Objective: The aim of the study is to assess maternal satisfaction with intrapartum care using the Birth Satisfaction Scale Revised Indicator (BSSRI). Methods: This study employed a quantitative descriptive cross-sectional survey design to evaluate maternal satisfaction with intrapartum care at Murtala Muhammad Specialist Hospital in Kano, Nigeria. The research involved a sample of 203 women who had recently delivered at the hospital. The BSSRI was utilized to measure various dimensions of maternal satisfaction during labor and delivery. Result: The findings of the study revealed that the level of satisfaction with intrapartum care is high, as more than three-quarter (76.8%) of the respondents reported high level of satisfaction with intrapartum care. Parity, educational status, presence of companion during birth, educational qualification, and sex of birth attendant were the identified factors influencing maternal satisfaction with intrapartum care. Other factors reported by the respondents include newborn outcome, functional medical equipment, and encouragement and reassurance by health care personnel. Conclusion: The study concludes that there is a high level of maternal satisfaction with intrapartum care. Parity and educational status, neonatal death, presence of companion during birth, qualification of birth attendant, sex of birth attendant, newborn outcome and willingness to visit and recommend hospital delivery, functional medical equipment, and staff encouragement and reassurance are the identified factors influencing maternal satisfaction with intrapartum care.

BACKGROUND : The definition of natural birth remains nebulous for birthing people. There is a lack of consensus regarding the factors that render a birth experience no longer natural or normal. In the United States, Indigenous birthing people experience some of the highest rates of maternal mortality, morbidity, and mistreatment during childbirth, yet there remains a paucity of research on the root causes of these findings. Some Indigenous birthing people have continued to use midwives throughout the past century, indicating that even before the organized 1970s Indigenous movement to remove any connotation that pathologized birth, individual birthing people were demonstrating their resistance to the medicalization of Indigenous birth practices. Theres a gap in understanding, however, of indigenous birthing people’s use of medication pain relief. We argue that understanding the intricacies of Indigenous birthing people’s notions of and preferences for “natural” birth can guide the development of interventions that increase access to desired options and thereby support autonomy. Moreover, existing literature on birthing people’s preferences and birth outcomes in the United States tends to examine the experiences of American birthing people generally, but to the authors’ knowledge, there remains no research specifically exploring Indigenous preferences regarding the mode of delivery, hospital versus home birth, pain management, and use of midwives. AIM : This study’s aim was to fill this gap, contribute to an understanding of the full scope of North American Indigenous birthing people’s preferences, and catalyze further discussion regarding interventions to increase Indigenous birthing people’s access to the birthing options they need and desire. METHODS : A qualitative descriptive research design was used to investigate and convey the insights of Indigenous birthing people surrounding their birth experiences and desires. A qualitative description is an approach to naturalistic investigation often used in the examination of health topics because of its strength in relaying complex experiences in everyday language. SAMPLE : All adult, women-identifying, self-identified members of a specific state-recognized Indigenous tribe in the Gulf Coast region of the United States were eligible for inclusion in this study. Thirty-one interviews were conducted. DATA ANALYSIS : The interview transcripts were analyzed using qualitative content analysis, which entails categorizing and grouping pieces of a broader data set based on common themes. RESULTS : Three major themes emerged from the analysis of participants’ interviews: (a) beliefs about and desire for a vaginal birth, (b) pain management methods and preferences, and (c) beliefs about and use of midwives. This study found that Indigenous birthing people in the Gulf South have varying definitions of “natural” birth, ranging from home birth to vaginal birth to unmedicated birth to midwife-attended birth.

“If I Had a Choice, I’d Do It Natural”: Gulf South Indigenous Women’s Preferences and Experiences in Childbirth

BACKGROUND : Even though spontaneous abortion (SA) is an important public health concern, most studies in our country were confined to induced abortion. Thus, this study aimed to assess the prevalence and associated factors with SA among reproductive-age women in Benchi Maji, Kefa, and Sheka Zone, southwest Ethiopia. METHODS : An institution-based cross-sectional study was conducted from April 1 to 21, 2021, among 845 reproductive-age women in a public hospital that serves residents of Benchi Maji, Kefa, and Sheka Zones by using two stages simple random sampling technique. Data were collected by interviewer-administered semi-structured questionnaire entered into Epi-data version 4.2 and analyzed by SPSS version 23. Bivariate and multivariable logistic regression analyses were undertaken. The odds ratio at 95% confidence interval (CI) and p -value &lt; 0.05 was used to ascertain statistical significance. RESULTS : Nearly one-tenth (12.31%) of the participants had SA within five years preceding the survey. Being in between 15 and 19 years (AOR 0.102; 95% CI = 0.044–0.235) at ( p -value &lt; .001), being an urban resident (AOR 0.461; 95% CI = 0.255–0.833) at ( p -value = .01), having education level primary or below (AOR 23.5; 95% CI = 6.3–88.3) at ( p -value &lt; .001), having middle income level (AOR 3.89; 95% CI = 1.88–8.08) at ( p -value &lt; .001), and being married (AOR 0.319; 95% CI = 0.186–0.548) at ( p -value = .044) were factors associated with it. CONCLUSION : The prevalence of SA among the participants was within the worldwide average prevalence. Having planned pregnancies before 35 years, minimizing hard labor works, and other risks for SA are beneficial to reduce the prevalence of SA. Thus, health care providers should work on awareness creation toward preventable risks of SA among the study participants.

