Contemporary Prevalence of Gout and Hyperuricemia in the United States and Decadal Trends: The National Health and Nutrition Examination Survey, 2007–2016

Objective To investigate the role of the interleukin‐25 ( IL ‐25)/ IL ‐17 receptor B ( IL ‐17 RB ) axis in experimental Sjögren's syndrome ( SS ) and in patients with primary SS and primary SS –associated lymphoma. Methods Expression of IL ‐25, IL ‐17 RB , IL ‐17B, and tumor necrosis factor receptor–associated factor 6 ( TRAF 6) was analyzed on minor salivary gland ( SG ) samples from patients with primary SS and on parotid gland samples from patients with primary SS –associated B cell non‐Hodgkin's lymphoma ( NHL ). IL ‐17 RB expression and the frequencies of natural group 2 innate lymphoid cells ( ILC 2s), inflammatory ILC 2s, and M2‐polarized macrophages were assessed by flow cytometry in SG mononuclear cells and peripheral blood mononuclear cells ( PBMC s). Tissue distribution of ILC 2s was studied by confocal microscopy. The role of recombinant IL ‐25 and of rituximab in modulating IL ‐25 expression was investigated in in vitro studies. IL ‐25/ IL ‐17 RB and TRAF 6 expression and the role of IL ‐25 inhibition were also studied in the experimental murine model of SS . Results Activation of the IL ‐25/ IL ‐17 RB / TRAF 6 axis correlated with the focus score and was observed in patients with primary SS and in patients with primary SS –associated NHL . A significant increase in the frequency of inflammatory ILC 2s was observed both in SG mononuclear cells and in PBMC s. IL ‐25 stimulation of isolated SG mononuclear cells and PBMC s from patients and controls resulted both in inflammatory ILC 2 expansion and in increased autoantibody production. Rituximab modulated expression of inflammatory ILC 2s and IL ‐25 in primary SS . SG protein–immunized mice developed overt SS symptoms with increased IL ‐25 expression and increased frequency of CD 4+ IL ‐17 RB + TRAF 6+ cells. IL ‐25 neutralization attenuated disease progression and tissue pathology in mice with experimental SS . Conclusion IL ‐25 may promote the inflammatory state in primary SS and may be a potential target for novel disease‐modifying therapeutic strategies in patients with primary SS .

Interleukin‐25 Axis Is Involved in the Pathogenesis of Human Primary and Experimental Murine Sjögren's Syndrome

This study was undertaken to identify key disease pathways driving conventional dendritic cell (cDC) alterations in systemic sclerosis (SSc).Transcriptomic profiling was performed on peripheral blood CD1c+ cDCs (cDC2s) isolated from 12 healthy donors and 48 patients with SSc, including all major disease subtypes. We performed differential expression analysis for the different SSc subtypes and healthy donors to uncover genes dysregulated in SSc. To identify biologically relevant pathways, we built a gene coexpression network using weighted gene correlation network analysis. We validated the role of key transcriptional regulators using chromatin immunoprecipitation (ChIP) sequencing and in vitro functional assays.We identified 17 modules of coexpressed genes in cDCs that correlated with SSc subtypes and key clinical traits, including autoantibodies, skin score, and occurrence of interstitial lung disease. A module of immunoregulatory genes was markedly down-regulated in patients with the diffuse SSc subtype characterized by severe fibrosis. Transcriptional regulatory network analysis performed on this module predicted nuclear receptor 4A (NR4A) subfamily genes (NR4A1, NR4A2, NR4A3) as the key transcriptional regulators of inflammation. Indeed, ChIP-sequencing analysis indicated that these NR4A members target numerous differentially expressed genes in SSc cDC2s. Inclusion of NR4A receptor agonists in culture-based experiments provided functional proof that dysregulation of NR4As affects cytokine production by cDC2s and modulates downstream T cell activation.NR4A1, NR4A2, and NR4A3 are important regulators of immunosuppressive and fibrosis-associated pathways in SSc cDCs. Thus, the NR4A family represents novel potential targets to restore cDC homeostasis in SSc.

