The purpose of this study was to evaluate the efficacy of bone ring augmentation technique for three-dimensional augmentation of severely defective sockets along with placement of immediate dental implants.A clinical study was conducted on 14 patients with 15 defective sockets requiring extractions and immediate implant placement. Following extraction, bone ring with an implant osteotomy, harvested from chin, was sculptured and fitted into the extraction socket. The bone apical to the socket was prepared for implant through the central osteotomy of the ring. Implant was then placed through the ring into the apical bone. After 6 months, prosthesis was placed. Clinical and radiographic examination was done to evaluate the soft tissue and crestal bone level around implants immediate postoperatively and at 1st, 3rd, 6th, and 9th month postoperatively.Out of 15 grafted sockets, 14 showed an evidence of bone healing with no significant crestal bone resorption. One ring showed soft tissue dehiscence which underwent severe resorption at 6th month postoperative visit.The bone ring augmentation technique is an effective method for three-dimensional augmentation of severely defective sockets. This technique helps in reducing the overall treatment time by allowing grafting and placement of implant simultaneously in a single visit. However, longer observation periods are needed to draw more definite conclusions on the success of bone ring augmentation technique.

Bone ring augmentation around immediate implants: A clinical and radiographic study

Abstract Introduction: Closure of palatal fistula is difficult due to excessive scarring of the palate after previous surgery. The facial artery musculomucosal (FAMM) flap is considered a better option for palatal fistula closure due to its axial blood supply and minimal donor site morbidity. The aim of this study was to evaluate the results of harvesting FAMM flap for closure of medium and large palatal fistulas in terms of its versatility, reliability, feeding and speech assessment. Materials and Methods: A retrospective analysis was performed on patients with post-palatoplasty fistula admitted to the outpatient clinics of the Department of Plastic Surgery and the Department of Maxillofacial Surgery of Assiut University Hospital from March 2022 to February 2024. Results: Twenty patients aged 3–11 years, with female:male = 3:2 ratio, were included in the study. Two flaps experienced venous congestion and then partial flap necrosis. In addition, two flaps had wound dehiscence with mild infection. One patient experienced complete flap loss and then a late recurrent fistula. Most patients were able to feed well, except one patient was unable to swallow and had nasal regurgitation of fluids, that was relieved after second surgery by dorsally based tongue flap. Most patients had intelligible speech during verbal communication, only one patient had nasal tone and hypernasality and two patients were unable to communicate verbally. Conclusion: The FAMM flap is suitable for closing medium and large fistulas due to its versatility and vascular reliability. It has fewer complications and sufficient functional and aesthetic results.

Outcomes of Facial Artery Musculomucosal Flap in Repair of Post-palatoplasty Fistula - A Retrospective Study

Abstract Introduction: Surgical removal of lower third molar is one of the most common surgical procedures and is quite stressful for many patients. In this study, two different routes of administration of dexamethasone 8 mg (intravenous [IV] vs. submucosally infiltrated) were used to evaluate the role of dexamethasone in reducing the post-operative inflammatory sequelae following lower third molar removal. Materials and Methods: Sixty patients who had to undergo surgical removal of mandibular third molars were randomly divided into two groups, each group consisting of 30 patients. One group of patients was administered 8 mg dexamethasone submucosally five min before the surgery. Another group of patients received 8 mg dexamethasone intravenously five min before the surgery. Facial swelling and pain were measured on the 2 nd , 4 th and 7 th post-operative days. Results: The results of this study revealed that both the routes were effective in controlling post-operative pain and swelling. IV route of dexamethasone showed higher efficacy compared to submucosal (SM) route of dexamethasone in reducing the post-operative inflammatory sequelae in the surgical removal of impacted lower third molar teeth. Discussion: It was seen that IV dexamethasone (8 mg) and submucosal dexamethasone (8 mg) had equivalent ratings in terms of reduction of swelling and pain. Although the results of this study showed both the routes are effective in controlling post-operative swelling and pain after third molar surgery, the study concluded that certain benefits of submucosal route make the sm route to be a valuable alternative to iv dexamethasone.