Spontaneous Abortion and Associated Factors Among Reproductive Age Women in Benchi Maji, Kefa, and Sheka Zone, Southwest Ethiopia: Institution-Based Cross-Sectional Study

BACKGROUND : Ultrasonographic (U/S) evaluation of the fetus, placenta, and amniotic fluid is an integral part of antenatal care (ANC). U/S results confirm fetal viability and gestational age, and multiple gestations. U/S can also provide evidence to rule out fetal abnormalities related to anatomy or blood flow. However, a woman’s knowledge about prenatal U/S is not clear. Therefore, the aim of this study was to assess pregnant women’s knowledge of this prenatal U/S assessment technique. METHODS : An institutional-based cross-sectional study was conducted on 303 pregnant women attending ANC in Jimma Town, Ethiopia, from July to August 2021. Women were interviewed in Jimma town public health care facilities. The sample size was calculated using the single population proportion formula, and samples were taken after proportional allocation was performed for the hospital and health centers using the proportion allocation formula. Data were entered into Epi-data version 3.1.1 and exported into the Statistical Packages for Social Sciences version 21.0 for cleaning and further analysis. Logistic regression analysis was performed to determine the association between the explanatory and response variables. The strength of the association of the dependent and independent variables was presented as crude and adjusted odds ratios at a 95% confidence interval. For the multivariable logistic regression, the level of significance was a p -value of less than 0.05. RESULTS : In this study, 190 (62.7%) of the respondents had a good knowledge of obstetric ultrasound. Residency, primary school (grades 1–8), an education greater than secondary school, and a history of at least one abortion were significantly associated with participants’ knowledge about prenatal ultrasound. CONCLUSIONS : Approximately two-thirds of pregnant women had a good knowledge related to prenatal U/S. However, the proportion with good knowledge in the current study was lower than the World Health Organization recommendation.

Pregnant Women’s Knowledge of Prenatal Ultrasound in Jimma Town Public Health Institutions, Ethiopia

Leukemia in pregnancy is uncommon and may be a fatal condition. Using chemotherapy treatment during pregnancy gives adversity to the mother and fetus, resulting in a dilemma about the proper management plan. A 33-year-old pregnant woman who is G3P2L2 presented at 22 weeks of gestation with on-and-off back pain and progressive pancytopenia. During this admission, she was suspected to have mild pericarditis, yet the echo report was normal. Clinically, she had small multiple axillary lymph node enlargement, and abdominal ultrasound showed mild hepatosplenomegaly. Diagnosis of pre-B acute lymphoblastic leukemia (pre-B ALL) was confirmed by bone marrow biopsy. She was treated with UK-ALL protocol chemotherapeutic regimen (Table 1), considering some modifications in the standard protocol due to pregnancy. She delivered a healthy boy by lower uterine segment cesarean section at 36 weeks of gestation. She was encouraged to breastfeed her baby for only 2 weeks in order to pursue chemotherapy treatment afterward. Chemotherapy management during pregnancy carries more risks to both the mother and the fetus and is more challenging to clinicians; therefore, clinicians need to be aware of early diagnosis of leukemia and have high suspicion, especially during pregnancy.

Acute Lymphoblastic Leukemia in Pregnancy: Pregnant Woman Battled Against Leukemia Until 36 Weeks of Gestation: A Case Report

妊娠期急性淋巴细胞白血病：孕妇与白血病抗争至36周妊娠：一例报告

INTRODUCTION: Globally, studies have reported that many pregnant women often dread the childbirth experience. Its prevalence is higher in developing countries than in the developed countries. It is a problem more common among primigravid women, with consequences for their health and implications for their labor, birth, and postpartum period. Hence, there is a need to assess the level of childbirth fear and birth-related mindset among primigravida attending antenatal clinics in selected hospitals in Ibadan. METHODOLOGY: This study adopted a descriptive cross-sectional design. Two hundred and four primigravid women were selected using a simple random sampling technique. Data were collected with the aid of a questionnaire, and the result was analyzed using SPSS version 25.0. RESULTS: Findings of the study revealed that slightly above half (55.9%) of the respondents had a high level of childbirth fear. Also, majority (52%) of the respondents had a natural birth-related mindset. It further revealed that there is a significant association between age and childbirth fear with a p value of &lt;.001. Also, there is a significant association between childbirth fear and birth-related mindset with a p -value of &lt;.001. CONCLUSION: The study concluded that there is a high level of childbirth fear among primigravida. It is recommended that there should be an increased focus on the fears of women of all ages toward childbirth. This should include providing tailored sensitive interpersonal care to them at all levels of care.