Ectopic lymphoid structures (ELS) develop at sites of infection, autoimmunity, and cancer. In patients with Sjögren's syndrome (SS), ELS support autoreactive B cell activation and lymphomagenesis. Interleukin-27 (IL-27) is a key regulator of adaptive immunity and limits Th17 cell-driven pathology. We undertook this study to elucidate the role of IL-27 in ELS formation and function in autoimmunity using a murine model of sialadenitis and in patients with SS.

Short‐term risk of cardiovascular events in people newly diagnosed with gout: comment on the article by Cipolletta et al.

Objective In rheumatoid arthritis (RA) and spondyloarthritis (SpA), managing persistent pain remains challenging. Little is known regarding impaired pain pathways in these patients and the impact of bDMARDs. The objective of the RAPID (Rheumatism Pain Inhibitory Descending pathways) study was to assess pain thresholds and descending pain modulation in patients with active RA or SpA following introduction of a TNF inhibitor. (TNFi) Methods Patients with active disease (50RA and 50SpA), naïve to bDMARD/tsDMARD and starting a TNFi were included. Patients were followed for 6 months after TNFi initiation with clinical, psychological, and pain assessment. At all visits, participants underwent quantitative sensory testing with heat and cold pain thresholds (HPT‐CPT) and descending inhibition by conditioned pain modulation (CPM). Descending pain control (CPM effect) was assessed as the change in HPT (°C) following a conditioning stimulus. Results Of the 100 patients (59 women, mean age 45.8 ±14.6years), 74 completed the 6‐month follow‐up. Thermal pain thresholds did not significantly change during follow‐up. CPM effect improved significantly during follow‐up: 0.25 ±2.57°C (baseline) and 2.96 ±2.50°C at 6months (p&lt;0.001). At the end of follow‐up, mean CPM effect was significantly higher in patients without significant pain compared with patients with persistent pain &gt;3/10 (3.25°C ±2.68 vs 2.47°C ±2.11; p=0.04) and in patients achieving remission/low disease activity compared with patients with active rheumatism (3.31°C ±2.68 vs 2.18°C ±1.87; p=0.01). Conclusion In active inflammatory rheumatisms, impaired descending pain modulation, but not thermal pain thresholds, is improved after TNFi treatment, suggesting a possible effect of TNFi on central pain modulation.

Changes in descending pain modulation during anti‐<scp>TNF</scp> therapy. A prospective study in rheumatoid arthritis and spondyloarthritis

Objective People with axial spondyloarthritis (axSpA) have increased fracture risk relative to the general population, possibly related to chronic inflammation. We assessed the impact of treatment with tumor necrosis factor inhibitors (TNFi) and non‐biologic conventional synthetic disease‐modifying antirheumatic drugs (csDMARDs) on hip and spine fractures in axSpA, relative to nonsteroidal anti‐inflammatory drugs (NSAIDs). Methods We conducted a nested case‐control study using 2006‐2021 data from the Merative TM MarketScan® Database. We included adults 18‐65 years old with ≥1 inpatient or ≥2 outpatient axSpA ICD‐9 or 10 diagnosis codes separated by ≥7 days. The primary outcome was hip and/or spine fracture, defined by ICD‐9 or 10 diagnosis or procedure codes. For each fracture case, we selected up to 10 controls without fracture. We evaluated medication exposure (TNFi, csDMARDs, NSAIDs [referent], or none) hierarchically using pharmacy claims and procedure codes. We assessed the relation of medication exposure with hip and spine fracture risk using unconditional logistic regression with confounder adjustment. Results Our main analysis included 13,519 individuals with axSpA, comprising 1,229 cases and 12,290 controls. Individuals on TNFi had 29% lower odds of fracture compared to those on NSAIDs (OR 0.71, 95% CI 0.59‐0.85), accounting for age, sex, and diagnosis year. Results were similar in the fully adjusted model (OR 0.75, 95% CI 0.62‐0.91) and when stratified by sex. Conclusion Using a large US insurance claims database, we found evidence for a protective effect of TNFi on fracture risk in axSpA underscoring a potential impact of TNFi in diminishing comorbidities linked with axSpA.
