma up to 14%, and of alcohol intoxication from 5 to 26%. Conclusion Limited studies have been published regarding this topic, and the overall quality of the evidence was graded as low. Causes that should not be missed include acute medical illness, trauma, alcohol intoxication, primary hypothermia, thyroid failure, and drug-induced hypothermia.

Association between dietary vitamin E intake and cognitive decline among old American: National Health and Nutrition Examination Survey.

Abstract Aims Sarcopenia is linked to impaired physical function and exercise tolerance. The aim of this systematic review and meta-analysis was to examine the association of sarcopenia and low appendicular skeletal muscle (ASM) with biomarkers of cardiac function, B-type natriuretic peptide (BNP) and its N-terminal fragment (NT-proBNP), in patients with heart failure (HF). Methods and results From inception until May 2023, a systematic literature search of observational studies was undertaken utilizing the PubMed, Web of Science, Scopus, and Cochrane Library databases. A meta-analysis employing a random-effects model was used to compute the pooled effects (CRD42023418465). Overall, 16 studies were included in this systematic review and meta-analysis. Our main analysis showed that sarcopenia in HF was linked to significantly higher levels of BNP (MD: 87.76, 95% CI 20.74–154.78, I 2 = 61%, P = 0.01) and NT-proBNP (MD: 947.45, 95% CI 98.97–1795.93, I 2 = 35%, P = 0.03). Similarly, low ASM was associated with significantly higher levels of BNP (MD: 118.95, 95% CI 46.91–191.00, I 2 = 93%, P &lt; 0.01) and NT-proBNP (MD: 672.01, 95% CI 383.72–960.30, I 2 = 2%, P &lt; 0.01). The quality of the included cohort studies was considered moderate, using the binary AXIS checklist and the Cochrane Tool to Assess the Risk of Bias in Cohort Studies. Conclusions In patients with HF, sarcopenia and reduced ASM are associated with considerably higher plasma levels of BNP and NT-proBNP. Future research is required to investigate whether sarcopenia may express dysregulated biomarkers of cardiac function. Graphical abstract

Abstract Purpose Frailty in older adults is associated with multiple adverse health outcomes, while evidence on its successful prevention has been scarce. Therefore, we analyzed the effectiveness of different interventions for the prevention of frailty onset. Methods In this systematic review, eight databases were searched for randomized controlled trials of interventions in non-frail (i.e., robust or pre-frail) adults aged ≥ 60 years that assessed frailty incidence at follow-up. Additive component network meta-analysis (CNMA) was conducted to isolate the effect of different intervention types on the main outcome of frailty incidence, reporting relative risk (RR) with 95% confidence intervals (CI). The effect on gait speed was analyzed as an additional outcome using a classic network meta-analysis and the standardized mean difference (SMD) with 95% CI. Results We screened 24,263 records and identified 11 eligible trials. Nine trials (842 participants, all categorized according to the physical phenotype) in pre-frail (seven RCTs) and robust/pre-frail (two RCTs) older adults were included in the CNMA. Physical exercise significantly reduced frailty incidence at follow-up (RR 0.26, 95% CI 0.08; 0.83), while this was not found for nutritional interventions (RR 1.16, 95% CI 0.33; 4.10). Interventions based on physical exercise also improved gait speed (SMD 1.55, 95% CI 1.16; 1.95). In addition, 22 eligible trial protocols without published results were identified. Conclusion Interventions based on physical exercise appear to be effective in preventing the onset of frailty in older adults. Although the available data are still limited, results from ongoing trials may add to the body of evidence in the foreseeable future.

Abstract Background Concerns about falling (CaF) are common in older adults. They can lead to various negative outcomes, including an increased risk for future falls. The Worlds Falls Guidelines recently published recommendations for assessing and treating CaF. However, the extent to which these guidelines have been adopted into falls prevention practice (in addition to the barriers preventing implementation) are currently unknown. Methods A cross-sectional survey was completed by 114 healthcare professionals working in falls prevention and rehabilitation services in the UK and Ireland. The survey explored their experiences and perceptions regarding the management and assessment of CaF. Results Only 39% of respondents reported using the Falls Efficacy Scale-International (FES-I), a tool recommended by the World Falls Guidelines for assessing CaF. Healthcare professionals in hospital settings were significantly less likely to use the FES-I compared to those working in the community ( X 2 = 6.324, p = 0.043). While there was no significant difference between settings regarding the type of intervention used to manage CaF, only about 50% of participants adopted a holistic approach combining physical and psychological strategies as recommended by the World Falls Guidelines. The most commonly identified barriers to clinical management of CaF were a lack of both time and perceived effective interventions, particularly for those working within hospital settings ( X 2 = 6.209, p = 0.013 and X 2 = 3.752, p = 0.053, respectively). Conclusion These findings indicate low levels of adoption of the World Falls Guidelines recommendations for assessing and managing CaF, especially in hospital settings. Future work should focus on addressing these barriers to maximise the clinical adoption of these recommendations.