Childbirth Fear and Birth-Related Mindset of First-Time Mothers Who Registered for Antenatal Care in Public Hospitals in Nigeria

Background: Birthing choices play significant roles in maternal outcomes. This article describes the birthing choices made by some women on Mageta Island (hereafter called simply “the Island”) of Western Kenya in relation to Universal Health Coverage (UHC) and free maternity services (FMS) policy ideals. Kenya has officially rolled out UHC and has also initiated universal coverage of maternal healthcare services to reduce maternal morbidities and mortalities. In 2016, the government of Kenya expanded FMS, called the Linda Mama (Taking Care of the Mother) initiative, which targets all pregnant women, newborns, and infants by offering free maternal healthcare services. Despite the existence of UHC and FMS in Kenya, women living on the Island have no access to such services. They therefore devise mechanisms for accessing maternal healthcare services. Design: This study is based on an ethnographic design. Data were collected for 5 months using informal conversations, participant observation, and note-taking with mothers at their homes. The study included interviews with six pregnant mothers who were living on the Island at the time of our interviews. Discussion: It emerged that women relied on their agency to find alternatives to biomedical maternal healthcare services, despite the existence of UHC. Such alternatives included having traditional midwives attend their births in their own homes, migrating to the mainland for proper care, and using kin relations as alternatives for birthing beyond the Island. This article highlights how the implementation of the UHC policy might be tailored to specific local contexts, which could help avert maternal mortalities in hard-to-reach areas such as the Island.

Birthing Choices Made by Pregnant Women on Mageta Island, Western Kenya

AIM: The aim of the study is to evaluate the trend in adverse obstetric outcomes of parturients across the spectrum of body mass index. METHODS: This is a cohort study of pregnant women who received obstetric care at the Obstetrics and Gynecology Department of the Ekiti State University Teaching Hospital, Ado-Ekiti, between January 2015 and December 2015. The data were prospectively collected into the Obstetric Data Registry of the department. Composite adverse maternal outcomes and composite adverse fetal outcomes were obtained for each group. Data were analyzed with SPSS version 24, and a p -value of less than .05 was considered statistically significant. RESULTS: Out of the 1,006 parturients, 334 (33.2%) were obese. Composite adverse maternal outcome increased from 35.1% for women with normal weight to 62.3% for obese women ( p &lt; .001). Composite adverse fetal outcome also increased from 25.9% for women with normal weight to 30% for obese women ( p &lt; .001). The odds of composite adverse maternal outcome were increased in overweight women (adjusted odds ratio [aOR] 1.52, 95% confidence interval [CI] [1.12, 3.49]) and obese women (aOR 1.65, 95% CI [1.21, 2.24]) compared with women with normal weight. Also, the odds of composite adverse fetal outcome were reduced in overweight (aOR 0.64, 95% CI [0.44, 0.93]) but increased in underweight (aOR 4.78, 95% CI [2.00, 11.44]) and obese (aOR 1.16, 95% CI [1.03, 12.39]) women. CONCLUSION: Composite adverse obstetric outcomes were increased in underweight, overweight, and obese women when compared with women with normal weight.

Comparison of Obstetric Outcomes Among Various Weight Strata in a Teaching Hospital in Southwest, Nigeria

Objective: The objective of this article was to gain a deeper understanding and insight into the training and characteristics of childbirth doulas on the maternity care team. The aim was to integrate knowledge obtained from two studies: an integrative review (IR) of doula education programs globally (Study 1) and the qualitative experiences of those training to become a doula (Study 2). Methods: For the IR (Study 1), the modified Whittemore and Knafl (2005) framework and Preferred Reporting Items for Systematic Review and Meta-Analysis guidelines were used. Five electronic databases were searched (CINAHL, ProQuest Education Databases, ERIC, MEDLINE, and Scopus) using doula and education/training as key terms. Nineteen papers were included in the review. For the qualitative study (Study 2), semi-structured interviews with 10 student doulas were analyzed using inductive thematic analysis. Results: The results highlighted ambiguity around the education (review) and scope of practice (qualitative) of a childbirth doula. The review showed a variety in duration, content, learning, certificate/accreditation, and educator qualifications in doula training programs, globally. The variety in student doulas’ understanding of their role in the maternity care context emerged from the key themes: (a) what influenced the decision to become a doula, (b) personal attributes perceived as essential to be a doula, and (c) what constitutes the role of a childbirth doula. Conclusions: This article highlights a variability in doula training legislation and accreditation, as well as program content and length. This variation may be reflected in student doulas’ understanding of their role in the maternity context. A better understanding of doula training and a defined scope of practice can fill the current gap in continuity of care and improve communication within the maternity care health team.
