trimestral RESUMENPaciente de 38 años, diagnosticado de asma y ansiedad que utilizaba 2 medicamentos (salbutamol 100 mcg inhalador (2 pulv.cada 6 horas) y diazepam 5 mg (0-0-1)), acude a la Farmacia Comunitaria (FC) para retirar un tratamiento prescrito por el Médico de Atención Primaria (MAP) tras diagnóstico de un cuadro ansioso-depresivo.Desde el Servicio de Dispensación (SD) se detectó un Problema Relacionado con el Medicamento (PRM) de un potencial error en la prescripción, que podría tener asociado un Resultado Negativo asociado a la Medicación (RNM) ante uso concomitante de desvenlafaxina y mirtazapina, así como un PRM de Problema de Salud (PS) insuficientemente tratado ante una propuesta de suspensión de uso de diazepam, que podría derivar en un RNM de Necesidad de Tratamiento ante el riesgo de empeoramiento de episodios de ansiedad.Desde la FC, se elaboró un informe de derivación con recomendaciones que fueron aceptadas por el MAP.Se llevó a cabo un seguimiento del caso que permitió realizar un mejor control de los PS presentados por el paciente, así como la resolución de los PRM y RNM detectados, garantizando así un uso racional, seguro y eficaz del medicamento.

RESUMENPresentación del caso.Varón de 100 años presenta un episodio de bradicardia profundo.El cuidador del paciente avisa a urgencias y estos retiran el bisoprolol controlándose la bradicardia.Una vez solucionado el problema nos preguntan si alguno de sus medicamentos puede tener relación con la bradicardia.Estudio y evaluación.Al revisar todo el tratamiento del paciente, muy complejo, no podemos establecer una relación clara entre alguno de sus 19 medicamentos y la bradicardia, salvo el bisoprolol ya retirado, pero encontramos otros 6 problemas que intentamos solucionar.Resultado.De los 6 cambios propuestos se aceptan 3. Comentario final.La revisión de un tratamiento complejo probablemente permita detectar algunos aspectos mejorables en el mismo.The review of the treatment of a very complex patient easily finds areas for improvement.Case report

The reason for existence of a scientific journal is to disseminate knowledge of science. The objective is a continuous improvement in the quality of what is published and an increase in the number of readers. In this sense, the impact factor is an indicator that helps us visualize the improvement in the quality of our journal. To improve this impact factor, indexing in the most used databases is very important.Farmacéuticos Comunitarios is indexed in several index journals but we were missing the most used ones. In this last year we have requested indexing in the most important ones, SCOPUS and Medline. In November we received SCOPUS approval and are close to getting it on Medline. In September we were approved by the editorial criteria and in December by the scientists. We only need the technical criteria to be able to appear in Medline and in its online version PubMed. We are working on it and it is very likely that within a year we will be indexed in PubMed.

Monitored Dosage Systems (MDS) are an efficient, reliable and approved device for drug reconditioning in pharmacy. These systems imply a review on proper drug use and the collaboration between primary health care and pharmacists. The case study describes a female patient with a surgical intervention due to lumbosciatica in 2021 and 2022. Patient describes uncontrolled chronic pain and confusion related to improper drug use. During regular dispensing of her medication, these medicine-related problems (MRP) were detected and the patient was referred to the MDS service. After its implementation, the patient's confusion was eliminated and pain management was achieved, increasing her quality of life. As a conclusion, the different health services provided by the pharmacy can improve a patient's quality of life, treatment adherence and MRP detection.

Aging is a global reality that is accompanied by an increase in polypharmacy. In this vital stage, caregivers emerge as relevant actors since their intervention impacts on the safety of the medication use process and on the quality of life of the patient they assist. As the knowledge about the treatments that caregivers supervise for the patient under their care conditions the health outcomes of the medications, the main objective of this study is to characterize the caregivers of patients who use opioid analgesics and to identify factors that increase the risk of the appearance of Medication Related Problems (DRP).The results reveal that the caregiver profile affects the health outcomes of this therapeutic group in both the prevalence and type of DRP. Of the 63 patients using opioid analgesics under pharmacotherapeutic follow-up during this study, 17 (27%) had caregivers. The caregiver in this study was predominantly female (61.1%).Considering DRP and kinship, a higher prevalence of DRP was observed when the caregivers were children or external personnel.Analyzing the pharmacotherapy of these caregivers, 4.8% have started antidepressant treatment, 3.2% hypnotic treatment after becoming caregivers of these patients. The Zarit Test reveals that 29.4% of the caregivers of these patients manifest overload.We conclude that Pharmaceutical Care should consider the opportunity and relevance of designing and implementing intervention and education protocols focused on caregivers.

Regulatory agencies such as the Food and Drug Administration (FDA) and the European Medicines Agency (EMA) recognize pharmacogenetics as a key tool in their pharmacological guidelines for pharmaceutical counseling. In this context, community pharmacies play a crucial role in addressing this healthcare need, which could lead to a significant improvement in patients' quality of life by preventing ineffective or contraindicated treatments.In this work, we conducted a systematic review of the available scientific evidence regarding druggene interactions relevant to community pharmacy. We identified the main genes and polymorphisms associated with treatment response and adverse effects in primary care. Finally, we propose a model for implementing pharmacogenetic services in community pharmacies.
