Ultrasound imaging has long been playing a central role in detecting abdominal aortic aneurysms (AAAs). With a recent trend of reducing prevalence of AAAs, ultrasound screening is only recommended for men aged 65 to 75 years with previous smoking history, and a national level of a screening program for women is currently not recommended in the US. In the 2000s, several research groups demonstrated the utility of finite element stress analysis using patient-specific images, which was promising for an accurate assessment of the rupture risk, but physical models remain to be enhanced by considering patient variability and multi-physical characteristics. This review aims to provide a survey of emerging and alternative technologies and new methodologies, such as personalized medicine and data-driven approaches, that may make potential breakthroughs on detection of small AAAs, monitoring of patients during the follow-ups, prediction of AAA growth, assessment of the rupture risk, and post-surgical prognosis for AAA patient management.

Lobar sliding is an often-overlooked aspect of lung mechanics that presents a potential source of variance in the lung mechanics and physiology of different subjects. The goal of this study was to develop a finite element model suitable for studying lobar sliding in the lungs to gain insights about the extent to which too little or too much lobar sliding impacts breathing. We focused on the left lung as it has a single lobar fissure. The model geometry was derived from whole lung and lobar segmentations of a CT image at end inhalation and was divided into three components: a thoracic cavity outer shell, a volumetric upper lobe, and a volumetric lower lobe. Deformation of the lung tissue was driven by non-zero displacement boundary conditions applied to the thoracic cavity shell to simulate exhalation and contact constraints between each of the three geometric components. To verify the numerical methods used in this contact mechanics model, we replicated a contact mechanics benchmark problem from the literature. Further, numerical simulations were performed to determine the optimal contact mechanics parameters for the lung model. The resulting model displacement field exhibited discontinuities at the lobar fissure indicative of lobar sliding and had an average error of 2.3 mm when compared to displacements between anatomical landmarks on the CT images, which is consistent with lung simulation models in literature. The developed finite element contact mechanics model of the lung is novel in the lung mechanics field and may be interrogated to gain insights about the role of lobar sliding in breathing mechanics.

Most cardiac diseases affect the left ventricle (LV), with its treatment being the focus of the majority of cardiovascular medical therapies and mechanical support. Many diseases also affect the right ventricle (RV) and pulmonary circulation, either directly or from secondary effects on the LV and systemic circulation. The development of therapies to treat RV failure is critical for the management of many patients with advanced heart and lung disease, but models to develop mechanical therapies to target the RV are currently lacking. While the RV and pulmonary vasculature have many similarities to the LV and systemic vasculature, there are notable differences between them at a morphological and functional level. This results in distinct patterns of coupling between the RV and pulmonary circulation. Models such as the Windkessel use lumped parameters that characterize the mechanical properties of the entire RV and pulmonary circulation. Contemporary computational approaches that have been used to model the pulmonary vasculature rely on patient-derived anatomy for the proximal pulmonary arteries and other approaches, such as fractal-based models for the distal vasculature. At this time, these models are still being optimized and significantly more work will be required to fully implement them and test their utility across multiple patient-derived anatomies. The development of these models for the RV and pulmonary circulation will be critical to address the unmet need of mechanical therapies for disease of the RV and pulmonary circulation.
