Background: Breast cancer is the most common malignant tumor affecting women worldwide. Surgery is the main treatment for breast cancer and can obtain a good curative effect. However, it can also entail trauma and stress for patients, leaving them susceptible to negative emotions, poor sleep quality, and various postoperative complications, which can affect the treatment effect and prognosis. Therefore, it is of great significance to explore scientific and effective nursing methods for patients with breast cancer. This study aimed to examine the application effect of whole-process nursing and mind mapping in patients with breast cancer. Methods: The present study enrolled 256 patients with breast cancer who were admitted to the hospital from March 2019 to October 2020. The study participants were divided into a control group (routine nursing intervention) and an observation group (nursing intervention including a whole-process escort playing a relative role combined with mind mapping) according to the random number table method, with 128 cases in each group. At admission and before surgery, the psychological status [Hamilton anxiety scale (HAMA) and Hamilton depression scale (HAMD)], physiological stress [cortisol (Cs) and adrenocorticotrophic hormone (ACTH)], fatigue degree (Piper fatigue scale), and sleep quality [Pittsburgh sleep quality index (PSQI)] were compared between the two groups, and the incidence of complications in the two groups was also compared. Results: The psychological status (HAMA and HAMD) scores, physiological stress levels (Cs and ACTH), fatigue degree (Piper fatigue scale), and sleep quality (PSQI) score were all significantly higher before surgery than at admission in both groups, and were significantly lower in the observation group than in the control group (P<0.05). There were no significant differences in the incidence of complications between the two groups (P>0.05). Conclusions: Nursing intervention using a whole-process escort playing a relative role combined with mind mapping can effectively relieve the anxiety and depression felt by patients undergoing breast cancer surgery, and can reduce their physiological stress and fatigue degree, as well as improve their sleep quality. Nursing intervention using a whole-process escort playing a relative role combined with mind mapping in patients undergoing breast cancer surgery: a randomized trial

Background: Inguinal hernia repair is a common surgical procedure; however, the effects of laparoscopic and open mesh repair in the treatment of recurrent inguinal hernia are unclear.This study aimed to evaluate the effects of laparoscopic and open mesh repair in the treatment of recurrent inguinal hernia by conducting a meta-analysis of randomized controlled trials (RCTs).Methods: A comprehensive, meta-analysis of RCTs on the effects of laparoscopic and open mesh repair in the treatment of recurrent inguinal hernia was performed.English-and Chinese-language studies published up to December 30, 2017 were identified via a systematic search of the PubMed, Embase, Science Direct, Cochrane Central Register of Controlled Trials, China National Knowledge Infrastructure (CNKI), and Wanfang databases.Summary odd ratios or weighted mean differences (MDs) with 95% confidence intervals (CIs) were adopted to calculate each outcome using a fixed or random effects model.Results: Ten RCTs involving a total of 1,017 patients were included.There was no statistically significant difference in the rate of recurrence (P=0.23;OR: 0.74, 95% CI: 0.45-1.21),hematoma (P=0.47;OR: 0.71, 95% CI: 0.28-1.79),urinary retention (P=0.94;OR: 0.97, 95% CI: 0.46-2.07)and acute pain (P=0.71;OR: 0.74, 95% CI: 0.14-3.76)between the laparoscopic and open mesh repair groups.The incision infection rate (P=0.02;OR: 0.28, 95% CI: 0.10-0.81) of the laparoscopic group was lower and the length of hospital stay (P<0.0001;MD: -3.65, 95% CI: -4.76 to -2.53) was significantly shorter than those of the open repair group.However, the laparoscopic group had a longer operative time (P=0.0002;MD: 20.30, 95% CI: 9.60-31.01). Conclusions:The laparoscopic approach is superior to the open mesh approach for the repair of recurrent inguinal hernia in some aspects, including the incision infection rate and length of hospital stay.However, more high-quality studies on the effects of laparoscopic and open mesh repair for the treatment of recurrent inguinal hernia are warranted.

