Background: Resection is the only potentially curative treatment for pancreatic cancer. While previous studies have demonstrated outcome disparities at low volume facilities and in underinsured patients, few have evaluated institutional variables in a large sample using recent data reflective of our rapidly changing healthcare system. We investigated the impact of facility academic status, case volume, and insurance status on length of stay, and 30- and 90-day mortality. Methods: Data were retrieved from the National Cancer Database for 34,718 pancreatic cancer patients who underwent pancreatectomy between 01/01/2010–12/31/2015. Facilities were classified as “Very Low Volume” (≤5 surgical cases/year), “Low Volume” (6–16 cases/year), or “High Volume” (>16 cases/year). Multivariable logistic regression was used to investigate associations between facility or insurance factors and 30- and 90-day mortality. Results: Insurance status was the strongest predictor of positive outcomes, with privately insured patients demonstrating the shortest length of stay (9.6 days) and lower 30-day mortality [OR (95% CI) 0.61 (0.50, 0.74)] and 90-day mortality [OR (95% CI) 0.68 (0.60, 0.78)] than publicly insured patients (P<0.001). High-volume facilities displayed lower 30-day mortality [OR (95% CI) 0.60 (0.49, 0.72)] and 90-day mortality [OR (95% CI) 0.68 (0.60, 0.78)] than very low volume facilities (P<0.001). Compared to non-academic programs, academic programs displayed lower 90-day mortality [OR (95% CI) 0.84 (0.75, 0.94), P=0.002], but equivalent 30-day mortality. Conclusions: These data illustrate the persistent outcome gap affecting underserved or underinsured patients with pancreatic cancer despite efforts in quality improvement and healthcare reform.

Fluoropyrimidine-based maintenance chemotherapy after FOLFIRINOX for patients with metastatic pancreatic adenocarcinoma: a retrospective analysis

Background: Pancreatic cancer (PC) is a lethal form of cancer; surgical excision is the sole opportunity for achieving a cure. Contrast-enhanced computed tomography (CECT) is considered as the main imaging method for evaluating resectability. Endoscopic ultrasound (EUS) is valuable for gathering further data especially when there is uncertainty regarding blood vessel or lymph node (LN) involvement. The current work aimed to determine the precision of EUS compared to CECT for assessment of the resectability status of pancreatic and peri-ampullary carcinomas. Methods: This cross-sectional study included 25 patients who were diagnosed as pancreatic or periampullary cancer by CECT. All patients underwent EUS to evaluate vascular involvement before performing surgical resection. Results: EUS was better than CECT in staging of both pancreatic and peri-ampullary cancers. There was a significant difference in favour of EUS for detection of vascular and LN involvement (P=0.02 and 0.04 respectively). Moreover, EUS changed the resectability status of three patients who were diagnosed as resectable by CECT to being borderline resectable and these findings were confirmed during surgery. EUS demonstrated sensitivity, specificity, and accuracy of 70%, 93.3%, and 84%, respectively for venous invasion and 66.7% sensitivity, 90.9% specificity and an accuracy of 88% for arterial invasion. Conclusions: EUS is a highly successful method for evaluating vascular invasion in PC, with high levels of sensitivity, specificity, and accuracy. EUS findings correlated well with operative findings during surgery thus making it a better tool for assessment of resectability of these cancers, with the additional benefit of providing tissue samples.

Effects of treatment delays on outcomes in pancreatic adenocarcinoma

Background: Pancreatic cancer staging is used to predict prognosis accurately in early stages of disease, however, stage IV with fewer treatment options, is harder to define an accurate life expectancy. Various machine learning methods have been used to improve predictive accuracy of survival. Markov chains are another way to mathematically model a sequence of probability vectors or eigenvalues and could provide a simple yet accurate method for predicting pancreatic cancer survival. The aim of this investigation was to use matrices, eigenvalues and Markov chains to predict survival rates in pancreatic cancer patients based on stage, particularly stage IV. Methods: Matrices and eigenvalues/eigenvectors were used to create transition coefficients that were subsequently feed into the Markov chain and modelling. Outcomes were compared to literature values and decision tree analysis. Results: For all pancreatic patients, 4-week survival is 85% at the time of diagnosis using Markov modelling. The Markov modelling revealed that, for those with advanced disease (stage IV) at presentation, 4-week mortality is 13.3% for those where treatment is undertaken and 21.3% where treatment is not an option. Matched pairs t-test revealed that Markov modelling had a 0.798 correlation coefficient compared to decision tree analysis (R2=0.637) and similarly 0.804 with the published literature (R2=0.647). Conclusions: The decision tree analysis provided modelling of survival at a more granular level and as a result, would be more suitable than Markov modelling or current models based on regression analysis for predicting survival for patients and their families.
