Abstract Purpose of Review Xenotransplantation represents the most readily available solution to the shortage of donor organs for transplantation. This review seeks to address progress made in xenotransplantation thus far, and forecast barriers and opportunities for eventual clinical translation. Recent Findings Over the past several years, significant progress has been made to advance xenotransplantation to the clinic. Increasingly complex genetic manipulation of the donor source pigs has allowed for prevention of hyperacute rejection and graft function with clinically-available, FDA-approved immunosuppression. The Parsons Model utilizing recently deceased humans as recipients has allowed for the study of xenografts in a pre-clinical human model. These studies have confirmed function of kidney xenografts and maintenance of renal physiology. Two cases of human heart xenotransplantation have been reported, with limited early success. Two human recipients have received porcine kidney grafts. Moving forward, careful recipient selection will be needed for the first clinical trials. Diligent screening of the donor animals will reduce the risk of zoonotic infection, and post-transplant surveillance of recipients will remain important. Summary Studies to date have demonstrated the feasibility of xenotransplantation with promising early results. With additional studies designed to address the remaining unanswered questions, xenotra nplantation may finally realize its potential and become a clinical reality.

Strategies for Liver, Heart, and Lung Viability Assessment While Using Donor In-Situ or Ex-Situ Advanced Perfusion

The purpose of this review was to summarise how machine perfusion could contribute to viability assessment of donor livers.In both hypothermic and normothermic machine perfusion, perfusate transaminase measurement has allowed pretransplant assessment of hepatocellular damage. Hypothermic perfusion permits transplantation of marginal grafts but as yet has not permitted formal viability assessment. Livers undergoing normothermic perfusion have been investigated using parameters similar to those used to evaluate the liver in vivo. Lactate clearance, glucose evolution and pH regulation during normothermic perfusion seem promising measures of viability. In addition, bile chemistry might inform on cholangiocyte viability and the likelihood of post-transplant cholangiopathy.While the use of machine perfusion technology has the potential to reduce and even remove uncertainty regarding liver graft viability, analysis of large datasets, such as those derived from large multicenter trials of machine perfusion, are needed to provide sufficient information to enable viability parameters to be defined and validated .

Abstract Purpose of the Review Recent advancements in tissue engineering and regenerative medicine have paved the way for innovative solutions in skin regeneration, particularly for extensive burns and full-thickness wounds where traditional approaches are limited. The purpose of the review is to explore the integration of bioactive materials, stem cell therapies, and tissue-engineered skin substitutes and their role in revolutionizing wound healing and skin transplantation. Recent Findings Studies leveraging natural and synthetic biomaterials as scaffolds, alongside the regenerative capabilities of mesenchymal stem cells (MSCs) and other cellular therapies, underscore the potential to enhance tissue repair, minimize scarring, and improve overall clinical outcomes. The development of multifunctional biomaterials and the advent of cutting-edge techniques such as 3D bioprinting and nanomedicine further propel the field, offering personalized and effective solutions. As these technologies evolve, they hold promise for more efficient, patient-specific skin grafting, reducing the need for systemic immunosuppression and enhancing graft survival. Summary The critical advancements in biomaterials, stem cell therapies, and tissue engineering, outline a course toward more effective and personalized skin regeneration therapies.
