Clinical Applications of the Markers of Lithogenic Activity in Kidney Stone Disease

Mesenchymal stromal cells (MSCs) are a promising resource for cell therapy due to multiple differentiation potentials and the ability to exert a complex pro-regenerative effect on injured tissues. The results of numerous studies indicate the possibility of their use in the treatment of the most common diseases of the male urogenital system. In particular, experimental models of toxic, ischemic or traumatic testicular injury, cryptorchidism and fasting-induced testicular insufficiency have shown the beneficial effect of MSC transplantation or injection of their secretory products on the restoration of spermatogenesis and testosterone production. Due to their immunomodulatory properties, MSCs may be useful for the treatment of acute and chronic prostatitis. It is possible that their inherent tropism to tumors can also be used in the treatment of prostate cancer. However, due to the ambiguity of the MSC effect on the tumor process, their use in this field of medicine should be considered with caution. There are also encouraging results of MSC transplantation in erectile dysfunction caused by age-related changes, diabetes mellitus, impaired penile blood supply or innervation, irradiation or Peyronie disease. Experimental data on the therapeutic effects of MSCs obtained in numerous animal studies, and in some cases in clinical trials, can become the basis for the development of new approaches to the treatment of diseases of the main organs of the male urogenital system.

Prospects of using mesenchymal stromal cells for the treatment of the male urogenital system

The review article discusses modern aspects of the physiological role of zinc in the prostate gland, as well as the consequences for the prostate gland of deficiency of this one of the most important vital trace elements. The mechanisms of intracellular zinc transport, as well as various physiological processes in prostate tissue requiring the mandatory presence of zinc, are covered in more detail. In a separate section of the literature review, the modern evidence base of the relationship of zinc deficiency in the prostate gland with chronic prostatitis is presented and the results of modern studies showing the need for zinc replacement therapy in the framework of complex pharmacotherapy of chronic prostatitis occurring against the background of zinc deficiency are summarized.

The role of zinc in a healthy prostate gland and in chronic prostatitis

Introduction. Antioxidant therapy is the optimal type of treatment for idiopathic male infertility due to the good ratio of efficacy and safety. Considering that oxidative stress is one of the pathogenetic links in sperm DNA damage, antioxidant drugs can also reduce the DNA fragmentation index. Men who are overweight or severely obese have a high risk of sexual dysfunction and reproductive disorders. Antioxidant therapy plays a leading role in infertility associated with metabolic syndrome. The main active ingredients in «BESTFertil-DHA» are L-carnitine, eicosapentaenoic and docosahexaenoic acids, the action of which is complemented by other components, which makes the use of the complex attractive in combination with male infertility and metabolic syndrome. However, it is unknown how well they work with a combination of these pathological conditions. Aim. Is to evaluate the effectiveness of administration of Russian complex «BESTFertil-DHA» on spermogram parameters and the index of sperm DNA fragmentation in men from infertile couples with metabolic syndrome. Materials and methods. The study included 60 patients divided into cohort A- 26 patients with metabolic syndrome, cohort B- 34 patients without metabolic syndrome. All patients received «BESTFertil-DHA». All patients underwent a spermogram, an analysis for the fermentation of sperm DNA, blood for sex hormones, markers of metabolic syndrome on the first visit, and on the 2nd visit after therapy. Results. In cohort A, significant results were observed in terms of weight, BMI, C-reactive protein level, and HOMA-IR index. There is a significant improvement in the parameters of the spermogram, a decrease in the index of fragmentation of sperm DNA, a decrease in estradiol, normalization of the T/E2 index. In cohort B, changes in weight, BMI and a number of spermological parameters were observed, there were no changes in metabolic and endocrine parameters. Conclusion. Combined antioxidant therapy in the form of a complex of nutrients «BESTFertil-DHA» effectively reduces the index of fragmentation of sperm DNA and leads to normalization of spermogram parameters, both in the general cohort of patients and among men with metabolic syndrome. Moreover, in the latter, the «BESTFertilDHA» complex in combination with measures to reduce body weight gives additional therapeutic effects, affecting carbohydrate metabolism, aromatization of testosterone and the severity of systemic inflammation.

Prostate cancer is one of the most common cancers and ranks second in frequency among men in the Russian Federation. In this regard, timely and accurate diagnosis of prostate cancer seems relevant. This review is devoted to the relevance of using magnetic resonance imaging with diffusion-weighted images in the diagnosis of prostate cancer. The review represents modern diagnostic methods used in the primary diagnosis of prostate cancer, disease staging, assessment of response to therapy and detection of cancer recurrence. The authors discuss the advantages and disadvantages of the method, and further prospects for the use of diffusion-weighted images in the examination of patients with prostate cancer. The search for the preparation of this review was performed using electronic bibliographic databases PubMed, E-library, Medline, Google Scholar.

Diffusion-weighted imaging in diagnosis of prostate cancer: a review

Benign prostatic hyperplasia is the most common urological disease in men over 50 years of age. The decrease in the quality of life of patients with benign prostatic hyperplasia dictates the need to study the risk factors for the development of this disease, as well as to develop effective treatment strategies. The development of benign prostatic hyperplasia is characterized by proliferation of stromal and epithelial cells in the transitional zone of the organ that surrounds the urethra, which leads to compression of the urethra and the development of symptoms of the lower urinary tract. The drugs of first choice for the treatment of patients with prostatic hyperplasia are α1-adrenoblockers, among which the selective drug Tamsulosin is the most popular. According to the literature, the use of Tamsulosin is associated with the risk of developing a number of serious side effects, which can negatively affect the quality of life of patients and reduce their adherence to treatment. The article provides information about a combined drug containing bioregulatory peptides and tamsulosin in the form of suppositories, which helps to reduce the systemic effect of the drug and the development of undesirable side effects.