Prevention of platelet transfusion refractoriness by platelet antigen gene matching: a systematic review and meta-analysis

Background: Postoperative gastrointestinal dysfunction (PGD) leading to nausea, vomiting, and abdominal distension are common complications after laparoscopic surgery for abdominal diseases. However, drugs cannot completely stop PGD. Over the years, transcutaneous electrical acupoint stimulation (TEAS) therapy has shown potential in preventing PGD, but there is no medical evidence that TEAS represents the best choice for the treatment of PGD. This network meta-analysis sought to evaluate the effectiveness of TEAS therapy in preventing PGD in patients after abdominal laparoscopic surgery. Methods: Articles (published from the establishment of the databases to July 2021) were retrieved from the following databases: PubMed/Medline, Cochrane Library, Web of Science, Embase, China Biomedical Literature Database (CBM), China Technical Journal VIP Database (CQVIP), China Knowledge Infrastructure (CNKI), and the Wanfang Database. The Cochrane risk of bias tool was used to evaluate the quality of the included studies, and a network meta-analysis was performed using RevMan 5.20. Results: A total of 7 randomized controlled trials, comprising 440 TEAS-treated patients and 468 control patients, were included in the meta-analysis. The meta-analysis showed that there was no significant difference between the TEAS treatment group and the control group in relation to postoperative nausea and vomiting [relative risk (RR) =0.66; 95% confidence interval (CI): 0.37–1.21; P=0.18], postoperative abdominal distension (RR =0.53; 95% CI: 0.40–0.72; P<0.0001), the time of first postoperative fart (imply gastrointestinal motility) [mean difference (MD) =–7.31; 95% CI: –11.33 to –3.30; P=0.0004], and the time of first postoperative bowel movement (MD =–5.28; 95% CI: –7.23 to –3.33; P<0.0001); there were significant differences among these 3 indicators. Discussion: We found that TEAS can promote postoperative fart and bowel movement, and has good clinical value in promoting postoperative gastrointestinal function recovery.

Systematic review and meta-analysis of the effect of transcutaneous electrical acupoint stimulation on gastrointestinal function after laparoscopic surgery

Background: Family caregivers of patients on prolonged mechanical ventilation (PMV) may encounter challenges concerning medical decision-making besides witnessing patient suffering. Palliative care (PC) should be a good support for both patients and caregivers; however, for PMV families, PC is not always a choice through long companion time. This qualitative study clarifies family caregivers' burden of assisting patients on PMV and evaluates the need for PC information and support. Methods: Interviews were caregivers of patients on ventilator support for more than 60 days in five hospitals of the Taipei City Hospital System. Based on phenomenology, this study was conducted by using a semi-structured questionnaire comprising three questions: (I) what was the most crucial moment of deciding to intubate? (II) how would you describe the quality of life of your ventilator-dependent family member? (III) what type of assistance do you expect from the PC team for your ventilator-dependent family member? Results: Twenty-one caregivers of patients on PMV in five hospitals of the Taipei City Hospital System agreed to participate in face-to-face interviews. The identified themes, including stressful decision-making, companion pain/discomfort, and unwillingness to accept PC, elucidated the difficulties experienced by caregivers when providing care. Conclusions: Understanding family caregivers' experiences can enable physicians to improve communication with them, encourage the PC team to support them during surrogate decision-making for patients on PMV during critical moments, and enhance the overall PC service.

Stepped palliative care: considerations for equitable implementation

Hepatobiliary malignancies (HBMs), primarily hepatocellular carcinoma (HCC) and cholangiocarcinoma (CCA), share the common traits of having a generally poor prognosis, late presentation, and high symptom burden related to both the disease process itself and underlying poor liver function. The incidences of both malignancies have been rising in recent decades for unclear reasons. Curative options remain limited given the general aggressive disease course despite advances in diagnosis, therapies, and surgery. Early integration of palliative care into the routine care of patients with HBMs is an essential, but underutilized, component of care to improve the functional and symptomatic quality of the lives of patients and their families. While formal guidelines for its integration are currently lacking, palliative care can and should be provided in parallel to disease specific care at any stage to address the physical, emotional, and spiritual needs of patients with HBMs. In this review, the special needs of this patient population are examined ranging from early symptom management at the time of diagnosis all the way through to end-oflife care. Key barriers that prevent the early provision of palliative care for patients with HBMs are identified and discussed and recommendations provided on how to improve early integration.
