The objective of this study is to investigate the clinical characteristics and treatment of patients with early-onset gout.We retrospectively reviewed 327 adult patients with a first diagnosis of gout from 2008 to 2016 using the database of a multispecialty group practice in New England. Patients were classified into the following groups: age 30 years or younger at first diagnosis (group 1), age 31-40 years (group 2), and age over 40 years (group 3). The clinical characteristics and treatment of gout were compared among the three groups.We identified 87 patients in group 1 and 140 patients in group 2. Group 3 included 100 patients randomly chosen from the 7216 patients with a first diagnosis at age over 40 years. Patients within group 1 had significantly higher serum uric acid (sUA) levels at the time of diagnosis and a more prominent family history of gout. Younger patients (groups 1 and 2) had a significantly higher body mass index than patients over 40 years of age (group 3). A substantial number of younger patients also had hypertension or hyperlipidemia. The majority of younger patients met the 2012 American College of Rheumatology (ACR) guidelines for initiating urate-lowering therapy (ULT) on the basis of frequency of gout attacks, whereas the majority of patients over 40 years of age met the guidelines for ULT on the basis of chronic kidney disease. Patients over 40 years of age were more likely to achieve an sUA level less than 6.0 mg/dl.Patients with a first diagnosis of gout at age 40 years or younger frequently had cardiovascular risk factors and were less likely to achieve an sUA level less than 6.0 mg/dl compared with patients over 40 years of age who were treated in routine clinical practice. Clinicians should be aware that patients with early-onset gout may be an undertreated population with poor adherence to ULT and increased risk of recurrent gout and cardiovascular diseases.

Characteristics That Predict Psoriatic Arthritis by the Classification Criteria for Psoriatic Arthritis in Patients With Juvenile Idiopathic Arthritis 18 Years After Disease Onset

Objective Axial spondyloarthritis (axSpA) is an immune‐mediated disease that predominantly affects the axial skeleton. The objective of this study was to describe the frequency and clinical characteristics of axSpA in Black Americans in Shelby County. We also aimed to report diagnostic delays, disease severity, and health disparities in this population. Methods This was a retrospective observational study in which electronic records of patients with axSpA were reviewed across five centers in Shelby County. Patients were identified using International Classification of Diseases codes. Data on demographics, clinical characteristics, and imaging were collected. The frequency was reported. Two‐group comparisons were done using the chi‐square test and two‐sample t ‐test. Multivariable logistic and linear regression methods were used to compare sacroiliitis grades and delay in diagnosis, respectively, after adjusting for confounders. Results Black Americans constituted 32% (n = 78) of the 244 patients with axSpA identified. Black Americans had significantly more hip involvement, more elevations in C‐reactive protein, less HLA–B27 positivity, and less family history positivity when compared with White patients. After adjusting for age, sex, and HLA–B27 , Black Americans had twice as higher odds of having advanced grades of sacroiliitis on x‐rays compared to White Americans (odds ratio 2.32 [95% confidence interval 1.23–4.44]). AxSpA was associated with significant diagnostic delays in both races. Conclusion A significant proportion of patients with axSpA in Shelby County were Black Americans. The study identified that Black Americans have more odds of having advanced sacroiliitis on x‐rays, more hip involvement, and higher markers of inflammation. It also showed that, regardless of race or ethnicity, there is a significant diagnostic delay.

Axial Spondyloarthritis in Black Americans: An Observational Study From Five Centers in Shelby County, Tennessee

Objective Autoantibodies serve as essential clinical biomarkers and may indicate etiological mechanisms in rheumatic diseases. In light of the increasing knowledge concerning the diversity and biologic implications of anti–citrullinated peptide/protein antibodies (ACPAs), we have re‐evaluated the association between the ACPA response and the HLA‐DRB1 allelic groups, known to represent a major genetic risk factor for rheumatoid arthritis (RA). Methods We explored a collection of 4,392 well‐characterized incident patients with RA of White European descent from the Epidemiological Investigation of Rheumatoid Arthritis (EIRA) new‐onset RA study, as well as 1,199 cases of patients with RA of Southeast Asian origin from the Malaysian EIRA study. We focused on a quantitative analysis of the levels of anti–cyclic citrullinated peptide IgG antibodies, including those falling below the diagnostic threshold. Results Our data show that non‐shared epitope alleles HLA‐DRB1*09 and *15 exhibit significant associations with ACPA levels. Notably, these novel associations were independent of ethnicity. To validate our findings, we conducted an additional replication study in an independent pool of 4,109 patients with RA of White European origin. Conclusion These results indicate a new, previously overlooked, role for the HLA locus in the regulation of the levels of ACPA RA‐specific autoantibodies that goes beyond the shared epitope‐defined gene variants.