Introduction. Insufficient surgical experience and inadequate choice of surgical treatment options may be the reasons for the failure of treatment of urethral stricture. In this regard, a clinical case from our practice is very indicative. Case: The article is devoted to the description of a clinical case of urethral injury in a man who, at the age of 22 years, suffered a fracture of the pelvic bones as a result of a traffic accident. From the moment of injury to the final operation, the following types of surgical interventions were sequentially performed in this patient: anastomotic endto-end urethroplasty, augmentation urethroplasty using a skin graft, augmentation urethroplasty using a graft of the tunica vaginalis of the testicle, bougienage of the urethra, urethroplasty with a vascularized radial fasciocutaneous flap on a vascular pedicle, 2-stage urethroplasty using a graft of the buccal mucosa according to the type of Bracka I and II operations, perineal urethrostomy. Urethrostomy became the final option for urine diversion; no recurrence of urethral stricture was observed during 4 years of follow-up after urethrostomy. Conclusion. Until recently, the lack of uniform approaches to the treatment of urethral stricture has led to the use of inadequate methods of surgical treatment in various clinics. The recent approval of federal clinical guidelines for urethral stricture should help optimize the treatment of this disease. Keywords: urethral stricture, urethroplasty, urethrostomy

The use of a large number of methods of surgical treatment of urethral stricture in one patient: a clinical case

Introduction. The effect of antibiotics on the microbiota of different organs is the subject of close attention and an urgent problem of modern medicine. The literature mainly contains works related to the effect of antibiotics on the intestinal microflora and some other organs other than the genitourinary organs. On the other hand, the microbiota of many organs, including the prostate, largely depends on the gut microbiota. Objective: to conduct a comparative analysis of the microbiota of prostate secretion (PS) in patients with chronic bacterial prostatitis (CBP) before and after antibacterial treatment. Material and methods. A prospective study of PS was conducted in 38 patients with CBP aged 25 to 50 years. The study of the microbiota of prostate secretion was carried out using cultural methods, real-time PCR and using a gas chromatograph of the Maestro mass spectrometer (Interlab LLC, Russia), by the methods of mass-spectrometry microbes markers (MSMM). Results. According to the statistical analysis of absolute indicators obtained from the MSMM data, a sharp decrease in the number of almost all groups of microorganisms (MO) has been established. At the same time, the level of the cocco-bacillary group decreased from 1,049.50 x cells/g to 142.00 x 105 cells/g, the level of anaerobes from 1,638.00 to 221.50, the level of actinobacteria from 197.00 to 40.00, the level of enterobacteria from 452.50 to 87.00, the level of gram-negative rods from 79.00 to 25.00, the level of fungi and yeast from 56.00 to 23.00, the level of viruses from 24.50 to 3.00x105 cells/g (p &lt;0.0001; p = 0.0004; p = 0.0033; p = 0.0007; p &lt;0.0001). In addition, there was a decrease in the level of resident MO, transient opportunistic, normally non-occurring MO and MO sum, as well as a decrease in the level of plasmologens and endotoxin sum (p &lt;0.0001). When analyzing the relative value in the microbiome of individual MO, a decrease in the proportion of cocco-bacillar flora, actinobacteria, enterobacteria, viruses, transient and normally non-occurring microorganisms was noted, while the proportion of grams of negative rods increased by 48.4% (p = 0.0033), fungi and yeast by 74.4% (p&lt;0.0001), anaerobes by 6.3% (p&lt;0.0001), resident bacteria by 5.3% (p&lt;0.0001). The decrease in the level of plasmologen reached 63.9% (p&lt;0.0001), endotoxin 64.3% (p&lt;0.0001). Conclusion. The analysis showed that antibiotics lead to a decrease in the level of absolute values in almost all microorganisms, regardless of belonging to different taxonomic groups, while the relative values in some groups increase and in others decrease. The state of the microbiota of prostate secretion in patients with chronic bacterial prostatitis before and after antibacterial treatment

According to the Russian Society of Urologists, almost half of men aged 27 to 77 suffer from erectile dysfunction. ED should be considered not only as a medical nosology, but also as a socio-psychological problem with a demographic aspect. There are many methodsfor improving erectile function, including psychological consultations, drug therapy and penile prosthetics. Given the accumulated international experience,the key pointis an accurate diagnosis ofthe cause of erectile dysfunction to determine the correct approach to treatment. Among the many causes of erectile dysfunction, vascular causes are quite common. Given the rapid development of endovascular and low-traumatic approachesto vascular pathology,the technique of improving inflow and outflow obstruction in vasculogenic erectile dysfunction is becoming increasingly obvious and effective. The presented article discussesthe currentstate ofthe issue and possible prospectsfor endovascularsurgery for vascular erectile dysfunction, including significant venous leakage.

Erectile dysfunction: current state of the problem, diagnostic methods and X-ray endoscopic surgical treatment

In this work, we developed and introduced into clinical practice direct percutaneous X-ray endovascular occlusion of the veins of the prostatic plexus. Indications for this operation were a decrease in the quality of life associated with erectile dysfunction, the presence of proximal pathological venous drainage from the corpora cavernosa of the penis, lack of sufficient effect from taking of inhibitors phosphodiesterase type 5, refusal to use intracavernosal injections, refusal to undergo penile prosthetics, absence or low quality of visualization of the deep dorsal vein of the penis when using antegrade approaches to the prostatic plexus (transdorsal) as well as the complex anatomy of the pelvic veins when using retrograde approaches to the prostatic plexus (transfemoral and transcubital). The advantage of this technique was safety, extremely low morbidity and minimal hospitalization time.