Abstract Rationale: This case report highlights the essential role of dental and medical professionals in the early diagnosis of Gardner’s syndrome (GS). Patient Concerns: A 21-year-old patient presented with painless, progressively enlarging swelling on both sides of the face, which had developed over the past few years. Diagnosis: Imaging, including a panoramic image and multislice computed tomography (CT) scan, revealed multiple osteomas. The patient was referred to gastroenterology, where endoscopy and colonoscopy identified multiple adenomatous polyps. Interventions: The patient subsequently underwent facial aesthetic corrections. Outcomes: The patient is currently undergoing regular gastrointestinal monitoring and managing colon polyps, with follow-up appointments scheduled annually. Take-away Lessons: To improve the prognosis, this report emphasises the vital role of dental and medical professionals in the early detection of GS, advocating for a multidisciplinary preventive approach.

Gardner’s Syndrome: Vigilance Better than Negligence - A Case Report

Abstract Background: This study was conducted with the aim to systematically review the efficacy of autofluorescence in the detection of tobacco-associated oral mucosal lesions. Objectives and Data Sources: Clinical studies with patients that focused on the autofluorescence method in the detection, visualisation, or management of tobacco-associated oral mucosal lesions were included in the literature search across bibliographic databases. Results: Autofluorescence visualisation showed positive results, with high sensitivity scores when compared with the gold standard of histopathology in the clinical evaluation of common tobacco-associated oral mucosal lesions. However, the low specificity values indicate its limited ability to distinguish between dysplasia and other benign conditions. Conclusions and Implications: This review provides promising evidence for the application of autofluorescence in the detection of tobacco-associated oral mucosal lesions where autofluorescence is used in conjunction with conventional examination with the gold standard of histopathological examination. Efficacy of Autofluorescence in Detection of Tobacco-associated Oral Mucosal Lesions - A Systematic Review

The field of craniofacial surgery has witnessed remarkable advancements in recent years, driven by groundbreaking research and technological innovations. Among the most promising developments is the exploration of inorganic materials for tissue regeneration and repair. In recent groundbreaking studies published in Acta Biomaterialia and Advanced Science, researchers from Texas A&M University have unveiled the potential of inorganic ions and nanosilicates to stimulate bone and cartilage formation in stem cells. These findings represent a significant departure from traditional approaches that rely on organic or biologically derived molecules. By harnessing the natural healing properties of inorganic materials, these researchers have opened the door to a new era of regenerative medicine.[1] The ability to induce natural bone formation has profound implications for craniofacial surgery. It offers the promise of improved treatment outcomes, reduced patient recovery times and a decreased reliance on invasive procedures and long-term medications. For patients with conditions such as osteoporosis, enhancing bone density can mitigate the risk of fractures, improve quality of life and reduce healthcare costs. Furthermore, the application of inorganic biomaterials in conjunction with three-dimensional bioprinting techniques holds immense potential for the creation of customised bone implants. This approach could revolutionise reconstructive surgery, particularly for craniofacial defects, by restoring both function and appearance. A joint team from the University of Geneva and Beihang University has identified the FOX13 gene as a critical factor in ear development. Mutations in this gene can lead to Goldenhar syndrome, a rare congenital disorder characterised by facial asymmetry, malformations of the auditory and ocular systems and spinal column anomalies. This discovery provides valuable insights into the genetic mechanisms underlying ear development and paves the way for targeted therapies for this condition in future.[2] Inspired by the remarkable ability of young infants to regenerate bone defects in their skulls, researchers at the University of Pittsburgh have developed a novel technique to stimulate bone regeneration in adults. By carefully stretching the skull along its sutures, they were able to activate skeletal stem cells and promote bone healing. This approach offers hope for patients with skull damage caused by trauma, congenital defects or surgery.[3] As we continue to explore the frontiers of craniofacial surgery, these findings offer a beacon of hope for patients and a testament to the power of scientific discovery. Science and necessity is the mother of invention.
