The objective of this study is to investigate the clinical characteristics and treatment of patients with early-onset gout.We retrospectively reviewed 327 adult patients with a first diagnosis of gout from 2008 to 2016 using the database of a multispecialty group practice in New England. Patients were classified into the following groups: age 30 years or younger at first diagnosis (group 1), age 31-40 years (group 2), and age over 40 years (group 3). The clinical characteristics and treatment of gout were compared among the three groups.We identified 87 patients in group 1 and 140 patients in group 2. Group 3 included 100 patients randomly chosen from the 7216 patients with a first diagnosis at age over 40 years. Patients within group 1 had significantly higher serum uric acid (sUA) levels at the time of diagnosis and a more prominent family history of gout. Younger patients (groups 1 and 2) had a significantly higher body mass index than patients over 40 years of age (group 3). A substantial number of younger patients also had hypertension or hyperlipidemia. The majority of younger patients met the 2012 American College of Rheumatology (ACR) guidelines for initiating urate-lowering therapy (ULT) on the basis of frequency of gout attacks, whereas the majority of patients over 40 years of age met the guidelines for ULT on the basis of chronic kidney disease. Patients over 40 years of age were more likely to achieve an sUA level less than 6.0 mg/dl.Patients with a first diagnosis of gout at age 40 years or younger frequently had cardiovascular risk factors and were less likely to achieve an sUA level less than 6.0 mg/dl compared with patients over 40 years of age who were treated in routine clinical practice. Clinicians should be aware that patients with early-onset gout may be an undertreated population with poor adherence to ULT and increased risk of recurrent gout and cardiovascular diseases.

Clinical Characteristics of Early‐Onset Gout in Outpatient Setting

Objective Electronic health applications (apps) allow patients with rheumatoid arthritis (RA) to report patient‐reported outcomes (PROs) between visits. However, almost no data exist on patients’ report of pain between visits and how that correlates with change in function and/or modifications to medication. Methods Patients with RA from one rheumatology practice provided data as part of a study of an app. Patients answered PRO short forms using the app, including the Patient‐Reported Outcomes Measurement Information System pain interference and physical function questionnaires. Each PRO was answered every eight days. We assessed the frequency of significant worsening in pain using the minimally clinically important difference (MCID) of ≥5 points. Logistic regression assessed whether worsening in pain was associated with functional declines and/or RA medication changes. Results Among 150 patients with RA using the app, 83 patients qualified for analysis; the mean age was 62 years, and 82% were female. The median number of pain interference PROs answered per patient was 37 over 52 weeks. Using the first five pain interference PRO responses as the baseline, 60 (72%) patients experienced worsening of at least the MCID, with a median duration of 16 days. The odds ratio (OR) relating significantly worsening pain to declines in function over the next 42 days was 2.71 (95% confidence interval [CI] 1.12–6.89). Worsening pain was not significantly associated with changes to RA medication (OR 1.59, 95% CI 0.62–4.17). Conclusion Clinically significant increases in pain among patients with RA are commonly associated with reductions in function but not with RA medication changes.

Longitudinal Assessment of Pain in Rheumatoid Arthritis: Increases in Pain and Their Relationship to Functional Declines and Medication Changes

Objective Prognostic factors associated with medication discontinuation in children with juvenile dermatomyositis (JDM) remain largely elusive. We aim to identify the predictors of medication‐free remission (MFR) in children with JDM. Methods In this retrospective study, patients diagnosed with JDM according to Peter &amp; Bohan criteria and followed for ≥18 months at a tertiary care center from 2006 through 2022 were included. Data extracted included demographics, physical examination, laboratory results, and medications. MFR was defined as inactive JDM after discontinuation of all systemic immunosuppressives for ≥6 months, in line with international consensus guidelines for trials of therapies in idiopathic inflammatory myopathies. A two‐sided P &lt; 0.05 was considered statistically significant. Results Of 55 patients with JDM (63.6% female, age median [interquartile range (IQR)] 6 [3.5–12] years), 29 (52.7%) achieved MFR after a median (IQR) of 33 (22.5–55.2) months. MFR was more common in those who were younger at JDM diagnosis (median 5 vs 8 years, P = 0.008), had early resolution of disease activity (median 11 vs 18 months, P &lt; 0.001), and presented with Gottron papules (χ 2 = 5.25; P = 0.022) and elevated lactate dehydrogenase (χ 2 = 4.82, P = 0.028). Diagnosis of JDM before 5 years old (odds ratio 4.5, 95% confidence interval [CI] 1.2–16.7) was the only predictor of MFR in our multivariate model (area under the curve 0.65, 95% CI 0.53–0.76). Conclusion Half of our patients with JDM achieved MFR. Age at JDM diagnosis may be an important predictor of achieving MFR.
